Fall from ladders

Underground mechanic was traversing from the continuous miner onto a 4 foot tall step ladder. The mechanic had EE's left foot on the top step and was placing EE's right foot on the next step. When EE put EE's weight on EEs right foot, the ladder shifted and EE fell approximately four feet to the ground, landing on EEs right shoulder. This injury resulted in a fractured collar bone.

Struck against a moving object

Miner operating a UTV too close behind a haul truck. The haul truck stopped and reversed direction. The bed of the truck struck the windshield and ROPS of the UTV. Miner injured right arm during the collision.

Employee was exiting the cab of the skid steer and rolled an ankle on a piece of raised metal in the bucket.

Caught in, under or between a moving and a stationary object

Operator was adjusting the width of the forks on a telehandler. While making the adjustment, the operator's hand slipped, and their finger was pinched between the fork and the back frame. Operator received stitches on their finger.

While shortening the rope on the #2 hoist, the thimble assembly pin was removed and two employees pulled on the rope to remove it from the bracket. When the thimble was free of the bracket, the rope twisted, the thimble swung and struck the miner in the face (hitting miner's hardhat and safety glasses). The miner received lacerations around eye and was taken to urgent care.

Struck by flying object

Employee was using a pry bar to close a gate on the rail car. After closing the gate, the employee threw the pry bar to the ground, causing a rock to fly up in the air and strike the employee in the eye. The employee was not wearing safety glasses.

Struck against a moving object

Haul truck operator drove over a section of uneven ground and was not wearing a seat belt. The operator bounced up and hit head on the roof and hit knee on the dash. Operator was taken to the hospital for observation. Operator has been placed on modified duty.

Employee was in the process of changing bits on the continuous miner. Employee struck right thumb with hammer while installing bit sleeve causing contusion. Medical treatment only.

Fall from ladders

Employee was working on the conveyor cover on continuous miner 101 while standing on step ladder. A coworker was moving a wooden pallet and accidentally bumped the step ladder with the wooden pallet while operating a TH. The step ladder became unbalanced causing the employee to fall from second rung of step ladder landing on left leg. Medical Treatment Only.

Accident type, without injuries

Collar door switch at #2 shaft collar faulted out. Due to this fault the hoist operator did not know if the door had closed or not. This made it unsafe to operate.

Fall from machine

Employee was exiting cab of dump truck. In order to exit cab safely employee needs to utilize the three vertical steps and two hand grips. Employee was on the first step when EE lost EE's grip and fell approximately four feet to the ground.

Employee was repairing a 980K wheeled loader. In the process his right thumb got pinched between two components resulting in a minor laceration to his right thumb.

There was no accident injury or illness of persons. A spring cup of one of the brakes mechanism was showing signs of ware and had to be replaced. It took more than 30 min. To trouble shoot and install the hoist was back in production at 2:00pm

Preliminary Investigation: The operator turned his head to face forward while applying brakes. The braking process resulted in his nose striking the canopy.

Employee applied brakes, right wheel went into hole. Momentum caused employee's body to twist while right hand was on steering wheel, causing shoulder pain. Was not reportable till he started physical therapy on 4/26/10.

Disconnected a hydraulic line while repairing a main control valve and the bucket on the machine shifted downward causing the machine to shift downward.

Fall from scaffolds, walkways, platforms

Employee was working on 988-B loader. He was moving along a platform ~ two feet off ground. Employee slipped and fell, catching himself with his palms down. He felt pain in his left wrist. He continued working until ~ 4:00 pm. at which time he was taken to local clinic to examine his wrist. X-rays determined he had ""nondisplaced fracture"".

WHILE ELECTRICALLY TROUBLE SHOOTING A DOWNED JUMBO, EE RECIEVED FLASH BURNS TO HIS EYES. HE WAS FIVE FROM DRILL WHEN POWER WAS TURNED ON. FUSE CONTACTS ARC. EE DID NOT REPORT ACCIDENT. WE RECI EVED NOTICE FROM CLINIC 2-22-04. EE REPORTED ON SHIFT 2-17-04.

AT ABOUT 10:20 AM THE HOIST MAN ON DAY SHIFT NOTICED THE WEST SIDE SKIP NEVER LEFT THE POCKET. UPON INVESTIGATION THE WEST SIDE HOIST ROPE HAD COME APART AND BROKE JUST ABOVE THE LAST CROSS B Y CLAMP ON THE THIMBLE ATTACHMENT.

WHILE CONDUCTING A #2 SHAFT, WEST COMPARTMENT INSPECTION, THE INSPECTION CREW PULLED THE SIGNAL CABLE TO STOP THE HOIST. AT THAT TIME THE SIGNALBELL STUCK IN THE ""ON"" POSITION. COMMUNICATION WAS ESTABLISHED BETWEEN THE SURFACE AND INSPECTION CREW, THE SIGNAL CABLE BELL REPAIRED AND THE HOIST RETURNED TO SERVICE AT 6:15 PM ON 6-19-01.

Struck by falling object

RAISING THE BASKET HOLDING CLOTHING IN THE MINE DRY. A PIECE OF RUST FELL A BECAME LODGED IN THE EYE. AFTER 1/2 OF THE SHIFT THE EYE WAS STILL IRRITATED AND A PIECE OF RUST HAD TO BE REMOVED.

THE #2 LOADER WAS BROUGHT INTO THE SHOP BECAUSE OF AN UNUSUALLY LARGE AMOUNT OF BLACK SMOKE COMING FROM THE EXHAUST. WHEN THE LOADER WAS STARTED IT BAGAN TO INCREASE IN RPM'S AND EXHAUSTING A LARGE AMOUNT OF BLACK EXHAUST INTO THE SHOP. AT THIS TIME ALL PERSONNEL IN THE SHOP AREA EVACUATED TO THE FRESH-AIR SIDE OF THE SHOP'S VENTILATION DOOR. BY 1:45 AM ALL 9 MINERS WERE IN FRESH
