Employee was crawling out of CR02 chute after using a jack hammer to break rock plugging chute. Employee was pushing the jackhammer in front of EE. it caught a rock causing the bit end to pop up towards the employee, striking EE in the teeth. resulting in a broken tooth.

As the employee was ascending the ladder, felt a pop in left shoulder
Employee picked up a plywood bulkhead for a concrete form and when they turned to put it in place it caused a strain in their back.
When they placed a hand up to stop the door/panel from slamming, their left ring finger went into the hinge section. When the door closed it caused injury.
Employee was picking up a 5-gallon bucket of ore one handed when they felt a pop and pain in right shoulder

On 05/08/25 employee struck in arm by foreign object did not report the incident. The following week the incident was reported. Foreign body removed. 4 stitches received. Notified 05/27/2025 that the injury was accepted as an industrial injury.
Injured employee was spinning a pulley sheave by hand to remove an impeller and caught two of EE's left fingers in the belt. EE's left middle finger was badly lacerated and EE's left ring finger sustained a fracture.
While conducting a team lift of a front end of a light plant employee got their finger pinched in between the tongue and the ground, sustaining a fracture/break to the tip right index finger. Employee was evaluated at the Occupational clinic and allowed to return and resume normal work activities, once finger was cleaned and splinted.
Employee was cleaning the lime storage tank due to faulty work boot, employee used someone else's PPE and failed to inspect it prior to use, resulting in blisters, treated as first aid and on 10/24/2024 work restricted duty.
The employee was working on changing out EGR coolers on the engine of the MM485 water truck. In the process of removing the passenger side EGR cooler employee had arms out and had good control over the cooler (about 40 lbs). As EE was bringing it down, EE heard and felt a pop in their left forearm/ elbow area.
Upon pulling the cord, the cord jerked EE forward causing a popping sensation in EE's right arm resulting in a right wrist strain.
The Crane operator was lifting a down line into place. EE placed hand on top of the down line, and EE thought could remove hand in time to make the connection.
The employee placed a deck bushing over the bit sub pin. The sub slid down the mammoth wrench approximately 20"-24" landing on the employee's right foot.
The employee was cleaning up 1/2" expanded steel plate scrape pieces, when they picked up one of the pieces weighing estimated 50-55 pounds one side slipped and dropped landing on a finger and pinching it between the plate and the welding table.
Employee was loading toolbox into truck bed then felt a pop in R shoulder. Employee continued to load more toolboxes and pain increased.
Employee was moving a spicket at the tailings Dam, as EE was proceeding with the task a wheel on the spicket debris came out from the center of it into EE's left eye. EE was evaluated at a hospital and was given antibiotic eye drops.
Employee was doing a cathode press while adjusting the cathode, the copper bar released from the strong back arm striking right forearm sustaining a laceration that required 4 sutures.
While removing an access panel from a wheel motor the panel came off and fell before able to secure. The Panel struck left hand resulting in a fracture of the 5th digit.
An Asarco employee stated that when EE and the crew were working on installing a tramp cylinder on the #2 standard crusher. While lifting the cylinder into position the cylinder swung pinching EE's finger between the cylinder and the mainframe of the crusher. This caused a cut to the finger. (PPE and leather gloves were worn at the time of the incident).
Employee was off loading pipe from flat bed trailer when pipes band broke, causing it to break free of its restraining devices and roll off trailer, striking employee's right shoulder.
A Driller, in training, was cleaning the windows on the outside of the operator's cab and when Driller was returning into the cab, shut the door slamming left thumb between the door and the door jam.
Welder bent down to grab the dutchman plate rigging and stood up fast hitting the top of hardhat into the bottom of the knuckle boom causing the welder to fall back to the ground. After welder stood back up, felt pain in lower back. This did not become MSHA reportable until March 11, 2023.
IE and another EE were unloading a full oxygen tank at the warehouse. Other EE was on the ground pulling the tank and IE was pushing the tank off of the bed of the truck. IE was holding on to the tanks cap. The tanks cap slammed on the lip of the pickup truck bed while IE was holding on to it, pinning IE's right index finger.
Employee was installing glass fuse into Power Transformer by pushing the fuse by the glass instead of the metal not using gloves. Glass Fuse broke lacerating the employee right hand.
Secondary Crusher Helper was finishing up clean up when EE walked underneath piping connected to a vacuum truck while another operator was disconnecting it from above. After being disconnected, the pipe fell hitting the Helper on the right shoulder as EE was walking underneath it.
The injured employee was removing a belt from a pulley. As another employee rotated the equipment, the injured employee got their glove and finger caught in the belt and pulley. The injured employee sustained an avulsion-type wound on the left index finger. EE was treated at an emergency room and received sutures to close the wound.
The injured employee was using a battery operated impact to replace liners. EE needed a replacement battery and another employee attempted to slide it down along the liners. The battery caught the edge of a liner and bounced out striking the injured employee on the nose and cheek causing a fracture in the nose.
Employee was attempting to move pothead into position to pull off more cable to reach inside the building. While turning EE stumbled and the pothead slipped in EE's grip, slicing the left palm. EE then fell forward dropping the pothead cutting the skin on inside of left knee.
Pre-oping a drillers truck and closed the hood of the truck on their right pinky finger.
Moving radial stacker, EE's left index finger got stuck when EE had tension on the cable and EE's left index finger got caught in between the shackle and choker.
Cut open right thumb by trying to open a 50# can of 7018 welding rod.

Employee was assisting crane man while lifting cathodes from the floor when employee felt a sharp pain in left elbow. 12/9/21 is date of injury restrictions were applied on 12/14/21 with no heavy lifting of left arm only.
Employee was performing the monthly inspection on a fire extinguisher, while putting the extinguisher back into the bracket employee's hand caught part of the bracket causing a cut.

Employee Was Removing Driver Side Hoist Cylinder from haul truck. The hoist came down on the drift and employee caught right middle finger in pinch point causing a laceration to right middle finger 7 sutures.
Employee was lifting guard to load in the hopper, when as EE lifted EE twisted and felt a pain in EE's back.
Employee was trying to open product valve on trailer from EE's knees and then decided to sit down to get more leverage and opened product valve. Employee stated that EE applied full force to get valve to move. When employee bent down to open another valve 10 minutes later EE felt pain in EE's back.
Employee conducted Pre Op, front driver door on van struck and hit employee on right torso putting strain on left knee.
Employee was performing an inspection on the lime mill, as employees was performing inspection they noticed the floor was getting flooded with water mixed with lime. Employee then got the Submersible pump and tried to start it, they then noticed it wasn't working. Employee went to move the pump when the hose came off and lime splashed into left eye.
Employee was backing off the rock breaker when they felt a tug on pant leg. Employee examined the leg and noticed a metal splinter. Splinter was removed and required one suture.
Employee was doing blast hole sampling when allegedly a small piece of rock became embedded in left elbow causing irritation. Employee did not report injury until 5/26/2020. Employee did not become Lost Time until surgery 6/17/2020.
Employee went to move a traffic cone from out of the street and twisted a knee.
EE was placing 4 cable bridal ring onto crane hook. Crane hook safety latch pinched right middle finger.
Employee was removing right wheel tires on Haul Truck when arm was pinched between reaction arm and wheel assembly.

Employee was replacing bags in Number 3 Filter. As employee lifted and removed each sector and cut off each bag and replaced it with a new filter bag employee felt pain on left side of back.
