The employee was installing blades on a shredder in the resource recovery area. They slid a blade onto the shaft and pinched their finger between the blade and the spacer.

A member of the office cleaning company was cleaning a restroom in the administration building. EE mixed two chemicals in the restroom which caused EE to inhale fumes. The vendor employee was taken to the emergency room and released later that day.

The employee was lowering a coil of hose from on top of a silo to the ground using rope. the weight of the hose caused the employee to lose grip on the rope momentarily. When the employee regained grip on the rope, they hit their right thumb on a guardrail causing the injury.

The employee was laying on grating reaching to install a bolt on the bearing housing, while a Johnson bar was leaning on a nearby handrail. Another employee leaned against the handrail which caused the Johnson bar to tip over and strike the employee on the head.

While opening the quarry gate by pulling the gate open and walking along side of the gate; a rock became lodged under the gate causing a sudden stop of the gate's motion. The employee's arm/shoulder was pulled back suddenly causing a pain in the shoulder.

The employee was manually stacking 75LBS cement bags. After doing so, the employee recognized pain in lower back and leg. On May 15,2023 the employee completed the accident notification to the company. After a medical evaluation, the employee was returned to work with a restriction and having been prescribed medication.

The employee was raising a face shield which was attached to EE's hard hat. Dust had settled on the brim of the hard hat. When the employee raised the face shield, dust fell between the hard hat and the face shield landing behind EE's safety glasses into left eye. All dust was flushed out, the employee was issued prescription medication for inflammation.

The employee was loading blades onto a forklift. Once on the tine, the blade shifted forward smashing the employee's finger between that blade and a blade that had been previously loaded onto the forklift tine. On 11/22/22 the employee sought medical care and was diagnosed with a nondisplaced fracture of the left little finger

During the course of restarting finish mill 2 the employee was checking air slides for blockages. The employee opened an inspection door of an air slide and a blockage from upstream became dislodged. Dust from the dislodgement entered the employee's eyes although employee was wearing safety glasses.

The employee was assigned to remove lids on a dust collector in finish mill 3. After removing the bolts that secure the lid in place the employee (along with a second employee) began to lift the lid. While lifting the lid, it became stuck in place. The employee attempted to quickly pull the lid out of the stuck position and strained a muscle in their chest.

While performing the task of removing aeration grates from the cooler floor they said they lifted a grate out of place and as they stood back up felt a pope in their lower back.

The employee was working on a blower on the preheater tower which was locked out prior to working on the equipment. The employee was using an air lance to dislodge a stuck blower. Employee attempted to push the blades with hand in order to loosen them. The blades began moving unexpectedly and at a high rate of speed pinching three fingers between the blades and the housing.

The employee had hand pinched between a hydraulic cylinder and a base plate at the #4 roller in the raw mill. The employee was holding the cylinder (approximately 50 LBS) in place with one hand and cleaning the lower threaded section with other hand. The cylinder slipped and struck the employee in the hand that was cleaning the threaded section causing the laceration.

An employee was on knees reaching into a box attached to the FK pump at finish mill 2 to remove a flapper arm. When employee lifted the flapper arm employee felt a sudden pain in lower back. The employee was issued a work restriction and prescription medication.

The employee was entering the raw mill to supervise preventative maintenance. As EE opened the door, EE moved EE's hand from the handle and placed it on a bracket, which created a pinch point. EE's fingers were caught in the pinch point causing a laceration, contusion, and fracture of EE's fingers.

All American Silo employees were walking pass feeder equipment when door on feeder was blown off. Employee took the blunt hit and others received little to no dust. Employee was taken to hospital for flushing of eyes after flushing them on site.

On 04/09/21 the employee was working changing bags in the bypass dust collector. On 04/12/21 they informed a supervisor that lower back felt strained. On 04/13/21 they informed the safety manager that they wanted a medical evaluation. The employee was issued a work restriction and prescription medication.

On 01/28/21 the employee was working to free a stuck cyclone in the vertical raw mill by using a shoulder to push against the equipment. In doing so the employee strained neck and shoulder and was issued a work restriction.

The employee was removing a door on the BE-23 bucket elevator (head end); when employee felt a pain in lower back. The employee did not want a medical evaluation at the time of the injury. On 05/04/2020 the employee did not work scheduled shift due to back pain, and was evaluated by the occupational medicine clinic resulting in a restriction of duty.

The employee was grinding the finish mill # 2 clutch while the clutch was being supported by one forklift tine, a steel component weighing approximately 60lbs was resting on the other forklift tine. When the employee finished grinding the exposed lip, the employee attempted to rotate the clutch and knocked the steel component off of the tine causing it to fall onto foot.

The employee was on top of the 3rd bank silos and felt something in eye. EE rubbed eye in an attempt to dislodge what was in the eye. In doing so EE caused a corneal abrasion.

The employee was in the process of changing a tire. He picked the replacement tire up and felt a burning sensation in his arm. He set the tire down and when he looked at his arm there was a knot in the bicep area. He has been referred for further evaluation.

The third finger on the left hand of the employee was pinched between material being moved and a hand rail causing a contusion. On 11/16/2014 the employee returned to the Occupational Medicine Clinic due to swelling and discomfort. The finger had become infected and the employee has been referred to a specialist. The employee has not missed days or been transferred.

On 06/20/2014 the employee was evaluated. Only 07/03/2014 the employee was evaluated. On 08/11/2014 the employee brought documentation that he was to only work in a seated position

Employees were pulling bags off cages in a baghouse with hooks when one employee's hook slipped and hit the other employee in the elbow. The Supervisor treated it with basic first aid. Several days later, the employee experienced swelling in his arm. We sent the employee to a doctor who prescriped anti-inflammatory drugs.

Mobile equipment mechanic was inserting a radiator hose on a CAT D-11 dozer through the bottom of the radiator mounting frame. While pushing the hose through the opening, his hand slipped and he lacerated the top of his right ring finger.

Employee was removing damaged idlers from conveyor line. While placing one of them in a basket of a man lift, his right ring finger became caught between the idler frame and roller. He suffered a crushing injury to the upper half inch of his finger. Diagnosis was a broken finger.

While moving pieces of debris, a piece slipped in the employee's hands, and while adjusting to secure the piece, pinched his right ring finger on a scaffold pipe. The pinch cut the employees finger at the bottom knuckle. The employee was taken to the hospital where he received 6 stitches and was released back to work. The employee was wearing leather gloves at the time.
