EE was stepping off the Haul Truck ladder onto uneven ground, when EE felt and heard a loud pop, and states EE felt instant pain.

Employee was picking up a 5-gallon bucket of ore one handed when they felt a pop and pain in right shoulder

Employee was walking in the tie down area around the water truck, when they slipped and landed on their right wrist, resulting in a fracture. Employee was placed on restricted duty.

Employee lost EE's footing and stumbled causing EE to lacerate EE's head on a steel window frame of a connex box. Employee received 6 Staples

Employee was walking in a dim area when EE tripped and fell over forks from a forklift, resulting in right shoulder pain. On 4/22/2025, employee was placed on Restrictive Duty

Employee was using 1 inch impact gun on bolt under truck. Bolt was not coming off causing the employee to adjust position. As employee was adjusting the impact gun it bounced back hitting employees face causing a small chip top of *R* tooth. 4/10/25 EE did elect to repair tooth, dental work completed

While conducting a team lift of a front end of a light plant employee got their finger pinched in between the tongue and the ground, sustaining a fracture/break to the tip right index finger. Employee was evaluated at the Occupational clinic and allowed to return and resume normal work activities, once finger was cleaned and splinted.

Employee hooked toe under next to last step on stairs and fell on knees causing abrasions and swelling. Swelling and soreness in left knee is impairing mobility.

Employee was pulling on a cable when another employee stepped on the cable causing the cable to pull EE to the ground. When EE fell, EE fell with majority of EE's weight on EE's left side injuring EE's left shoulder. 2/27/2025 Surgical Repair was required

Using hytorc gun on loader when it came loose striking the L middle finger causing laceration and fracture to the finger.

Chipping stuck copper off plate when misjudged momentum caused L thumb to be struck by hammer. L Thumb was struck between hammer and plate. L Thumb 1/2in by 1/2in laceration. Light bruising. Light swelling.

Employee was loading toolbox into truck bed then felt a pop in R shoulder. Employee continued to load more toolboxes and pain increased.

Employee working on haul truck 552 when right index finger was struck by moving air conditioning belt. Causing a scratch/abrasion.

Employee was backing into 579 shovel in HT552 to be loaded when the dipper bumped the truck jarring the driver. treated as first aid. On 11/20/2023 company was notified recent symptoms are related to the work injury and physical therapy has been ordered.

employee was stepping on to the ground from the last step of a haul truck hydraulic ladder at the end of EE's shift. EE states EE felt pain in EE's left hip. The ladder retracted close to the ground leaving the last step shorter than the rest

A Driller, in training, was cleaning the windows on the outside of the operator's cab and when Driller was returning into the cab, shut the door slamming left thumb between the door and the door jam.

Employee going to work area to check bolts on valve, when EE tripped over a tracer wire attached to HDPE pipe. Employee was holding battery powered drill when EE fell to the ground pinching right ring finger between rock & drill causing injury to EE's fingernail. The employee's fingernail was pulled up but was not removed and there was no other injuries.

Employee was in the process of removing adapters off of the 368 bucket when a J- hook they were using stretched out and came off the adapter striking the employee in the left forearm.

Employee was climbing up the ladder of the acid trailer to unhook the airline in the crows nest after unloading. EE lost balance as EE was climbing up the ladder and slipped while trying to grab the bar/railing. EE did not fall all the way down the ladder but caught self on the ladder while falling causing strain on EE's body.

Employee was lifting guard to load in the hopper, when as EE lifted EE twisted and felt a pain in EE's back.

Employees were repairing the latch bar on the shovel dipper. The employee was prying with a bar to loosen the piece of the bar. The dipper door moved and pinched off the fingers on the employee's left hand.

Employee was using a lift to moving power cable up to the cross member when it came loose and pinched EE's finger against the handrail on the lift.

Employee was walking down the stairs at the SXEW when EE slipped and struck knee on the handrail.

Employee was doing blast hole sampling when allegedly a small piece of rock became embedded in left elbow causing irritation. Employee did not report injury until 5/26/2020. Employee did not become Lost Time until surgery 6/17/2020.

