Employee was assisting with the movement of machinery, with hook, clevis and chain attached to a track hoe. Clevis did not fit, so they were changing it out with another hook and clevis located in the track hoe. Track hoe operator tossed the hook from the excavator to the ground. EE tossed the clevis moments later, but employee reached for hook, and then clevis hit EE's hand.

Employee was cutting 2x4 lumber to make crates for finished stone. The lumber ""kicked"" from the circular table saw, hitting the left thumb.

Struck by rolling or sliding object

Left foot to shin got stuck in mud as employee was assisting in the mobilization of quarry saw track. The track moved but employee could not and track hit the knee.

Fall to the walkway or working surface

Employee left EE's work station to shut off a piece of equipment. Slipped on a spot of oil on floor and fell.

Over-exertion in wielding or throwing objects

Employee was working to remove slabs from a Hilmer Gang Saw machine. While using a crow bar to separate the slab material, the crow bar slipped causing the employee to strain their back. Employee continued working until job was completed.

Struck by flying object

Employee working on airline trying to tighten rusted fitting to prevent leaking. Fitting blew out of housing on main plant air feed line injuring hands. Employee has impact burns and small fragments of shrapnel in hands.

Employee operating stationary belt saw when belt jammed in rock. Tried to wedge and pry rock apart. While hitting wedge, chunk of rock/marble broke loose and struck employee in shin. Required 5 stitches.

Employee was removing first slab from a block cut on belt using a crane and pry bar. The pry bar slipped causing employee to loose balance and fall against another block behind him. A slabs sharp edge came in contact with arm causing a laceration.

EE removing rusty bolts from piece of eq. by means of drilling. He used punch to indent bolt. He struck punch it fragmented and struck left earlobe. Small piece of metal removed from ear and rec. tetanus shot.

Employee operating stationary belt saw and completing a cut on a block of marble. Standing on platform at control panel, noticed movement to his left. Saw a slab tilting outward, reached to left to very edge of slab. At same time, a 4"" x 6"" x 5' timber used to hold slabs in place came loose toward employee, came in contact with tip of finger on left hand.

Employee taking slab off polishing line with jib crane/grabs to place on A frame. Slab was leaning and not stabilized. She tried to reposition the grabs but the slab cut her left leg and the weight of the slab or the impact caused her heel to fracture.

Struck by falling object

Employee were trying to lift up a slab of marble and a piece broke free. The employee tried to move out of the way but the material hit him in the leg causing a deep laceration that required stitches.

Over-exertion in lifting objects

Continuous twisting while lifting marble on the tile line. EE complained of lower back pain. Diagnosis-bilateral muscle stain of lower back.

EE working on gang saw trying to work a slab loose that was stuck in blades. When the slab broke free, the EE lost his balance and noticed his knee bleeding after the job was completed, with no pain.

Fall from scaffolds, walkways, platforms

EE fell. EE was measuring a slab located on A-frame. Right foot got stuck on the A-frame and without EE realizing it EE fell when he went to move.

EE was operating an automated polishing line. The hose which supplies water to it came off. He cut off all electrical, then went to back of machine to cut off water source where he slipped and knee buckled causing fall.

No conditions contributing to accident. Tripped while stepping up to platform. Became unbalanced and fell on right leg, then both knees.

Marble material on saw bed, in slab form, used jib crane to adjust slab on bed. Began to lift with suction device and released off slab, causing crane stabilizing bar to move up and hit his chin.

OFC. FURNITURE WAS BEING MOVED IN EE'S OFC. EE WAS WATCHING & DIRECTING BUT NOT MOVING ANYTHING. A MARBLE TOP WAS LAID ON FLOR BUT LEANING AGAINST A DESK. OTHER EE BUMPED DESK ACCIDENTALLY CAU SING TOP TO MOVE. EE TRIED TO PREVENT MARBLE FROM FALLING BY REACHING OUT FOR IT.

EMPLOYEE WAS MAKING A CRATE FRO MARBLE USING NAIL GUN. ON 4TH EJECTION OF NAIL, IT SPLIT THE WOOD AT EDGE CAUSING NAIL TO GO THROUGH WOOD, THEN DEFLECTED ON A METAL SQUARE MAKING NAIL HIT FING ER IN RIGHT HAND.

EE WAS OPERATING A STATIONARY AUTOMATIC SHAPING MACHINE. HE FELT SOMETHING IN EYE. FLUSH WAS APPLIED AND HE FELT OK. LATER AT HOME EYE BECAME RED & IRRITATED REQUIRING EXAMINATION.

EMPLOYEE WAS ADJUSTING A RUBBER CUSHION ON A PUSHER BAR THAT ALIGNS TILE INTO THE BLADES FOR CUTTING. WHILE ADJUSTING THE RUBBER INTO THE SLOTTED BAR, THE RUBBER SLIPPED, THE FINGER GOT CAUGHT IN SLOT AND LACERATED RIGHT INDEX FINGER.

EE WAS ALIGNING MARBLE SLAB INTO SAWS BY HAND. SLABS RUN INTO SAWS BY CONVEYOR. THE EDGE OF THE SLAB WAS ROUGH AND SHARP. WHEN SLAB MOVED THE FINGER GOT JAMMED BETWEEN MARBLE AND CONVEYOR ROLL ER CAUSING LACERATION.

EE WAS WORKING ON AN AUTOMATED LOADING MACHINE THAT PICKS UP MARBLE SLABS BY SUCTIONED THE SLABS STUCK TOGETHER. EE TRIED PRYING THEM APART BY HAND CAUSING STRAIN.
