Employee picked up a 80 pound piece of steel and while holding it slid out of their hands and landed on top of boot. Employee stated that it did not hit the steel part of the boot.

Employee picked up a 80 pound piece of steel and while holding it, it slid out of EE's hand. The steel then landed on top of EE's boot. EE stated it did not hit the steel part of the boot.

Employee bent over to pick up his hard hat and when he stood back up to put it on he hit his head on the tail pulley on belt #3. He cut his head and had to have seven staples put in his head.

Employee reached into a heater fan that he was working on to retrieve something that had fallen in. The fan came on and cut his 3rd finger on his right hand. The cut required stitches.

Employee was helping remove a rock from a truck tailgate. The rock fell out and the tailgate shut on the employee's hand cutting and pinching his two middle fingers on his right hand. He required 8 stitches in his fingers.