Employee was stripping copper. When EE squatted down to pick up an air hammer and felt pain in EE's right knee.

Employee was cutting sandbags and the knife employee was using slipped off the material and employee cut left hand.

Truck shop employee fractured pinky finger as the employee was removing a starter from a pickup truck.

Employee was walking through some mud and debris on the wash pad when EE slipped and twisted hurting EE's back.

Employee was hand stripping copper in the stripping station using a bar. The bar slipped and hit the employee in the chin. EE received 9 stitches.

Employee was swinging a double jack to drive out a pin on 324 grader. EE needed to switch to swinging left handed because of the awkwardness of pin. Later in the day EE started to feel pain in EE's lower back. The employee did not report the injury until the next day.

Employee stepped off a bundle of copper and rolled ankle.

Employee was walking and tripped on a rock and fell. They reached out their right hand as they fell, causing a broken finger as they landed.

Employee was using a 6lb hammer to knock mud off of a haul truck and afterwards felt pain in EE's right elbow.

While drilling out holes for fender with mag drill, the employee stepped on the cord, unplugging the mag drill causing the drill to demagnetize falling on employee's hand.

The joystick in the shovel failed and the operator could not stop the loaded dipper, and the dipper hit the haul truck, jarring the employee. Employee did not seek medical attention until April 20.

Employee was climbing off of equipment when employee knee had pain. Employee did not seek medical attention until April 23.

Employee was checking edge strips on the blanks, when one fell, cutting on the back of EE's right hand. Employee received 2 stitches.

Employee was moving a 4"" pipeline when EE felt pain in the lower back.

Employee felt a pop in the right elbow when driving in a grounding rod for testing. Employee did not seek medical attention until 2/7/2019.

Truck shop employee was working on haul truck 551 right side front tie rod. When raising the rod into place, the tie rod slipped off the jack and caught EE's left hand between the tie rod and jack. Employee received 13 stiches.

Employee miss stepped on the cell line during harvest and fell, rolling right ankle.

Employee was removing the driveline from under a service truck, when it came apart and struck employee above the right eye requiring 3 stitches.

Employee was working under the loader. As EE came out from under the machine EE felt a pop in the knee.

Employee was installing some hoses on the mast of a drill, when they got stuck EE pried on the hoses with a small pry bar to move them and felt pain in EE's right shoulder.

Employee was washing water truck when EE slipped on the stair and hit EE's knee on the step.

Employee tripped and fell on left knee.

Employee lost grip as climbed out of the engine compartment and fell against the deck of the truck.

Employee was attempting to move a light plant. EE fingers were between the hitch and truck when the jack failed the trailer fell pinching ee fingers.

Employee stepped off the forklift and rolled ankle.

Employee grabbed at a pair of tongs as they fell cutting their right middle finger requiring 4 stiches.

Employee caught finger between a lead anode when it fell onto a anode storage rack. EE received a finger tip fracture and several stitches.

Employee was removing teeth from a shovel dipper when EE's finger got pinched between teeth as EE set it on a pallet.

10/11/2016 employee dismounted the dozer and turned to take a step and felt left knee pop. Employee kept working, but knee continued to bother employee off and on. Employee opted for surgery and became a lost time 1/2/2017.

Employee was striking a copper started in the stripping machine when a bent piece of copper cut the employee on the left wrist requiring two stiches.

Employee was walking around shovel when slipped in some loose rocks. Put out hand to catch fall and cut left palm. Had three stiches.

Employee was using a bar to align cutting edges on a track dozer when EE heard/felt a pop in EE's elbow followed by pain, 3/11/2016. Referred to specialist, doctor took employee off of work 4/27.

Employee was tightening flange bolts on discharge valve when EE felt pain in EE's lower back.

Employee alleges January 14, 2016 incident caused an injury to back that EE was not aware of. We feel this incident was not caused by an occurrence at work and is not work related. We are filing this report only to terminate a citation. We will contest this citation.

