Injured Person was replacing the classifier bearing assembly. The assembly became lodged due to alignment. The injured employee had hands underneath the assembly, attempting to correct the alignment. Subsequently, the assembly dislodged and fell onto the injured employee's hands. This incident resulted in a broken ring finger and lacerations to the ring and index fingers.

Showing all 46Operator was doing pre-shift on a D8N Dozer while closing engine inspection door they slipped and fell, they broke their wrist as a result of the fall.
Affected employee was cleaning out #5 Mill feeder and injured EE's elbow (elbow contusion) in the process.
The Operator was loading a truck out of 8 silo. EE attempted to hold the hose and ladder simultaneously. Employee shoe slipped and gate sprung back into its original position. This resulted in a strained shoulder. EE was given instruction to take over the counter medication, and not to lift anything over than 10lbs.
Employee was called out to install a hose for 10 silo. The clamp failed and EE had to come back out and install it again. During the second installation the hose was found in the car, full of clay. Another Employee helped EE pull the hose out of the car and this was when the employee felt a pain in the lower stomach.
EE was installing a new 4 wide drive belt on the drive belt on the Dryer Air Supply Fan for the Furnace. Belt was on driven pulley and they were rotating non driven pulley to install belt. When the belt jumped into place EE finger was between the belt and pulley.
Employee was putting up parts while placing a part on a lower shelf another part from the upper shelf fell striking employee in the head.the impact caused a cut to the head that required stitches
Mechanic got hand caught in a conveyor chain resulting in a right hand laceration.
On April 21, A Bulk Loader bumped elbow on safety railing while putting on harness, it resulted in swelling to the right elbow. EE was seen by a doctor on May 4th, EE was given a restriction of lifting no more than 20 lbs. Therefore it became reportable on May 4th.
The employee was restacking pallets that had fallen onto the ground by hand. When the employee picked up the third pallet, EE felt a pop in the lower back with pain traveling down the right leg. EE notified the supervisor about the issue, received medical treatment and returned to work on restricted duty after being diagnosed with a lower back strain.
While opening an inspection door to a hammer mill, Employee got finger smashed between plate door and dump hopper sitting close by.
After performing various electrical and maintenance tasks on July 16th, 2018, a mechanic working on site indicated that lower back was stiff. The employee could not identify a single instance or incident that caused the stiffness/soreness. The employee reported the incident at the end of the regularly scheduled shift.
Employee was moving barricades to line them up for installation when one of the beams slipped out of the holder and cut their palm.

Employee was putting a fork on the forklift when they lost control of the fork and it fell striking their right foot. The blow caused a small cut on the top of the right foot just above the steel toe on their boot
DISCHARGE SHOOT WAS CLOGGED AND EMPLOYEE PICKED UP THE HOSE ATTACHED TO SHOOT. HOSE BROKE AWAY FALLING ON EMPLOYEE. HIT EMPLOYEE AND CAUSED FRACTOR TO FOOT.

Employee was assisting in the replacement of the Blunger pump. Employee was guiding one end of the pump and lifting on that end while another employee was pushing from the other end. Employee's hand was guiding end of pipe attached to the pump. The pump moved suddenly catching the employee's finger between two pipe ends
The employee stumbled at the Rough Room Door. Extended his right arm which was hit by the door as it was closing. Causing pressure into right shoulder possibly causing injury.
Employee was loading a bulk truck and the clay had backfilled into the discharge pipe. To free the discharge the employee had to lean over and apply extra force to maneuver the discharge pipe free. Upon doing this the employee felt as if something had slipped in his back or he pulled something.
He was pulling something out of a track-hoe and hurt his right elbow.
Employee hit finger while driving ground rod for service pole and received four stitches.
Hammer mill rotor was locked down with clay. Used vacuum truck to remove clay and then tried to free up rotor by rotating it. Employee pushed on rotor and when it broke free it caught his left hand middle finger between wall and a hammer causing a laceration.
Employee was cleaning Carbon Testing Machine when her eyes began to burn. She washed her eyes with eye wash solution. It seemed to help. Employee worked the remainder of her shift and on her way home her eyes continued burning so she decided to go to the doctor. Employee was back at work on Tuesday which was her next regular scheduled day to work.

