Adjusting saddle on 255 packer, saddle started to slid towards the floor, employee attempted to catch it. Right thumb caught in pinch point, resulting in cut requiring 13 stitches at local ER.
Employee wiped face and rubbed unidentified foreign object into right eye. Treated by company doctor as well as eye specialist.
EE was making a delivery to the truck shop. When he stepped down from the cab of the truck, his foot turned under him twisting his ankle.
Two maintenance employees were attempting to tighten a pressure gage, both were using wrenches. One wrench slipped and struck the injured employee over the right eye, resulting in small cut. Injury was treated at local ER, 3 stitches were required.
Employee was walking on catwalk near Dust collector 42 at Plant 4, twisted left ankle resulting in minor strain.
Employee was attempting to lift a 50# bag off the reject line and strained his lower back. Treated at local clinic and placed on restricted duty.

Employee took one step down stairs inside Plant 3, woke up on the stair landing, does not remember falling or how he got to landing. Had small contusion on left elbow. Was treated at local ER for elbow and tests to determine why he fainted.

Climbing down off tanker truck, stepped on gravel, twisted ankle which led to twisting of lower back.
Employee was attempting to open a window in the electrical shop. He pushed on the window frame and his left hand broke a pane which cut his left palm. Six stitches required for the cut.

Employee was stepping down off truck at Plant 13 loading area, slipped in loose gravel and twisted right ankle.
Employee was attempting to clear a rock hung up in the crusher feeder with a steel pry bar. The bar slipped and caught his left thumb between the bar and the edge of the crusher box. Cut to end of thumb and chip to bone in the last joint. Thumb was x-rayed, bandaged and placed in splint.
Employee was removing a piece of rock from the railroad track area between Plant 5 and Plant 13. Picked up rock, copperhead snake was underneath the rock, employee was bitten on the right thumb. Taken to hospital for treatment.
Employee was opening lid on customer tanker trailer, pulling up on cam on lid, felt pop in right wrist.
Employee was climbing down ladder on rear of truck trailer, slipped on third step from bottom, fell three feet landing on platform on rear of trailer. Bruise to upper chest.

Employee was using a cordless drill to tighten a screw. Drill twisted in his hand , cutting his middle finger on his right hand against a fan housing. Taken to ER and received 7 stitches. No work restrictions.
Employee was leaning over to place a pallet on a bagging station and dust blew into his right eye. He was wearing safety glasses at the time. First aid was administered, but he still required eye washing at the local ER.
Employee was beginning to work on bearing on tube mill #3, relieved water pressure initially, started to relieve water pressure again, water line came loose and left forearm was burned by hot water.
Employee was placing a pallet on the semi bulk bagger and felt pain in his lower back
Employee was working on #2 Tube Mill, twisting and turning performing maintenance work, strained lower back sometime after lunch. No exact time given for accident.
Employee was sitting on catwalk near Plant 5 screens, throwing spilled rock off catwalk. When he stood up he felt a tightness in his lower back. Did not report to supervisor until 10/03/2005 at 11:00 am. Went to company doctor on 10/06/2005 and was placed on restricted duty ( no lifting over 20 #).
Supervisor was walking up stairs at Plant 5, walked under low steel beam, stood up thinking he was clear and struck his head on the bottom of the beam. He was wearing a hardhat at the time.
Employee was holding a metal rod while another employee was striking with a hammer trying to free up a machine part. Hammer struck employee's pinky finger on his right hand, resulting in two small fractures
Employee was using a hoist and a come-a-long to lift and position a air sifter. The come-a-long slipped pinching his right index finger.
Employee was stepping off ladder of tanker truck onto the ground and turned left foot.
Loading trucks. Employee was pulling on a latch while opening a lid and felt pain in the right rib area.
Employee was bagging super sacks (2000# bags). When he released the holding straps, the filled bag began to lean and roll towards him. He attempted to manually hold the bag in place, it pushed him to the concrete floor, straining his lower back.
73 year old employee walking down steps near CS-11 bagger, stepped from first step to concrete floor, twisted left foot. Hairline fractures in left ankle and left foot.
Employee was using a come-along to lift a roll of conveyor belting. Other miners attempted to slide the roll into position. The come-along slipped out and the chain struck the injured employee in the head. Required 6 stitches in his left earlobe and 3 stitches behind his left ear.
Employee was in a manbasket attached to a forklift. The basket was 5 feet above the ground. While lowering the basket, the forks stuck momentarily, the chain continued to move, then the forks fell 3-1/2 feet stopping the basket 1-1/2 feet above the ground. The sudden stop casued the employee to strike his throat on the manbasket handrail.
EE was cleaning out from under palletizer at Plant 5. EE raised up and hit hard hat on steel framing of machine. Reported to supervisor on 3/24, requested medical treatment at that time. Strain to neck.
Employee was working on an airlock, hit pinky finger on left hand with hammer. Cut to tip of finger, requiring 4 stitches. Returned to work full duty.
Employee operates a Trackmobile. Over several days, developed a pain in the left knee while performing normal operator duties. Informed manager on 2/22/2005 of the pain and requested medical treatment. No record of an actual injury date. Doctor diagnosed slight strain to left knee. Returned to work same day.
Employee was walking down stairs in shop(4 steps) from restroom, missed a step and fell to the floor, approx 28 inches. Caught himself by his left hand, bruised/strained middle finger on left hand.
Employee was adjusting the tracking on a newly installed conyeyor belt, right hand slipped and was caught between the belt and a small tension roller. Cut to the right pinky finger (four stitches) and abrasions to the back of the right hand.
EE WAS GETTING OFF ROOF INTO MAN BASKET AND UNHOOKED SAFETY HARNESS BEFORE HOOKING INTO MAN BASKET. EE WAS ON UNSTABLE ROOF SHEETING IN DANGER TAPE AREA. ROOF SHEETING GAVE WAY AND EE FELL TO GROUND, APPROX. 18'8"".
Employee was shoveling product off a catwalk area, stepped backwards and fell through an opening in the expanded metal flooring. Fell 11 feet to the ground below, spraining his right shoulder
Supervisor were traveling thru the plant 4 parking lot area. they noticed adult male lying on the grount near a flat bed truck. he stated he had fall off his truck to the ground.he climbed his truck cab to sit in the air conditioning and thought he would feel better 30 min later request medical treatment have pain back. ambulance picked him up. he is not member of property.
Employee stepped of concrete ramp at a height of 18 inches to ground below, feet slipped and he fell on his lower back and hip. Continued to work with no problems until requesting medical treatment on 7/21. Visited doctor on 7/23, was given work restrictions and released back to work.
Employee developed an infection in both ear canals, aggrevated by wearing ear plugs. Employee will wear ear muffs in the future.