Employee was cleaning up around lathe work area when he cut his finger on some metal shavings.

EE claimed with the Industrial Commission a back injury 6/6/2015. There was no report of injury to us, and no cause of injury. We denied the claim. EE had surgery. We sent ee to an Independent Medical Exam and it was ruled compensable. We received the report 10/16/2015 from the IME doctor.

Employee was attempting to remove the dipstick to check the oil during his pre-operational inspection of the haul truck. While he was struggling to remove the dipstick he felt pain in his shoulder.

Employee was working on placing the front spindle assembly on a haul truck. He was using a pry bar to turn the assembly when he felt sudden pain in his left shoulder. Employee was taken off work 9/30/2014

Employee was lifting some dunnage when he felt a pop in his right elbow. Employee had surgery 7/24/2014 and became a lost time.

Employee was using the boarding ladder to climb down the shovel and slipped and fell to the ground.

Employee was operating a backhoe when he felt some discomfort in his groin area, around 11:30. The employee had lunch and went back to work. At 2:00 the employee requested medical care. Cause of injury is questionable.

The employee was opening oil filters to check for metal and cut his thumb with a knife.

Employee was laying over a 10"" piece of pipe working with a timber support. When he twisted he felt a pop and had pain on his right side at the ribs. Employee did not seek medical attention until March 21st.

Employee was working on the cutting edge on 834 rubber tire dozer. He had the moldboard suspended and when he wiped some dirt off the back side he cut his forearm.

Employee was dismounting road grader at the 2890 tie down and missed the last step causing him to fall backwards onto the ground.

Employee was troubleshooting the PP01 cables and erroneously touched the high voltage pothead and contacted one phase of 4160 volts with his right hand.

The employee was steam cleaning the drill. He was up on the mast of the drill. As he went to squeeze the handle to activate the cleaner, the nozzle jumped and he lost control of the wand. The high pressure water cut the tip of his boot and sprayed hot water onto his left foot causing 2nd degree burns to his foot.

Employee was assaying blast hole samples and she starting having pain in her arms.

Employee was closing the door to the warehouse when a gust of wind blew the door shut on her finger.

Employee climbed out from under a rubber tire dozer and stood up. The sun was in his eyes and he lost his balance. He fell and his right hand struck a lock ring from a big tire that was laying on the ground.

Employee was climbing up and down ladders all night working on the 944 loader. When he descended a ladder he felt a pop in his knee. Employee went in for surgery on knee 12/20/2012, becoming a lost time injury.

Employee was helping lift a shovel cable connector (pothead) and felt pain in left shoulder. Became a lost time when he went in for surgery 11/08/2012

Employee was working on a loader in a tight space for an extended period of time causing a pinched nerve in his neck.

Employee was being loaded at the 579 shovel when the shovel dipper clipped the bed of the haul truck. The truck jolted causing the seatbelt to tighten up aggravating abdominal problems of the operator.

Employee was working on the blast pattern when he slipped. As he fell he caught himself with the right arm causing pain to his right shoulder. Employee worked normal job. Went in for surgery 10/22/2012, became lost time.

Employee was standing on the right rear platform of the rubber tire dozer. When he turned around he lost his balance and fell approximately 4 feet landing on his shoulder and head. Employee has a broken collar bone.

Employee was changing out main crank pulley on haul truck when he pinched his finger between the impact drive and the frame of the truck.

Skeleton remains were found on mine property.

EE was cutting a piece of plate that was near the remaining bulk of the plate he was stepping backwards and his foot stepped into a gap on the plate, causing him to fall to his side with his foot lodged in the hole he had stepped in.

Employee was hooking up a pipe fusing machine to a pick-up truck when the machine moved forward and pinched the employees pinky finger of his left hand between the hitch and the receiver.

The operator of water truck 498 lost control and struck a berm. The water truck flipped over. Accident still under investigation.

Employee was placing grade indicators and had climbed up on a berm. When coming down, a rock shifted and employee adjusted his weight to steady himself. He felt a pain in his lower back.

