Over-exertion in lifting objects

The employee leaning across screen deck and prying on belt with pry bar and felt abdominal pain. Employee prescribed lidocaine patches for pain.

Struck by flying object

Employee performing maintenance on tail pulley bearing and strap-end being used to perform task slipped and became unhooked, causing strap-end to strike employee in back of head below hard hat area. The contact caused a laceration in back of head that required stitches to close.

Caught in, under or between a moving and a stationary object

EE was removing shim to adjust primary jaw opening . One side of shim hung up. EE attempted to free shim with left hand. Hydraulic mechanism then failed, allowing bottom of jaw to move and catch fingers in shim stack. X-rays disclose Lt finger fracture. Further exam is to occur 7/20/09 when swelling is reduced.

Struck by falling object

EE WAS ASSISTING A COWORKER IN THE REPLACEMENT OF AN IDLER ROLLER. EE PLACED HIS SIDE OF THE ROLLER INTO THE BRACKET AND PLACED HIS HAND ON A WOODEN BLOCK JUST BELOW THE ROLLER. THE COWORKER MOVED HIS SIDE OF THE ROLLER TO GET IT SET INTO THE BRACKET. THIS CAUSED THE ROLLER TO FALL AND HIT EE'S HAND THAT WAS ON THE BLOCK. CUT FINGER.

Fall from scaffolds, walkways, platforms

EMPLOYEE WAS SWEEPING DIRT OFF CATWALK OF LOADER. AS HE STEPPED FROM CATWALK TO ACCESS LADDER, HIS FOOT SLIPPED, CAUSING HIM TO FALL AND LAND ON THE GROUND. SUDDEN IMPACT CAUSED A SPRIAN OF LE FT KNEE. FIRST AID TREATMENT AND DIAGNOSIS WAS SUFFICIENT UNTIL NOV 21, 2001. A RETURN VISIT TO DOCTOR WAS NECESSARY.