EMPLOYEE WAS DRIVING 25 TON HAUL TRUCK UP GRADE ON HAUL ROAD, WAS HIT BY 35 TON HAUL TRUCK TRAVELING DOWN GRADE ON SAME HAUL ROAD. MINOR DAMAGE TO ONE TRUCK. HAUL ROAD ON SURFACE NEAR PLANT #5 . ROAD WAS WET AND VERY SLICK.
EMPLOYEE WAS HOLDING PLOW TIPS ON AN ANVIL. ANOTHER EMPLOYEE WAS HAMMERING THE TIPS, HIT PLOW TIP AT AN ANGLE CAUSING TIP TO TWIST. TWISTING ACTION PULLED BACK MUSCLE OF EMPLOYEE HOLDING TIP.
SADDLE FELL FROM PACKER, EE WAS ATTEMPTING TO PUT IT BACK INTO POSITION. WHILE HIS FINGER WAS UNDER THE SADDLE, HE HIT THE OPERATION LEVER, CAUSING THE SADDLE TO MOVE CATCHING HIS FINGER BETWE EN THE SADDLE AND THE SUPPORT BRACKET.
EE WAS ATTEMPTING TO MOVE A 50# BAG THAT HAD JAMMED ON THE CONVEYOR OF THE PLANT 2 PALLETIZER. HE FELT A PULL IN HIS LOWER BACK, DID NOT REPORT THE INCIDENT IMMEDIATELY. FINALLY WENT FOR MEDI CAL EVALUATION ON 5/12, SLIGHT BACK MUSCLE STRAIN.
EE WAS LYING ON THE FLOOR, PARTIALLY UNDERNEATH THE CARDBOARD SLIPSHEET DISPENSER AT PLANT 4, ATTEMPTING CLEAN UP ACTIVITIES. HIS FEET WERE POSITIONED IN THE ADJACENT TRAFFIC AISLE. SAFETY SHOES PREVENTED MORE SEVERE INJURIES.
EE WAS WALKING DOWN STEPS IN PLANT 8 & TWISTED HIS RIGHT ANKLE.
EE TURNED TO WALK FROM PIT 2 LOADING DOCK AND TRIPPED OVER A WOOD PALLET. CAUGHT HIMSELF WITH HIS RIGHT HAND AS HE FELL AND INJURED RIGHT WRIST.
EMPLOYEE WAS WALKING FROM THE MILL OPERATOR ROOM TO MAKE HIS ""ROUNDS"" TO CHECK EQUIPMENT AND WALKED UNDER A BLOWER THAT WAS BEING LIFTED BY MAINTENANCE EMPLOYEES TO INSTALL ON MILL #3. THE BLO WER FELL AND STRUCK THE EMPLOYEE ON THE LEFT FOOT BREAKING TWO TOWS. EMPLOYEE WAS WEARING SAFETY SHOES.
EE WAS ADJUSTING ELECTRIC EYE ON PALLETIZER IN PLANT #2. PUT MACHINE IN MANUAL, RESTORED POWER AND ATTEMPTED TO CYCLE THE MACHINE TO CHECK ALIGNMENT. EE PUT HIS LT. HAND TOO CLOSE TO THE MOVI NG PLATE AND RECEIVED CUTS TO THE MIDDLE FINGERS ON HIS LT. HAND.
EE WAS LOADING A BUILK TRUCK WITH 40-200, WHEN DUST BLEW INTO RIGHT EYE. EE AND SUPERVISOR WASHED OUT HIS EYE AND EE CONTINUED TO WORK, EYE CONTINUED TO BE IRRITATED, VISITED DOCTOR ON 7/30, E YE WASHED AND DROPS PRESCRIBED.