Employee entered lube room of the #575 electric shovel (P&H 2300) around 7:30pm. The door closed behind him. The latch was dirty and would not open from inside.

Employee was operating rubber tire dozer when sometime during the shift she hit a rough spot causing some discomfort to her neck.

While using a Cup Cutter tool to prepare an injury in an Earthmover Tire for repair the cup cutter bound up in the rubber and slipped to the side striking the associate on the right knee resulting in a laceration that required sutures. Associate returned to regular duty the next shift 9-1-2010.

Pinched and cut left ring finger with sharp piece of metal.

Employee on several strenuousness projects during the day and stayed over on over time until projects were completed. Employee the next day reported he had pain in his lower back.

Employee states that while working tripping out a hole, the pipe wrench he was using slipped and snapped back hitting his left open palm.

Employee states that while pulling rods at jobsite, his left index finger was crushed between bar and pipe wrench. Sustained laceration and fracture of finger.

Employee was reaching up to climb on a forklift when he felt popping and snapping in his shoulder.

Employee was hit in the forehead with u-joint cap. Employee was under 834 wheel loader installing the rear drive shaft. He was holding up the driveshaft into position so that another tech could start driveshaft bolts. His hard hat fell off and then the cap for u-joint came off and struck employee on forehead.

Employee was using an electric power snake to clean out a slow drain. During the process, the snake broke, cutting employees arm.

Employee was removing teeth from the Cat 994F loader when a sliver of metal struck him on the right knee cutting him.

Employee was in the process of loading holes for a shot when he stepped on the side of a rock and rolled his right ankle.

Employee was cleaning a track dozer at the washrack using a hose when he reported his right shoulder was bothering him. Employee did not seek medical attention right away.

Employee was working on pressing off the belt sheave on a shaft. As the employee hit the sheave, he smashed his index finger between the hammer handle and the sheave.

Employee was climbing down a ladder access at a drill. At the landing there was a rock that caused his left leg to twist when he stepped on it. He twisted and he felt pain in his lower hip and back.

Employee was adjusting the reflector in the drop box of the stripping machine. Communication got confused between the workers and the flapper arms were inadvertently activated, pinning EE's arm in a pinch point.

Employee was removing dipper handle pin from shovel dipper when the pin got stuck. The employee moved the pin up and down to free the pin. The pin released unexpectedly and came out completely. The pin pinched the employee's tip of the right ring finger.

Employee was standing up in the back of the welding truck 428 when he slipped and fell off backwards landing on the shop floor.

Employee was en-route to dump, when the vehicle hit a pothole, jarring the employee. Employee did not report the incident until 12/09/2008. Seen Dr. and was released without restrictions. 12/16/2008 employee had follow up visit and was given restrictions.

Employee's last audiogram showed he had sustained a standard threshold shift.

Employee was moving a bottle of welding gas. He was off loading it from the truck when he lost control of the cylinder and as the cylinder fell it opened the valve and the employee was blasted with gas from the bottle.

Employee was bending over and down to place a red-head marker for a drill hole in a drill pattern. He felt pain in his lower back.

Employee was working to free a stuck cathode in the stripper.When the blank was freed from the copper,the machine advanced the blank starter sheet and also advanced the next cathode in line.As the machine advanced the blank, the loose copper sheet struck the employee in the back.

Employee was troubleshooting an electrical problem the switch house for the 579 shovel. When testing the high voltage side there was an arc flash and the employee was burned on his left hand.

Employee was reaching up with his right hand opening a valve when he felt a pain in his right eye. The pain was off and on all shift. When examined the doctor found a small object in his eye.

Employee was tighten loose plow bolts and replacing a missing pin when later in the shift his back began to bother him.

Employee was lifting a five-gallon oil jug to the deck of 583 drill. He felt a pop in his right shoulder. The shoulder pain got worse throughout the shift.

