The EE was cleaning out of the bottom door on the bucket elevator. The top door was held open with a bungee cord, the bungee cord let go and the door closed on their thumb.

The employee was working on a hub assembly of a cage mill. While trying to turn the hub and shaft employee's hand slipped. The rough worn surface had a sharp edge that cut through employee's glove causing a laceration on the palm near right thumb. It required 4 stitches with no restrictions. The employee removed own stitches on 5-25-2020.

Employee pulled a groin muscle trying to adjust a bulk bag on a pallet as EE was beginning to fill. EE felt a sharp pain. EE was taken to the local ER where EE was diagnosed with a strained left groin. EE has been placed on light duty for one week.

Contractor was cleaning and picked up heater to move it. While moving the heater, two elements burned out and heater started sparkling. Contractor threw heater down, ran away and twisted EE's wrist.

Employee was cleaning the area around 4 mill when EE decided to move a few 55# grinding media bags off of a platform about 42" above the floor without assistance. EE grabbed a bag in each hand and as EE swung the left handed bag to the floor, EE felt a pain and pull in EE's abdomen. Diagnosis hernia and placed on restricted duty.
Employee was struck by an airline hose when it slipped from the hose clamp holding it. The employees hard hat was knocked off and EE received a laceration to the top of the head. Employee received three staples and was released to work full duty.
Two employees were moving a diesel generator to another level. When they were connecting the 8" flexible hoses, one of the employees strained side (front wall of thorax) while trying to get the hose positioned in order to close the cam-locks.

Employee was on top of a bulk tanker opening the hatch at the load station. When they twisted to pull the last latch, they indicated that they felt a muscle strain in the lower back from body position.

A bulk loader was loading a bulk trailer. EE began opening the center hatch of the bulk semi. As EE was opening the hatch EE found all the hatch latches to be difficult to open but EE was able to open the first four. On the fifth latch EE pulled and felt a strain in right shoulder. After EE felt the strain in EE's shoulder EE used a hammer to pry the 5th and 6th latch open.

Employee was working on a dryer discharge chute in the pelletize building. The employee's right hand middle finger came across some sharp metal on the chute area causing a laceration that required medical treatment and four stitches with prescription. The employee returned to work immediately with no lost time or restrictions.

EE was shoveling upstairs in Plant 4 & felt a burning sensation in right shoulder. Requested to have a medical evaluation. Employee was taken to the ER, received X-rays, a prescription for an anti-inflammatory, and was medically cleared to return to work on EE's next scheduled shift with no restrictions. *EE received 2 days restricted duty on follow up visit.

Dry, well lighted; Miner was opening bulk truck hatch when one of the spring loaded hatch locks sprang back to the closed position resulting in the Miner jamming his little finger (right hand) back into the hatch cover which resulted in a dislocated joint on impacted finger. Confirmed via x-ray. ER Doc realigned joint, splinted and prescribed meds for pain.

Employee was assisting welder cut some metal by supporting metal with gloved hands. The heat from the cutting torch got too close to the small area of exposed skin between the gloves and shirt of the assistant causing a small burn to the skin. The burn was classified as 2nd degree.

He complained of sore and achey wrists. He had been moving/stacking pallets and running the forklift the shift prior. He was diagnosed with "Tenosynov Hand/Wrist Neck"
