originally Diagnosed with a non reportable strain to EE's left knee, sustained while connecting a hose to a tanker truck. On follow up with Orthopedic Dr. 02/12/26 EE was scheduled for MRI and PT. Case elevated when EE started medical treatment (physical therapy) 03/09/2026.

EE was going up to the top of the upper shelf lime silo to unclog and get it to flow when EE opened the smaller round door to probe the lime or rodding if you will, it broke loose and came out of there like water getting all over EE and into EE's right eye.

Employee was removing a broken pallet from the turntable on our production line. The employee slipped on the rollers on the turntable, and strained lower back.

Poor engineering. Employee's finger was smashed by swinging door.

first aid incident occurred on 10/30/23. No medical treatment prescribed. After months of continued pain, employee asked to be seen by doctor again. Doctor prescribed medical treatment (physical therapy,) elevating the first aid case to a reportable accident.

Employee reported to work with pain in shoulder, elbow and wrist

Employee was stacking bags of Portland cement and suffered chemical burns on fingertips

Employee was palletizing cement when a bag busted and some cement got into the employees eye

Employee reported pain in mid back after stacking bags of sand, and requested the EE be seen by doctor.

EE was helping change motor on sand line palletizer. The crew went to tighten the motor but had an alignment issues with the motor. When they tried to adjust the motor it started to fall to the ground. EE took the motor slowly to the ground and in the process bruised left forearm.

Employee was unloading bags from the reject shoot (roller bed that sends bag down to table, for removal by hand.) The employee lifted a bag, then turned to put bag on a pallet. When EE turned, EE struck EE's knee on the leg of the table. Dislocation of right knee cap.

Employee was assisting in the installation of a drive unit on a mixing screw. The unit was under tension, being pulled into alignment. Employee was using fingertip to check alignment of bolt holes. The unit unexpectedly and suddenly moved, causing the amputation of left pinky fingertip including the fingernail.

Operator said they got right shoulder strain from performing hand add for the batch operation. Operator did not report the condition until 3-19-19 and said it occurred back in October 2018.

Employee reported that the employee pulled a muscle in employee's back while moving a 50# bag of material

Large grains of sand fell into left ear. Miner was unable to remove sand, and was sent to clinic to have ear irrigated. Two large grains of sand were removed, and antibiotic drops were prescribed to prevent infection.

Operator was hand stacking tube sand bags and said ee pulled a muscle in right shoulder.

Operator was handing bulk bags, when removing bags from hooks EE got hand caught between strap and bag. Was not following proper communication with forklift driver and other operator.

Operator was hand stacking tube sand and said EE hurt back/bulging disc while performing the job.

Operator was in mixing room adding hand adds to batch system and then noticed swelling a couple days later at home. Accident was then not reported to plant until 2-13-17. Operator says suffered a hernia on right side.

EE was working on the main line packer and cleaned the feed spout that caused ee to cut hand. There is a cut on left thumb and looks like it needs stitches.

Sharp edge of banding on bundle of burlap bags cut employee's hand, while employee was moving the bundle. Laceration to left palm required 5 stitches. Employee was returned to work with limited use of hand for 10 days. Follow up to be done by attending clinic, in 10 days.

Employee was stacking product and twisted instead of using safe lifting practices causing a strain in back.

Employee pinched thumb between strap on bulk bag and hook that holds bulk bag when second operator lowered pallet holding bulk bag. Operator suffered bruising and did not seek medical attention until 6:30 on 9/2/2015

Cement dust go onto employee's arms and caused a rash.

Employee was attempting to dump portable dumpster when his hand slipped cutting his wrist on the lever.

A production operator was bent down to pick up some tubes and hit his right knee on some empty pallets.

On 5-2-14 employee and a co worker were changing screens on the mill. When they were lifting off the cover plate EE felt a "pop" in her right wrist. I asked if she needed medical care. She refused. On 5-7-14 EE came into the office and said her wrist was sore and she would like to get it looked at just in case. I sent her to HRRMC in Salida Co.

Employee was operating a semi-automatic palletizer when debris went under his safety glasses and entered his left eye.

Employee was pulling material out of the Dust Collector for the Q-Line from the opening above him. A chunk of material fell from inside the container and fell onto his head/face. Employee was not wearing eye protection as required by company SOPs. Portland cement dust got into his right eye.

Employee claims to have dropped a empty pallet on foot. Employee worked from 3:30 until 4:30 and did not report the accident. Employee reported accident following morning via voice mail. Accident resulted in a fractured toe. Employee missed one day of work and returned with no restrictions

Employee was walking across a traffic zone when a forklift backed into him causing a broken tibia bone in the employees leg.

The employee was shrink wrapping and complained of minor back pain.

Employee was picking up 50# AP sand bags and went to put it on a pallet when he felt his shoulder R pop out and in and now he is having pain in his shoulder. Diagnosis: Right Shoulder Stain.

Preforming bagging operation at plant, stacking 50# sacks on a pallet. Lower back strain.

A bag of portland cement was travel up an incline belt on its side. Miner pushed bag with broom handle to position bag. Broom handle pierced bag and portland cement went around safety glasses and got in employee's right eye. Cornea of right eye is scratched.

Miner's left ring finger was pinned between a piece of equipment being moved and a stationary pole. A fracture of the finger resulted.

Employee was on the sand line packer, a bag got caught he jumped down to clear the bag from the belt and hit his knee on a piece of steel, left knee.

Was lifting 100lb sand bags and turned to place it on pallet and wrist pooped, left wrist.

Employee said he hurt his back picking up a pallet. Employee said he has a pre-existing back problem and just needed some rest and he would be fine. After leaving work he went to his primary doctor and was taken out of work until 2/21/12. Employee resigned 2/16/12.

Miner was lifting pallets to tie bulk bags when he experienced pain in lower abdomen/groin area

Employee was hand stacking bags and stated he strained his back. This was not brought to management's attention until 7/25/2011.

Employee received a lime burn on his left wrist and arm. Employee was wearing the proper PPE including glasses, dust mask, gloves, and tyvek suit.

Employee complained of back pain. Was taken to IMA and released on light duty until 7-15-10. Employee worked light duty until 7/14. Employee went to another doctor 7/14 and was taken out of work for 7 - 10 days.

Employee bumped elbow on corner of shelf causing numbness and tingling in fingers. Employee was under a doctor's care from the time of the injury to present. The employee had surgery on 2/11/2011 which is when the injury became lost time. Employee worked full duty until 2/11/2011.

Caught fingers between sheave and belts while removing belts from elevator motor.

EE picked up 5 gallon pail - it slipped out of his hand, dropping on left instep.

While picking up bag, EE hit the back of his r. hand on the packer spout. This caused a laceration witich required stitches.

While removing propane tank from rack felt sharp pain in lower back.
