THE EMPLOYEE WAS OPERATING A FORKLIFT. THE GROUND SLOUGHED OFF SENDING THE EMPLOYEE AND THE FORKLIFT INTO THE POND.

Fall onto or against objects

Employee was carrying a piece of metal up a flight of stairs. He slipped and hit his left side on the railing.

Caught in, under or between a moving and a stationary object

IE was helping tie off pipe, the loader operator dumped an impeller out of the loader bucket which caused the pipe to move. The IE's leg was pinched between the pipe, knocking him off balance. Falling while his leg was pinched and broke his right fibula.

Fall onto or against objects

Employee was walking up the conveyor walkway and stumbled. He jammed his finger into the guard rail and sprained the finger.

Employee was changing screens. He strained his lower back while removing the screens.

Fall to the walkway or working surface

Employee was doing daily maintenance on the crusher. When he was climbing down his hand slipped and he fell with his right arm extended and dislocated his right shoulder.

The employee was helping change paddles in the log washer. He was inside the log washer and other employees were handing the paddles to him, he reached up to take a paddle and felt pain in his back. The employer wasn't notified of the injury until 2/29/12. The accident did not become reportable until 3/2/12 after seeing a doctor and being placed on work restrictions.

Struck by flying object

The injured employee was working in the same area as an employee who was grinding and a piece of metal flew into the employee's eye. The employee stated that he was wearing his eye protection at the time.

Over-exertion in pulling or pushing objects

Employee was tightening bolts with a ratchet, felt pain in his waist. Employee stated that he was okay and didn't want to go to the doctor. On 3/7/11 the employee went a doctor. X-rays were negative but the doctor treated him for back strain and put him on work restrictions. Accident became reportable at this time.

Fall onto or against objects

The employee was removing a guard. While engaged in this activity he stepped backwards and slipped, he then tripped over a bucket that contained materials he was using for his job and fell back against the tail pulley guard injuring his lower right side of the back.

Struck by falling object

Employee was trying to dislodge material that was stuck in the impeller. On his last attempt the rock broke free and fell on his hand. The employee stated that he did not want to see a doctor. Nothing was done other than first aid. On 3/7/11 the employee seen a doctor. The accident became reportable at that time.

Employee was changing a bearing on a gearbox. He is not sure how or when he got hurt, but he thinks it happened after pulling and moving the gearbox. He told us that the accident happened on 12/15/08 however he did not report it to his supervisor until 12/18/08.

Injured employee was changing a bearing on a pulley. A chain that was being used came loose an hit the employee in the hand.

The injured employee was changing screens. He raised up and hit his head on a bar above him.

Injured employee was changing screens in the screendeck. When the employee was attempting to remove a wire screen from the screendeck, he pulled on the screen and felt a pain in his lower back.

Injured employee was helping move pipe. The wind blew unknown particles into the employee's eye. Employee was seen by a physician and had the foreign body removed from his eye.

Employee was feeding the crusher with a front end loader. He applied the brakes too quickly and injured his lower back.

The injured employee was using an impact wrench to remove bolts. The employee states that while doing this something got in his eye. He removed some type of foreign body from his eye the following day. His eye was red and sore.

THE INJRED EE WAS RIDING ON THE FRONT OF A FORKLIFT WHEN THE OPERATOR/WITNESS LOWERED THE BOOM, STRIKING THE EE IN THE HEAD. RIDING EQUIPMENT IS PROHIBITED BY COMPANY POLICY. THE INJURED EE WA S WEARING A HARD HAT AND SAFETY GLASSES. TO THE BEXT OF COMPANY KNOWLEDGE, THE ACCIDENT DID NOT INVOLVED SKILLS, PROFICIENCY, TRAINING OR ATTITUDE.

EE WAS LAYING ON HIS BACK LOOSENING BOLTS ON THE UNDER SIDE OF A SCREEN. A SCREEN RAIL FELL OFF THE TOP OF THE SCREEN DECK AND HIT HIM IN THE RIBS. IT BROKE 2 RIBS ON THE LEFT SIDE OF HIS BODY .

EE WAS TIGHTENING A BOLT WITH A RATCHET. HE WAS USING A CHEATER PIPE. THE RATCHET SLIPPED AND THE CHEATER PIPE HIT HIM ABOVE THE RIGHT EYE. IT CUT HIS BROW ABOVE THE EYE AND NEEDED STITCHES.
