Caught in, under or between a moving and a stationary object

Adjusting saddle on 255 packer, saddle started to slid towards the floor, employee attempted to catch it. Right thumb caught in pinch point, resulting in cut requiring 13 stitches at local ER.

Two maintenance employees were attempting to tighten a pressure gage, both were using wrenches. One wrench slipped and struck the injured employee over the right eye, resulting in small cut. Injury was treated at local ER, 3 stitches were required.

Employee took one step down stairs inside Plant 3, woke up on the stair landing, does not remember falling or how he got to landing. Had small contusion on left elbow. Was treated at local ER for elbow and tests to determine why he fainted.

Employee was attempting to clear a rock hung up in the crusher feeder with a steel pry bar. The bar slipped and caught his left thumb between the bar and the edge of the crusher box. Cut to end of thumb and chip to bone in the last joint. Thumb was x-rayed, bandaged and placed in splint.

Employee was removing a piece of rock from the railroad track area between Plant 5 and Plant 13. Picked up rock, copperhead snake was underneath the rock, employee was bitten on the right thumb. Taken to hospital for treatment.

Employee was leaning over to place a pallet on a bagging station and dust blew into his right eye. He was wearing safety glasses at the time. First aid was administered, but he still required eye washing at the local ER.

Employee was beginning to work on bearing on tube mill #3, relieved water pressure initially, started to relieve water pressure again, water line came loose and left forearm was burned by hot water.

Employee was working on #2 Tube Mill, twisting and turning performing maintenance work, strained lower back sometime after lunch. No exact time given for accident.

Employee was sitting on catwalk near Plant 5 screens, throwing spilled rock off catwalk. When he stood up he felt a tightness in his lower back. Did not report to supervisor until 10/03/2005 at 11:00 am. Went to company doctor on 10/06/2005 and was placed on restricted duty ( no lifting over 20 #).

Supervisor was walking up stairs at Plant 5, walked under low steel beam, stood up thinking he was clear and struck his head on the bottom of the beam. He was wearing a hardhat at the time.

Employee was bagging super sacks (2000# bags). When he released the holding straps, the filled bag began to lean and roll towards him. He attempted to manually hold the bag in place, it pushed him to the concrete floor, straining his lower back.

73 year old employee walking down steps near CS-11 bagger, stepped from first step to concrete floor, twisted left foot. Hairline fractures in left ankle and left foot.

Employee was using a come-along to lift a roll of conveyor belting. Other miners attempted to slide the roll into position. The come-along slipped out and the chain struck the injured employee in the head. Required 6 stitches in his left earlobe and 3 stitches behind his left ear.

Employee was in a manbasket attached to a forklift. The basket was 5 feet above the ground. While lowering the basket, the forks stuck momentarily, the chain continued to move, then the forks fell 3-1/2 feet stopping the basket 1-1/2 feet above the ground. The sudden stop casued the employee to strike his throat on the manbasket handrail.

EE was cleaning out from under palletizer at Plant 5. EE raised up and hit hard hat on steel framing of machine. Reported to supervisor on 3/24, requested medical treatment at that time. Strain to neck.

Employee operates a Trackmobile. Over several days, developed a pain in the left knee while performing normal operator duties. Informed manager on 2/22/2005 of the pain and requested medical treatment. No record of an actual injury date. Doctor diagnosed slight strain to left knee. Returned to work same day.

Employee was walking down stairs in shop(4 steps) from restroom, missed a step and fell to the floor, approx 28 inches. Caught himself by his left hand, bruised/strained middle finger on left hand.

Employee was adjusting the tracking on a newly installed conyeyor belt, right hand slipped and was caught between the belt and a small tension roller. Cut to the right pinky finger (four stitches) and abrasions to the back of the right hand.

EE WAS GETTING OFF ROOF INTO MAN BASKET AND UNHOOKED SAFETY HARNESS BEFORE HOOKING INTO MAN BASKET. EE WAS ON UNSTABLE ROOF SHEETING IN DANGER TAPE AREA. ROOF SHEETING GAVE WAY AND EE FELL TO GROUND, APPROX. 18'8"".

Employee was shoveling product off a catwalk area, stepped backwards and fell through an opening in the expanded metal flooring. Fell 11 feet to the ground below, spraining his right shoulder

Supervisor were traveling thru the plant 4 parking lot area. they noticed adult male lying on the grount near a flat bed truck. he stated he had fall off his truck to the ground.he climbed his truck cab to sit in the air conditioning and thought he would feel better 30 min later request medical treatment have pain back. ambulance picked him up. he is not member of property.

Employee stepped of concrete ramp at a height of 18 inches to ground below, feet slipped and he fell on his lower back and hip. Continued to work with no problems until requesting medical treatment on 7/21. Visited doctor on 7/23, was given work restrictions and released back to work.

Employee developed an infection in both ear canals, aggrevated by wearing ear plugs. Employee will wear ear muffs in the future.

Employee was draining oil from Raymond Mill #53 at Plant 5, hot oil burned his right index finger

EMPLOYEE WAS DRIVING 25 TON HAUL TRUCK UP GRADE ON HAUL ROAD, WAS HIT BY 35 TON HAUL TRUCK TRAVELING DOWN GRADE ON SAME HAUL ROAD. MINOR DAMAGE TO ONE TRUCK. HAUL ROAD ON SURFACE NEAR PLANT #5 . ROAD WAS WET AND VERY SLICK.

SADDLE FELL FROM PACKER, EE WAS ATTEMPTING TO PUT IT BACK INTO POSITION. WHILE HIS FINGER WAS UNDER THE SADDLE, HE HIT THE OPERATION LEVER, CAUSING THE SADDLE TO MOVE CATCHING HIS FINGER BETWE EN THE SADDLE AND THE SUPPORT BRACKET.

EE WAS LYING ON THE FLOOR, PARTIALLY UNDERNEATH THE CARDBOARD SLIPSHEET DISPENSER AT PLANT 4, ATTEMPTING CLEAN UP ACTIVITIES. HIS FEET WERE POSITIONED IN THE ADJACENT TRAFFIC AISLE. SAFETY SHOES PREVENTED MORE SEVERE INJURIES.
