He was inside the log washer and other employees were handing the paddles to him, he reached up to take a paddle and felt pain in his back. The employer wasn't notified of the injury until 2/29/12. The accident did not become reportable until 3/2/12 after seeing a doctor and being placed on work restrictions.
Employee was trying to dislodge material that was stuck in the impeller. On his last attempt the rock broke free and fell on his hand. The employee stated that he did not want to see a doctor. Nothing was done other than first aid. On 3/7/11 the employee seen a doctor. The accident became reportable at that time.

EE STATED THAT HIS BACK WAS SORE FROM SHOVELING.THIS INJURY WAS NOT PREVIOUSLY REPORTED IT IS BEING REPORTED NOW AS A RESULT OF A PART 50 INSPECTION.
EE WAS LAYING ON HIS BACK LOOSENING BOLTS ON THE UNDER SIDE OF A SCREEN. A SCREEN RAIL FELL OFF THE TOP OF THE SCREEN DECK AND HIT HIM IN THE RIBS. IT BROKE 2 RIBS ON THE LEFT SIDE OF HIS BODY .