While in the process of hanging a mirror in the mop/wash room of the mine shop the glass broke sticking a piece of it in his right arm.
Employee struck his head on a light fixture in a storage area of the office.
The employee went to push open a door when the glass in the door broke & caused cuts on his middle & ring finger & his arm.
The employee was putting some washers under a motor. He had the motor raised approximately 1 inch in order to get the washers under the motor. Some clay began to leak from above & the employee turned his head & when he did the motor fell on the tip of his index finger, mashing his finger. The finger required stitches.
Employee was adjusting the tip with his left hand he accidently compressed the trigger with his right hand, causing the high pressure water to cut his finger and the water was injected into his index finger on his left hand.
The employees was removing one of the two remaining truck tires off the fork of the forklift. As he began to roll the first tire away the second tire fell striking him about the knee knocking him to the ground.
2 employees were cutting down a pipe. One was controlling the hoist. When the pipe was cut, it swung and caught the finger of the employee controling the hoist. The finger was split open at tip. The employee was taken to the doctor, received 6 stitches, and returned to work without restrictions.
Employee was using a lathe and had dropped a chuck into the catch basin of the metal shavings that are left in the bottom pit on the machine. As he attempted to get the part that had fallen, his finger slid across the long slinder shavings and sliced the middle finger on his right hand
EE BENT DOWN, GOT A HOLD OF THE BOTTOM OF THE DOOR WHILE USING PROPER LIFTING TECHNIQUE, STRAIGHTENED AND PROCEEDED TO RAISE THE DOOR THE FULL LENGTH OF HIS REACH. AS HE DID SO PAIN ACCURED IN HIS LOWER BACK, AGGRAVATING AN INCIDENT HE HAD ON 9/10/03. HE HAD BEEN INSTRUCTED PREVIOUSLY ON PROPER LIFTING TECHNIQUES AND HAD RETRAINING IN JAN. 2004. HE SAID HE HAD USED PROPER LIFTING.
EE WAS MOVING A BOX OF METAL PARTS FROM THE CLAY SHREDDER EST. AT 50 LBS. AT THE BOTTOM OF THE SET OF STEPS HE TURNED TO SET THE BOX DOWN AND STRAINED THE LOWER RIGHT SIDE OF HIS BACK.
EMPLOYEE WAS WORKING ON REJECT LINE ON TOP OF REJECT SILO, STRUCK RIGHT HAND ON REJECT LINE COUPLING.

EMPLOYEE WAS STACKING 50 LB BAGS OF CLAY WHEN A COUPLE OF BAGS GOT TURNED SIDEWAYS ON THE BELT. EMPLOYEE PUT HIS RIGHT ARM OUT AND WAS HOLDING THE BAGS BACK. HE REACHED OVER WITH HIS LEFT HAND TO TURN OFF THE BELT, WHEN HE FELT A PAIN IN HISRIGHT ARM.
EMPLOYEE WAS CLEANING THROAT OF SHREDDER PRIOR TO ADJ TEETH. HE WAS USING A SCRAPER TO DIG OUT THE CLAY. HIS HAND SLIPPED AND STRUCK THE WIPER.THE WIPER HAD BEEN WORN SHARP AND SLICED OPEN HIS LEFT THUMB.
EE WAS CHANGING HAMMERS IN 300 MILL SYSTEM. WHILE RE-INSTALLING MILL DOOR, ONE EYE BOLT WITH CHAIN THRU IT CAME LOOSE, CAUSING DOOR TO SWING & DROP TO SIDE, WHICH SMASHED MIDDLE LEFT FINGER OF LEFT HAND BETWEEN DOOR & MILL.
EE ATTEMPTED TO ROLL A COUNTERWEIGHT OVER TO LIFT WITH A FORKLIFT TO RE-INSTALL ON FRONT END LOAD. WHILE ATTEMPTING TO ROLL HE FELT PULL IN RIGHT HAM STRING.
EMPLOYEE WAS WORKING ON LOADER 1527. HE WAS FIXING THE PARKING BRAKE. WHEN HE GOT SOMETHING IN HIS LEFT EYE. EE WAS TAKEN TO DR TO GET IT REMOVED.
EE WAS CHANGING OUT A BEARING ON THE MILL. WHILECLEANING THE MILL SHAFT WITH A SAND SCREEN, HIS HAND SLIPPED AND HE CUT HIS LEFT RING FINGER. HEHAD TO HAVE 3 STITCHES.
EE WAS CLEANING UP SPILT CLAY FROM UNDER 66 SHREDDER WITH A FLOOR SCRAPE. PULLING CLAY OUT FORM SHREDDER.