Using a ratacher tie-down strap to secure equipment to the bed of a broom truck, injured sustained a a cut to lower lip and broken tooth when the strap end came loose and struck injuried on the mouth.

While descending the steps from the control panel to the copper conveyor, the fiber-glass step gave away causing EE to fall through the opening up to his waist. The fall resulted in a scraped left shin, a sore right knee and a small cut to the left index finger.

At the ready line injured jumped over a puddle, landing on a rock and turning his right ankle and bruising his left knee.

While helping to remove broken concrete from the acid unloading station, injured sustained a corneal abrasion when a wind blown foreign body entered injured's left eye.

While removing a one-ton GMC left rear brake caliper, the caliper slipped from the EE's grasp pinching and fracturing the right ring finger. Injury occurred when finger was pinched between floor and caliper.

While operating #583 drill, operator looked down to his left at the drill stem then up to his right at the gauges and felt pain in the upper back. Restrictions were prescribed.

There was no incident involved. Our employee has suffered a standard threshold shift in his hearing. Notification confirming hearing loss was received on 2/6/08.

Employee caught finger in between gear.

While closing a 10"" water valve, injured's feet slipped and he lost his grip with the right hand and caught his weight with the left hand. This resulted in a left wrist strain.

Injured stepped down from approx one foot high concrete foundation and twisted his right ankle.

While using his left foot to reposition one of the lifting tines on a forklift, injured felt pain in the left lower back.

Injured was using a shovel to dig out a process water line, felt a ""pop"" in his left elbow. He was referred for a MRI.

While tipping the hood of a dump truck injured felt pain in the left sholder adn around left side. Physical therapy was prescribed.

While pulling on copper cathode to reposition it, the cathode fell on inj's right hand, resulting in a lacerated & fractured right long finger.

While moving cathode blanks around in the work area, injured felt a pain in the mid back. Light duty was prescribed.

While unreeling shovel power cable from a spool, injured ""yanked"" on the cable and felt a pop and pain in the right shoulder. Injured began losing time from work on 8-13-07.

While securing a load with a ratchet strap, injured felt a pull in his left wrist while pushing on the ratchet handle. Light duty was prescribed.

While placing his tool tray on bed of service truck injured felt pain in the lower back. Light duty was prescribed.

While stripping a thin copper cathode, the cathode fell striking & cutting injured's left thumb. Sutures were required.

During installation of the right track of a dozer, injured used a 16-pound hammer to reposition the zipper link. When the link was struck a small airborne piece of steel struck & embedded beneath the skin of his right abdomen.

While removing a 4 inch fusion machine from under a pipe, injured felt pain in his right wrist.

Injured pulled up shovel access ladder & it stuck causing injured to feel pain in lower back. Injured was returned to work ""regular duty"" on 3/21/07. On 4/23/07 ""light duty"" was prescribed, making injury reportable.

While walking up stairs in tankhouse injured stubbed his toe on a step. As he fell forward his left small finger struck a sharp piece of fiberglass on the mid rail of the handrail, resulting in a cut left small finger.

Injured was struck on right side of hard hat and neck when a new hoist cable & becket unwound from a spool. Injury occurred when injured cut the rope holding cable end to spool. Physical therapy was prescribed on 1/10/07.

While operating #336 rubber tire dozer, injured's left shoulder became sore. Injured went to ER and Dr placed left arm in sling.

While pulling on a 3/4 inch water hose, injured sustained a shoulder right dislocation. Injury occurred when hose hung up on a rock as injured was pulling it.

Injured, evading a swarm of bees, tripped and fell as he ran down an embankment toward his pickup. The fall resulted in a sore and bruised right shoulder. X-rays were negative. An MRI has been recommended.

While moving a 9 7/8"" drill bit on the drill deck the employee over-extended his body which resulted in back strain.

While taking amp reading, overloads failed and flashed across phase to phase causing an extreme arc resulting in flash to both eyes. Prescription drug was administered.

