Operator was adjusting the width of the forks on a telehandler. While making the adjustment, the operator's hand slipped, and their finger was pinched between the fork and the back frame. Operator received stitches on their finger.

Showing all 6While shortening the rope on the #2 hoist, the thimble assembly pin was removed and two employees pulled on the rope to remove it from the bracket. When the thimble was free of the bracket, the rope twisted, the thimble swung and struck the miner in the face (hitting miner's hardhat and safety glasses). The miner received lacerations around eye and was taken to urgent care.
Employee was using a pry bar to close a gate on the rail car. After closing the gate, the employee threw the pry bar to the ground, causing a rock to fly up in the air and strike the employee in the eye. The employee was not wearing safety glasses.
Employee was repairing a 980K wheeled loader. In the process his right thumb got pinched between two components resulting in a minor laceration to his right thumb.
While working to disassemble a piece of equipment, the suspended load shifted and pinched his left thumb
RAISING THE BASKET HOLDING CLOTHING IN THE MINE DRY. A PIECE OF RUST FELL A BECAME LODGED IN THE EYE. AFTER 1/2 OF THE SHIFT THE EYE WAS STILL IRRITATED AND A PIECE OF RUST HAD TO BE REMOVED.
