Employee was removing a sprocket on #3 aerator. The sprocket was heavier than the employee thought and dropped it onto the guard which was on the employee's right foot. The employee also smashed their right thumb between the sprocket and the guard. The employee's right thumb was fractured.
A maintenance employee conducted repairs inside Aerator #3. During repairs, the employee received burns from steam and lime.
Employee was bent over picking up a drive chain guard for installation when EE felt EE's back strain.
Employee was assisting mechanical maintenance replace belt on #3 palletizer bag flattener. While feeding the belt through the rollers, the employee felt a pop in the left shoulder. Employee later went to urgent care and was placed on restricted duty, cannot lift more than 3 pounds with left arm or 10 pounds combined arms and no ladder work.
Employee was using an electric pump to fill a sprayer with Mild Acid Detergent. The sprayer overfilled causing the hose to pop out of the sprayer. It sprayed the employee directly in the face. EE then began to flush eyes at the eyewash station. Employee was medically evaluated, treated at emergency room and was released to return to work on 8/16/2021.
EE was performing housekeeping activity at the screen plant with a shovel. While the EE was cleaning, there was a sharp pain in the lower back and lower back locked up. OCC Treatment & Restricted Duty
Employee was working on coal elevator and while shoveling strained back. Employee didn't report accident until Monday 3/30. We denied the BWC claim but lost the appeal and was notified on 5/21.
Lime in left eye from cleaning discharge conveyor with compressed air. Received prescription eye drops.
Employee reported to work at 6:30am and after morning crew talk was assigned to remove the slide in the four stone chute feeding the south jet belt. Employee obtained a steel bar used to remove slide and discovered it was stuck. Used the bar to bang on it a few times to loosen it up and attempting to lift and remove again, causing a lower back strain.
Employee was closing the east side store room dock door on March 21, 2018 when the door jammed and caused the employee's left arm to jerk. Employee continued working without issue but subsequent medical visits revealed a rotator cuff tear. Surgery was deemed necessary and performed on June 25, 2018.
Employee was pulling scale out of #3 kiln hood when a piece of scale became stuck in the cooler grate. When this happened, employee felt a strain in his lower back.
EE was trying to remove lid from a sample missile and while doing so he strained his right wrist. On 5/2/11 EE began P.T. which made this incident reportable.
EE was positioned near duct opening. Duct was on a bind and released from opening with force. Duct hit ee knocking him down.
EE was pulling on a wooden crate in order to align it so a fork truck could pick it up more easily. While pulling on the crate, it pinched EE's left middle and left ring finger tips, fracturing the tuft under the nail.
Poking #3 dust chamber, dust flew out and got in gloves burning left hand.
Cleaning up under crusher pan shoveling screenings into elevator when EE twisted and felt lower back pop. Had pain in lower back and down right leg.
Employee was closing tailgate with right hand and finger was caught between tailgate hinge pin and lock hook.
The employee was removing a crusher end cover and when the cover broke loose it swung towards the employee pinching his left hand between the cover and a bucket elevator housing.
Pulling scale #3 Hood Scale Rod slipped off of the piece causing hand to come in contact with the bottom edge of the primary air gate weights cutting finger. Went to emergency room and they put in 3 stitches.
PERFORMING DAILY LUBRICATION OF SECONDARY SCREENING PLANT. WALKING FROM ONE LOCATION TO ANOTHER, EE BENT DOWN TO PASS THROUGH A LOW-HEADROOM AREA. WHEN EE STRAIGHTENED UP, HE FELT A PAIN IN HIS LOWER BACK.
THE BONDCRETE BULK DISCHARGE FEEDER SLIDE WAS NOT COMPLETELY CLOSED. LIME SHOT OUT WHEN FEEDER INSPECTION DOOR WAS REMOVED.
EE WAS TRYING TO OPEN BULK TRUCK LID-DIDN'T GO TO DR UNTIL APRIL 1, 2002.

EMPLOYEE WAS SHOVELING STONE IN A HOT AREA AND COMPLAING OF CHEST PAINS AND SHORTNESS OF BREATH. RESCUE SQUAD WAS CALLED AND TRANSPORTED EMPLOYEE TO HOSPITAL. EMERGENCY ROOM PHYSICIAN DIAGNO SED HEAT EXHAUSTION & ADVISED HIM TO REST FOR 24 HRS.