Injured using a crowbar to advance a copper cathode into the load-out hopper, sustained a bruise to the top of the right hand. Injury occurred when the falling cathode caught the crow bar tip, causing injured's hand to strike a near-by metal brace. Light duty was prescribed.

He stepped down a distance of 18"" and felt pain in his right knee.

While riding in passenger seat of a haul truck, injured sustained a bruised right knee when trainee stopped quickly and injured's knee contacted lower dash area. Injured did not seek medical attention until 5-1-06. Physical therapy perscribed.

Injured was using a wrench on a valve handle. The handle broke and pipe wrench struck injured's right lower jaw. Injured states he momentarily lost consciousness. Injury resulted in minor soft tissue swelling of lower jaw.

INJURED'S FOOT SLIPPED ON 2ND RUNG OF EXTENSION LADDER. INJURED BROKE THE FALL WITH HIS LEFT HAND, WHICH STRUCK & WAS LACERATED BY A 1/2"" ALL-THREAD BOLT. SUTURES WERE REQUIRED & LIGHT DUTY WAS PRESCRIBED.

INJURED STUMBLED ON A ROCK WHILE WALKING. AS HE REGAINED HIS BALANCE, INJURED FELT PAIN IN THE LOWER BACK.

Injured was installing a shovel gantry cable. A choker cable was wrapped around the gantry cable & lifted into position. A crane was used to swing & pull the cable into the gantry equalizer. Pulling caused the choker to slip from around the gantry cable & crane sheave block struck injured in back.

WHILE PULLING ON A PIECE OF COPPER CATHODE THE CATHODE MOVED, CUTTING THROUGH EE'S GLOVE AND STRIKING HIS RIGHT INDEX FINGER KNUCKLE. THIS RESULTED IN A SMALL CUT REQUIRING TWO SUTURES.

WHILE MOVING A COPPER CATHODE, INJURED'S GLOVE WAS PULLED FROM HIS HAND. HIS HAND THEN CONTACTED CATHODE RESULTING IN A LACERATION OF HIS RT. THUMB. THE WOUND WAS SUTURED.

WHILE OPERATING 577 SHOVEL, INJURED STATES THAT PAIN WAS FELT IN THE LEFT SHOULDER AND DOWNWARD THROUGH THE ARM. PHYSICAL THERAPY WAS PRESCRIBED.

When removing an 18 inch butterfly valve, the valve slipped, pinching and cutting injured's left index and long fingers. The distal tip of long finger was also fractured. Light duty was prescribed, but no work days were lost.

WHILE STANDING IN A PICKUP BED, INJURED STEPPED FORWARD TO PUSH ON A TRASH CAN. AS HE DID SO, HIS RIGHT KNEE COLLAPSED, RESULTING IN A TORN LATERAL COLLATERAL LIGAMENT. SUBSEQUENT MEDICAL OPINION DIAGNOSED: ""STRESS FRACTURE"" OF TIBILA PLATEAU.

WHILE WALKING UNDER A PARTIALLY OPEN WAREHOUSE ROLL-UP DOOR, THE FRONT PORTION OF THE INJURED'S HARD HAT STRUCK THE DOOR BOTTOM. INJURE'S HARD HAT, MADE CONTACT WITH SAFETY GLASSES WHICH CUT T HE BRIDGE OF INJURED'S NOSE. THE LACERATION REQUIRED TWO SUTURES.

AS INJURED WAS EXITING THE #578 SHOVEL, A WIND BLOWN FOREIGN BODY ENTERED INJURED'S RIGHT EYE. THIS RESULTED IN A CORNEAL ABRASION TO THE RT. EYE. PRESCRIPTION DRUGS WERE ADMINISTERED TO THE EYE.

EE PICKED UP DRILL BIT AND STRAINED HIS BACK.

INJURED, PERFORMING ELECTRICAL GROUND CHECKS ON THE LOWER JIB HOIST AT THE EAST END OF THE TANKHOUSE, SUSTAINED A CORNEAL ABRASION TO THE RIGHT EYE. INJURY OCCURRED WHEN THE JIB BEAM WAS MOVED IN FRONT OF A LARGE WALL FAN AND DUST/ DEBRIS WAS BLOWN FROM THE BEAM INTO THE INJURED'S EYE.

