As employee was adjusting the impact gun it bounced back hitting employees face causing a small chip top of *R* tooth. 4/10/25 EE did elect to repair tooth, dental work completed

While conducting a team lift of a front end of a light plant employee got their finger pinched in between the tongue and the ground, sustaining a fracture/break to the tip right index finger. Employee was evaluated at the Occupational clinic and allowed to return and resume normal work activities, once finger was cleaned and splinted.

Fall down stairs

Employee hooked toe under next to last step on stairs and fell on knees causing abrasions and swelling. Swelling and soreness in left knee is impairing mobility.

Employee was pulling on a cable when another employee stepped on the cable causing the cable to pull EE to the ground. When EE fell, EE fell with majority of EE's weight on EE's left side injuring EE's left shoulder. 2/27/2025 Surgical Repair was required

Employee going to work area to check bolts on valve, when EE tripped over a tracer wire attached to HDPE pipe. Employee was holding battery powered drill when EE fell to the ground pinching right ring finger between rock & drill causing injury to EE's fingernail. The employee's fingernail was pulled up but was not removed and there was no other injuries.

Employee was in the process of removing adapters off of the 368 bucket when a J- hook they were using stretched out and came off the adapter striking the employee in the left forearm.

Employee was climbing up the ladder of the acid trailer to unhook the airline in the crows nest after unloading. EE lost balance as EE was climbing up the ladder and slipped while trying to grab the bar/railing. EE did not fall all the way down the ladder but caught self on the ladder while falling causing strain on EE's body.

Employee was swinging a double jack to drive out a pin on 324 grader. EE needed to switch to swinging left handed because of the awkwardness of pin. Later in the day EE started to feel pain in EE's lower back. The employee did not report the injury until the next day.

Employee was using a bar to align cutting edges on a track dozer when EE heard/felt a pop in EE's elbow followed by pain, 3/11/2016. Referred to specialist, doctor took employee off of work 4/27.

Employee alleges January 14, 2016 incident caused an injury to back that EE was not aware of. We feel this incident was not caused by an occurrence at work and is not work related. We are filing this report only to terminate a citation. We will contest this citation.

Employee was cleaning up around lathe work area when he cut his finger on some metal shavings.

EE claimed with the Industrial Commission a back injury 6/6/2015. There was no report of injury to us, and no cause of injury. We denied the claim. EE had surgery. We sent ee to an Independent Medical Exam and it was ruled compensable. We received the report 10/16/2015 from the IME doctor.

Employee was attempting to remove the dipstick to check the oil during his pre-operational inspection of the haul truck. While he was struggling to remove the dipstick he felt pain in his shoulder.

Employee was working on placing the front spindle assembly on a haul truck. He was using a pry bar to turn the assembly when he felt sudden pain in his left shoulder. Employee was taken off work 9/30/2014

Employee was lifting some dunnage when he felt a pop in his right elbow. Employee had surgery 7/24/2014 and became a lost time.

Employee was using the boarding ladder to climb down the shovel and slipped and fell to the ground.

Employee was operating a backhoe when he felt some discomfort in his groin area, around 11:30. The employee had lunch and went back to work. At 2:00 the employee requested medical care. Cause of injury is questionable.

Employee was dismounting road grader at the 2890 tie down and missed the last step causing him to fall backwards onto the ground.

Employee was troubleshooting the PP01 cables and erroneously touched the high voltage pothead and contacted one phase of 4160 volts with his right hand.

The employee was steam cleaning the drill. He was up on the mast of the drill. As he went to squeeze the handle to activate the cleaner, the nozzle jumped and he lost control of the wand. The high pressure water cut the tip of his boot and sprayed hot water onto his left foot causing 2nd degree burns to his foot.

Employee climbed out from under a rubber tire dozer and stood up. The sun was in his eyes and he lost his balance. He fell and his right hand struck a lock ring from a big tire that was laying on the ground.

Employee was standing on the right rear platform of the rubber tire dozer. When he turned around he lost his balance and fell approximately 4 feet landing on his shoulder and head. Employee has a broken collar bone.

EE was cutting a piece of plate that was near the remaining bulk of the plate he was stepping backwards and his foot stepped into a gap on the plate, causing him to fall to his side with his foot lodged in the hole he had stepped in.

While using a Cup Cutter tool to prepare an injury in an Earthmover Tire for repair the cup cutter bound up in the rubber and slipped to the side striking the associate on the right knee resulting in a laceration that required sutures. Associate returned to regular duty the next shift 9-1-2010.

Employee was climbing down a ladder access at a drill. At the landing there was a rock that caused his left leg to twist when he stepped on it. He twisted and he felt pain in his lower hip and back.

Employee was adjusting the reflector in the drop box of the stripping machine. Communication got confused between the workers and the flapper arms were inadvertently activated, pinning EE's arm in a pinch point.

Employee was removing dipper handle pin from shovel dipper when the pin got stuck. The employee moved the pin up and down to free the pin. The pin released unexpectedly and came out completely. The pin pinched the employee's tip of the right ring finger.

Employee was working to free a stuck cathode in the stripper.When the blank was freed from the copper,the machine advanced the blank starter sheet and also advanced the next cathode in line.As the machine advanced the blank, the loose copper sheet struck the employee in the back.

Employee was troubleshooting an electrical problem the switch house for the 579 shovel. When testing the high voltage side there was an arc flash and the employee was burned on his left hand.

Using a ratacher tie-down strap to secure equipment to the bed of a broom truck, injured sustained a a cut to lower lip and broken tooth when the strap end came loose and struck injuried on the mouth.

While descending the steps from the control panel to the copper conveyor, the fiber-glass step gave away causing EE to fall through the opening up to his waist. The fall resulted in a scraped left shin, a sore right knee and a small cut to the left index finger.

At the ready line injured jumped over a puddle, landing on a rock and turning his right ankle and bruising his left knee.

While helping to remove broken concrete from the acid unloading station, injured sustained a corneal abrasion when a wind blown foreign body entered injured's left eye.

While removing a one-ton GMC left rear brake caliper, the caliper slipped from the EE's grasp pinching and fracturing the right ring finger. Injury occurred when finger was pinched between floor and caliper.

While moving a 9 7/8"" drill bit on the drill deck the employee over-extended his body which resulted in back strain.

While taking amp reading, overloads failed and flashed across phase to phase causing an extreme arc resulting in flash to both eyes. Prescription drug was administered.

THE SHOVEL OPERATOR, WITH THE DIPPER POSITIONED IN THE AIR, ATTEMPTED TO SET THE HOIST BRAKE WITH HIS LEFT HAND. IN THE ATTEMPT TO SET THE HOIST BRAKE, THE BRAKE WAS UN-INTENTIONALLY RELEASED.
