Production Supervisor was cleaning out material from a choked screw conveyor with hand. The Supervisor's hand ran across the sharp flight of the screw and caused a laceration which required 4 stitches to close the wound. Screw was locked out at time of incident.

A production process attendant injured EE's left middle finger while pushing in a pin on the chute above the clinker cooler drag conveyor. When pushing in the pin to break up the chunk, the chunk cleared causing EE's forward momentum to strike EE's hand between the pin and the chute. This resulted in a laceration and fracture of the left middle finger.

An employee pinched a finger between a bearing housing and the pedestal base. Two employees were installing the tail drum for a small-scale belt. The injured employee was positioning a base bolt from underneath the pedestal and had their finger slightly under the load when it was let off causing a laceration of the skin. Received 5 stitches and restricted duty.

Employee was clearing an air slide on top of the cement silo when the dust collector flushed and cement dust hit the employee in the face. Cement dust entered the employees eyes, employee received a prescription for cornea abrasion.

Employee was climbing fixed ladder to go to the 914 drag conveyor. The employee's foot slipped off of the ladder rung. The employee caught self with right hand, the employee felt pain in right shoulder. Employee was seen by the doctor, diagnosed with a shoulder strain and prescribed physical therapy.

Maintenance Mechanic was sling a welding lead over the crusher when EE felt a pain and experienced numbness in the right shoulder.

Shipper/packer/loader was attempting to lift a barge winch cable off of the barge when ee felt a pain in lower back. Employee declined medical attention at that time. On May 3, employee requested to be seen by doctor. Employee was diagnosed with lower back strain and prescribed physical therapy.

Employee was attempting to clear debris around a yoke in order to align a pin slot in the secondary crusher. At the same time another employee dislodged a rock on the other side of the yoke and the crusher apron dropped. The employees left thumb was crushed between the yoke and the crusher apron, causing a chip of the bone and tissue damage to the employee's left thumb.

Employee was installing a guard back on coupling. The employee lost control of the guard and it slid over and pinched left ring finger between the guard and side of motor. This caused a laceration requiring 2 stitches and a fracture of the tip of the finger.

On 6/17/17 Employee was swapping compressed gas cylinders when employee lost balance and was struck by cylinder on the lower leg. Employee wound was treated contusion and abrasion with first aid. On 7/6/17, employee was still experiencing pain and wanted it evaluated. The employee's wound had gotten infected, employee given Rx for antibiotics

EMPLOYEE WAS USING TORCH TO DEMO STEEL LINERS OUT OF CHUTE. ONE OF THE LINERS WAS UNKNOWINGLY LOOSE AND WHILE PRYING ON THE LINER THE EMPLOYEE HAD JUST FINISHED CUTTING, THE LOOSE LINER ON THE ADJACENT WALL TIPPED OVER SMASHING THE EMPLOYEE'S FINGER BETWEEN THE TWO.

Employee was lifting a mill liner that weighs approx 65 lbs when the liner slipped from employee's grip and pinched the left ring finger causing a laceration. When in the process a wet condition caused the gloves to be wet. Employee was taken to a local hospital and received stitches to close the lacerations. X-rays showed no fractures.

Employee stated ee wanted to go to a doctor on 8/16 after getting struck on the side of hard hat with a cable while wearing it. EE claimed it occurred on 8/9 but waited 7 days to go see a dr to get checked out after suffering headaches and insomnia.

Mechanical Repairman was cutting up scrap metal when they attempted to squat and lift an 8' piece and felt a burning sensation in abdomen/groin area. EE ended up re-aggravating a prior hernia injury from 2014.

EE WAS CLEANING 55 GALLON OIL DRUMS IN PREPARATION FOR REMOVAL FROM SITE. EE WAS USING MINERAL SPIRITS & SOME OF THE SOLUTION GOT ON HER HANDS, CAUSING BURNS TO BOTH WRISTS.

EE WAS PERFORMING CLEAN UP WORK IN CLINKER TUNNEL AND SOME OF THE CEMENT MATERIAL GOT INTO HER BOOT. AFTER A PERIOD OF TIME SHE EXPERIENCED A CHECICAL BURN TO THE ANKLES.

THE EMPLOYEE WAS USING A RAG TO WIPE OIL FROM THE BODY OF A COMPRESSOR. THE SUCTION FROM THE FAN CAUSED THE RAG TO BE DRAWN INTO A SMALL AREA BETWEEN THE GUARD AND THE FAN MOUNTING. THE FAN BL ADE THEN CONTACTED HIS FINGER CAUSING THE INJURYWHICH REQUIRED SUTURES TO CLOSE.