INJURED, FUSING HDPE PIPE, SUSTAINED A BRUISE TO THE RIGHT KNEE. INJURY OCCURRED WHEN THE 24 INCH PIPE BEING FUSED WAS LIFTED ALONG SIDE A 12 INCH LINE AND SPRUNG BACK TO THE STRAIGHT POSITION WHEN IT CLEARED THE 12 INCH LINE. AS IT SPRUNG BACK, IT STRUCK THE INJURED'S KNEE. LIGHT DUTY WAS PRESCRIBED.

INJURED, OPERATING A HAUL TRUCK WAS BACKING INTO THE RIGHT SIDE OF #578 SHOVEL TO GET LOADED. THE SHOVEL OPERATOR, WITH THE DIPPER POSITIONED IN THE AIR, ATTEMPTED TO SET THE HOIST BRAKE WITH HIS LEFT HAND. IN THE ATTEMPT TO SET THE HOIST BRAKE, THE BRAKE WAS UN-INTENTIONALLY RELEASED. AS THE SHOVEL OPERATOR RELEASED THE HOIST CONTROL THE DIPPER FREE-FELL STRIKING THE DUMP BOX.

USING A HOOK BAR TO RETRIEVE A STAINLESS STEEL CATHODE, INJURED SUSTAINED A LACERATION TO THE TOP AND BOTTOM SIDES OF HIS RIGHT RING FINGER. INJURY OCCURRED WHEN THE HOOK BAR SLIPPED AND INJURED'S FINGER STRUCK A NEAR BY RAILING SUTURES WERE REQUIRED.

WHILE USING A PRY BAR TO PRY A HOSE OUT OF THE WAY, THE INJURED SUSTAINED A CUT TO THE RIGHT THUMB. INJURY OCCURRED WHEN THE PRY BAR SLIPPED ALLOWING INJURED'S THUMB TO MAKE CONTACT WITH A SHE ET METAL EDGE OF THE DECK TOP DOOR OF AN 834 WHEEL DOZER.

WHILE PICKING UP A COPPER CATHODE, INJURED LOST HIS BALANCE, CAUSING A LOWER BACK STRAIN. INJURY DID NOT BECOME REPORTABLE UNTIL DOCTOR PRESCRIBED PHYSICAL THERAPY ON 9-23-02.

WHILE BACKING THE HAUL TRUCK IN FOR THE LOADER TO DROP A BUCKET OF MUCK INTO THE BED, THE BED STRUCK THE LOADER BUCKET JOSTLING THE TRUCK DRIVER. THIS RESULTED IN A CERVICAL AND THORACIC STRA IN. PHYSICAL THERAPY WAS PRESCRIBED.

AFTER OPERATING A DOZER ALL SHIFT, THE INJURED TOLD HIS SUPERVISOR THAT HIS BACK HURT. THE ALLEGED INJURY OCCURRED 8/14/02. THERE WAS NO SLIP, TRIP, NOR UNUSUAL EXERTION. STATING HE WAS STILL EXPERIENCING DISCOMFORT, THE INJURED SAW THE DOCTOR ON 10/15/02. THE DOCTOR PRESCRIBED PHYSICAL THERAPY, MAKING THE INCIDENT REPORTABLE.

OPERATOR STEPPED OFF D-10 DOZER TO EXAMINE HIS WORK AREA. UNAWARE OF AN OPEN MONITOR WELL, THE OPERATOR'S RIGHT LEG FELL INTO THE WELL HOLE. A ROCK FEL ALONGSIDE HIS LEG, WEDGING THE LEG INTO THE HOLE. MEDICAL OBSERVATION REVEALED A BRUISE ON LEG. INJURED RETURNED TO HIS NEXT SCHEDULED SHIFT.

