While attempting to remove a chute cover, the cover came loose. The employee tried to stop the chute from falling by grabbing the bottom of the cover. The employee's finger was caught between the chute cover and a slide gate below the chute. Cutting their right ring finger.

Showing all 6While changing bucket wear liners, the employee placed a hand on the head of the plow bolt to hold it in place while removing the nut on the bolt holding the liner in place. The head of the bolt turned cutting the employee's hand.
While dumping a 2 cubic yard hopper used to collect dust. The employee inadvertently put a hand between the base of the hopper and fork of the fork lift while releasing the lock. When disengaged, the weight transfer of the hopper caused the employee's hand to get pinched. The injury caused ring finger to break and partially severed the tip of middle finger.
Employee was attempting to install a bulk bag in the bag loading machine. The employee hooked the rear two straps and pulled the bag towards themselves to make it easier to hook the front straps. The rear mechanism disengaged, releasing the bag, causing the employee to fall backwards. The employee tried to catch themselves with right hand and sprained a wrist.
EE WAS OBSERVING ANOTHER WORKER GREASE THE LOWER BUCKET PIN ON A LOADER. A FOREIGN OBJECT (GRIT) FLEW OUT OF THE RELIEF FITTING & STRUCK HIS RIGHT EYE. HE WAS WEARING SAFETY GLASSES.
EMPLOYEE WAS EXITING COLOR BATCH AREA DOWN 5 RUNG LADDER HE MISSED RUNG AND LANDED ONE FOOT ON FLOOR.
