Operator was using a portable bandsaw cutting uni-strut when the bandsaw came into contact with EE's forearm causing a laceration.

While attempting to remove a chute cover, the cover came loose. The employee tried to stop the chute from falling by grabbing the bottom of the cover. The employee's finger was caught between the chute cover and a slide gate below the chute. Cutting their right ring finger.

While changing bucket wear liners, the employee placed a hand on the head of the plow bolt to hold it in place while removing the nut on the bolt holding the liner in place. The head of the bolt turned cutting the employee's hand.
While dumping a 2 cubic yard hopper used to collect dust. The employee inadvertently put a hand between the base of the hopper and fork of the fork lift while releasing the lock. When disengaged, the weight transfer of the hopper caused the employee's hand to get pinched. The injury caused ring finger to break and partially severed the tip of middle finger.
Maintenance employee was using a hand grinder to clean up a welding slag off a piece of expanded metal for a catwalk. The employee claims that EE felt something in eye. EE thought EE got the object out. The employee claims EE didn't have any other issues until the morning of June 7, when EE went to the doctor to seek medical attention. Did not report the injury until June 7.

Employee was attempting to install a bulk bag in the bag loading machine. The employee hooked the rear two straps and pulled the bag towards themselves to make it easier to hook the front straps. The rear mechanism disengaged, releasing the bag, causing the employee to fall backwards. The employee tried to catch themselves with right hand and sprained a wrist.

The Truck Operator was accelerating the truck to clean out the auger, accidentally hit the swing on the auger controls knocking employee to the ground.
An employee was attempting to remove built up material next to load cells in a hopper. EE attempted to use a 5 foot long bar to remove this material. When EE got to more consolidated material, the employee used a hammer to strike the spade end of the bar assist in breaking up the material. A foreign particle became airborne and struck the employee in the chest and was embedded.

Employee was walking from the office to the employee break room near the end of the shift and slipped or tripped and fell contacting the wall of the break room.
Employee parked the pick-up next to the drill to check on driller. The employee went to open the back door of the truck and slipped and fell to the ground on ice. Employee tried to catch self with left arm to break the fall. Employee requested follow up. Has been prescribed Physical Therapy on 4-16-19.

Employee was attempting to climb a ladder to maintain a jaw crusher. When ee stepped on the third rung of the ladder, the ladder twisted causing the employee to lose balance and fall to the ground. The employee put out left hand to try to catch self, and hurt left wrist when ee impacted the ground.

EE attempting to step down from Ballast Loading Dock to track level(approx. 26"" verticle drop in elevation). Lost footing causing him to fall backward. Attempted to brace fall by extending right arm/hand to contact ground on loading dock. When hand contacted ground, EE felt pain in right arm. Completed shift. Saw DR. 11-25-08. Rx prescribed, X-ray & MRI ordered. Restricted Duty
Employee was remotely monitoring operation of crush/screen operation from a sitting position inside control room trailer. Sharp pains felt in left side of chest. Supervisor called and first aid was given assisted by MSHA personnel who were on property at that time. Transported by ambulance to hospital. Released same day return to full duty. Diagnosis incomplete.
EE was removing worn center section on bucket lip of #3 992 Cat loader in mobile shop. EE was attempting to dislodge section from bucket by striking the section with 2 lb hard surface hammer. Small sliver of metal segregated from section edge striking him on left side of abdomen & embedding slightly under skin. EE was seen by physicain (lanced & sutured) & released to full duty.

EE was climbing up ladder preparing to perform maintenance on sheave for No. 1 Screen conv. in the ballast plant. He decided to climb down the 12 ft straight ladder to access area from different location. As he was climbing down, the ladder slipped at top causing ladder and he to fall to ground and his right knee. He was seen by dr. and released for restricted duty.

EE was operating loader cleaning bottom of Quarry face. Rock fell from face, striking bucket of loader, jarring operator in loader. Top of face is approx 30' high. EE continued to perform same job duties on scheduled shifts until 9/22/06, at which time medical professional personnel advised EE not to return to work until future examination scheduled for 9/25/06.
This accident occurred 3/16/07 and became a lost time on 4/20/07. Employee was carrying oil pump to different location in mobile equipment shop. He stepped in a small crack in the concrete floor, lost balance and fell. Pump landed on palm of left hand. Employee received medical attention and was released. Additional medical diagnosis on 3/4/07 indicated surgery required on thumb.
Employee fell while trying to dislodge a rock from a self-cleaning tailpulley.

EE WAS OBSERVING ANOTHER WORKER GREASE THE LOWER BUCKET PIN ON A LOADER. A FOREIGN OBJECT (GRIT) FLEW OUT OF THE RELIEF FITTING & STRUCK HIS RIGHT EYE. HE WAS WEARING SAFETY GLASSES.

TWO EMPLOYEES WERE ATTEMPTING TO CHANGE THE V-BELTS ON THE #2 PRODUCT CONVEYOR DRIVE UNIT. ONE EE DISCONNECTED THE TURNBUCKLE FROM THE DIRVE UNIT. THE UNIT ROTATED FORWARD AND FORCED HIS FACE INTO THE FRAMEWORK OF THE BALLAST WASH SCREEN.
EE HAD FOLLOWED INCORRECT MIXING PROCEDURE AND PUT CLAY INTO A COLOR BATCH AS THE FIRST INGREDIENT. THE CLAY PLUGGED UP THE BOTTOM OF MIXING TANK. EE WAS CLEANING TANK, SLIPPED AND FELL AGAINS T A TANK BAFFLE WITH HIS HAND. HE WAS NOT WEARING HAND PROTECTION. RIGHT HAND WAS LACERATED BETWEEN INDEX FINGER AND THUMB.
EE WAS ATTEMPTING TO DRIVE A BEARING OFF A 4 3/8"" DIAMETER SHAFT USING A 3/16 HAMMER AND A PIECE OF KEYSTOCK. HE MISSED THE KEYSTOCK AND STRUCK HIS HAND WITH THE HAMMER. EE REC. 3 STITCHES AND WAS RELEASED TO WORK.

EE WAS EXITING LOCAL HARDWARE STORE AFTER PICKING UP MAINTENANCE SUPPLIES. HE FALL TO RIGHT LEG FOLDING IT UNDER HIM WITH HIS BODYWEIGHT LOCAL CLINIC TOOK X RAY WHICH WAS A FRACTURE TO RIGHT K NEE CAP. THEY REFERRED HIM TO ORTHOSCOPIC SURGEON.