WHILE HAMMERING A BUCKET LIP SHROUD A METAL CHIP FLEW FROM SHROUD, PUNCTURING INJURED'S LEFT INNER THIGH.

WHILE LIFTING A BAG OF FLOOR DRY, INJURED FELT PAIN IN THE RIGHT LOWER BACK. PHYSICAL THERAPY WAS PRESCRIBED.

WHILE BENDING, STOOPING & LIFTING A SHEET OF CATHODE COPPER, INJURED FELT PAIN IN THE LOWER BACK& BEGAN LOSING TIME FROM WORK ON 9/14/01.

WHILE OPERATING A RUBBER TIRE DOZER, INJURED SUSTAINED A TUFT FRACTURE OF THE RIGHT SMALL FINGER TIP. INJURY OCCURRED WHEN FINGER WAS PINCHED BETWEEN STEERING WHEEL SPOKE AND GEAR SHIFT LEVER.

INJURED, WORKING ON LEACH FIELD, FINISHED CUTTING A PIPE, TURNED TO WALK AWAY AND PLACED HIS LEFT FOOT ON A ROCK. THE ROCK ROLLED OUT FROM UNDER FOOT, CAUSING A LEFT KNEE SPRAIN. LIGHT DUTY WA S PRESCRIBED.

WHILE SITTING AT THE STRIPPING MAC;HINE CONSOLE, INJURED FELT A BURNING SENSATION INT HE LEFT EYE. PRESCRIPTION MEDICATION WAS USED TO TREAT FOREIGN BODY.

WHILE WALKING ON LEACH DUMP, INJURED TRIPPED AND CAUGHT HIMSELF, RESULTING IN A LOWER BACK MUSCLE STRAIN. ON 3/14/01 THE INJURED FELT THE LOWER BACK TIGHTEN. THE DR PRESCRIBED PHYSICAL THERAPY ON 3/14/01, MAKING THIS INCIDENT REPORTABLE.

THIS ILLNESS WAS FIRST REPORTED ON 2/6/01. SUBSEQUENT DIAGNOSIS WAS BILATERAL CARPAL TUNNEL SYNDROME WITH RIGHT HAND BEING THE MOST SEVERE. SURGERY WAS PERFORMED 7/13/01 & EE BEGAN LOSING TIME FROM WORK ON THAT DAY.

DURING MORNING BREAK, INJURED NOTICE AN IRRITATION IN THE RIGHT EYE. MEDICAL EVALUATION FOUND A FOREIGN BODY, WHICH WAS REMOVED. A ONE-TIME APPLICATION OF PRESCRIPTION MEDICATION WAS MADE.

USING A 3/4"" DRIVE IMPACT WRENCH TO REMOVE A TIRE, INJURED FELT A POP AND THEN PAIN IN HIS LEFT THUMB AND FOREARM.

WHILE DESCENDING EQUIPMENT ACCESS LADDER INJURED'S LEFT FOOT LANDED ON A ROCK CAUSING A SPRAINEDANKLE.

WHILE PULLING ON A FUEL HOSE, INJURED'S FEET SLIPPED AND INJURED FELL, LANDING ON HIS BACK. THIS RESULTED IN A LOWER BACK STRAIN AND PRESCRIBED PHYSICAL THERAPHY.

WHILE DRIVING A FORKLIFT OVER UNEVEN GROUND, INJURED STATES HE FELT PAIN IN THE LOWER BACK. INJURED RETURNED TO WORK WITH RESTRICTIONS.

WHILE SHOVELING DIRT INTO SLEEVES HE FELT HIS BACK POP. (INJURY DID NOT BECOME REPORTABLE UNTIL 10-3-00).

WHILE CLIMBING DOWN FROM A TRACK DOZER INJURED STEPPED ON A ROCK AND TWISTED HIS RIGHT KNEE.

AFTER MAKING A CUT OF A 1 1/2 INCH PIPE, THE PIPE END CAME OUT OF THE CHOP SAW WORK CLAMP, STRIKING AND FRACTURING INJURED'S LEFT RING FINGER TIP.
