Employee was rigging up the cone ring to remove it from the cone. It got bound up at an angle and would not move. EE reset the rigging then started to descend from cone. The cone ring did not have any pressure from the rigging. Descending from the cone onto the platform EE put EE's hand out for support, the ring fell into its resting position just 2" from bound position.

While removing an alternator along side another employee, the other mechanic was pulling the detached alternator through the top of the machine, while injured was pushing through the bottom. It slipped out of the other mechanic's hand and fell down smashing injured employee's right middle finger between the alternator and the engine mount bolt.

Employee was cleaning the right side windows on the outside of a Komatsu WA500 front end loader, when they fell from the top of the loader and landed on the ground next to the loader.

While disassembling a gear box from a super stacker, it was in park postion sitting on a pile of material, went to pull it off the shaft, when it fell it struck the employees foot rather than going into the loader bucket.

Conveyor belt had come to a stop, due to being overloaded with material. LOTO was performed, guarding was removed on the wing pulley. Employee used a shovel to remove material, and then they tried to scrape the rest of the material off pulley with right hand. It was at that time the weight conveyor to move backwards, causing the arm to be caught in between the belt arm, and pulley.

Employee was assisting changing out a water pump and a section of 10" diameter pvc pipe slipped from it's supported location and landed on employee's right index finger causing slight fracture to tip of finger bone and laceration that required approximately 4 stitches to close.

Employee was changing a spring on the wash plant, when EE's hand became caught in the spring. EE sustained a broken right ring finger and a laceration that required stitches.

Employee was involved in grinding operations and was wearing welding helmet and safety glasses. Employee received medical treatment on 5/25/13 to remove metal sliver from cornea. Employee was given medication and ointment for infection. Employee is fully recovered with no pain or vision impairment as of 5/28/13.

Miner was helping change screen on Crusher. He was pulling screen into position when he felt a slight muscle pull. He continued to work through his shift on 03/14/2012. Upon waking on 03/15/2012 he had considerable low back pain. At that time he called work to report injury. After treatment in ER he was released to light duty immediately.

EE was removing shim to adjust primary jaw opening . One side of shim hung up. EE attempted to free shim with left hand. Hydraulic mechanism then failed, allowing bottom of jaw to move and catch fingers in shim stack. X-rays disclose Lt finger fracture. Further exam is to occur 7/20/09 when swelling is reduced.

Employee was changing oil in the Jaw Feeder. When he pulled the plug, the oil splashed on to the right hand glove, soaked through the glove causing a 2nd degree burn.

Rock lodged in tail pulley of conveyor. Folded conveyor up to reduce belt tension. Removed rock. While lowering conveyor to operating position, rock fell/rolled from belt. Finger pinched between conveyor frame & rock. Four stitches required to treat injury.

The employee was assisting a co-worker replace a drive belt guard on the screen plant. As they were maneuvering the guard into place, the employee's hand slipped and struck the frame brace that is situated near the guard. His thumb required 7 stitches to close the wound. He was fitted with a metal protective guard on his thumb.

EE WAS ASSISTING A COWORKER IN THE REPLACEMENT OF AN IDLER ROLLER. EE PLACED HIS SIDE OF THE ROLLER INTO THE BRACKET AND PLACED HIS HAND ON A WOODEN BLOCK JUST BELOW THE ROLLER. THE COWORKER MOVED HIS SIDE OF THE ROLLER TO GET IT SET INTO THE BRACKET. THIS CAUSED THE ROLLER TO FALL AND HIT EE'S HAND THAT WAS ON THE BLOCK. CUT FINGER.

EMPLOYEE WAS WORKING ON THE ELECTRICAL PANEL BOX ON THE CRUSHER SPREAD. THE WIRE CABLE WAS STUCK. HE PULLED THEM TO LOOSEN THE JAM. WHEN IT CAME LOOSE. EMPLOYEE LOST HIS BALANCE AND FELL FO RWARD HITTING HIS CHIN ON THE EDGE OF THE PANEL BOX CUTTING HIS CHIN.

EE WAS LIFTING AN END OF A METAL WEAR PLATE WITH ANOTHER EE ON THE OTHER END LIFTING ALSO. THE TWO LIFTED THE METAL UP ONTO THE CRUSHER. EE STATES HE FELT FINE UNTIL THE NEXT MORNING WHEN HE A WOKE AND HAD PAIN IN THE ABDOMEN AREA.

EE WAS ATTEMPTING TO REENTER THE TRUCK AFTER A REST BREAK. AS HE STEPPED UP TO PUT HIS FOOT ON THE LOWEST STEP, HIS FOOT SLIPPED. HE WAS HOLDING THE HAND RAIL WITH HIS LEFT HAND, THIS SWUNG HI S BODY SHARPLY TO THE LEFT CAUSING A SEVERE STRAIN TO HIS LEFT SHOULDER. HE HAS BEEN REASSIGNED TO MODIFIED DUTY TEMPORARILY.