EE WAS USING A PIPE WRENCH TRYING TO LOOSEN A FEEDER MOTOR. THE WRENCH SLIPPED, STRIKING HIS FOREHEAD JUST BELOW HIS HARD HAT. HE WAS TAKEN TO THE LOCAL ER, GOT 5 STITCHES & RETURNED TO WORK.
EMPLOYEE WAS WALKING ALONG CONCRETE PAD NEAR RO SILO, SLIPPED AND FELL. HE ATTEMPTED TO CATCH HIMSELF WITH HIS RIGHT HAND ON THE GROUND, FRACTURED RIGHT THUMB. HE WAS TAKEN TO THE LOCAL E.R.
EE WAS OPERATING THE #10 PACKER. THE BAG SADDLE STUCK IN THE TILTED POSITION. HE TRYIED TO MOVE IT & HIS RIGHT MIDDLE FINGERS WERE MASHED. HE RECEIVED TREATMENT AT THE ER & RETURNED TO WORK.
EMPLOYEE WAS HOLDING NUT ON RAYMOND MILL SPIDER WITH WRENCH AS FELLOW EMPLOYEE TIGHTENED BOLT W/PNEUMATIC WRENCH. AS PNEUMATIC WRENCH WAS ENGAGED THE NUT/WRENCH SPUN, PINCHING THUMB ON RIGHT HAND AGAINST SIDE OF HOUSING.
EE WAS SITTING IN CAB P/U WHEN SOMETHING CAME FROM A/C VENT & WENT INTO LEFT EYE.
EMPLOYEE WAS CLIMBING OUT OF RR HOPPER CAR AND STRUCK EDGE OF MAN HOLE OPENING, KNOCKING OFF HARD HAT. HE THEN LEANED FORWARD AND STRUCK PRODUCT SPOUT HANDLE, CAUSING LACERATION TO HEAD.
EMPLOYEE WAS CLIMBING ONTO TOP OF BULK TANK TRUCK IN ORDER TO LOAD. AS HE GOT TO TOP OF TANK THE DRIVER IMMEDIATELY MOVED TRUCK FORWARD BEFORE EMPLOYEE COULD HOOK UP FALL PROTECTION. EMPLOYEE GRABBED SAFETY CABLE WITH LEFT HAND TO PREVENT FALLING AND STRAINED SHOULDER.
EE WAS INSTALLING TY ROCK SCREEN FOR PRIMARY CRUSHER. HE WAS LIFTING/PUSHING SCREEN BY HAND INTO PLACE. AS SCREEN DROPPED INTO FRAME, IT CAUGHT/PINCHED HEEL OF RIGHT HAND, REQUIRING STITCHES.
EMPLOYEE WAS STEPPING DOWN ONTO PLATFORM AT PALLETIZER WHEN HE SLIPPED, TWISTING OR SPRAINING HIS RIGHT KNEE.
EE WAS ATTEMPTING TO CUT A RUBBER HOSE WITH POCKET KNIFE WHEN BLADE FOLDED, CUTTING FINGER.
EE REACHED INTO MOVING SCREW CONVEYOR TO GET A PRODUCT SAMPLE AND THE MOVING SCREW FLIGHT CAUGHT THE END OF HIS RIGHT MIDDLE FINGER, RESULTING IN A MASHED FINGER.
