Injured Person was replacing the classifier bearing assembly. The assembly became lodged due to alignment. The injured employee had hands underneath the assembly, attempting to correct the alignment. Subsequently, the assembly dislodged and fell onto the injured employee's hands. This incident resulted in a broken ring finger and lacerations to the ring and index fingers.

Affected employee was cleaning out #5 Mill feeder and injured EE's elbow (elbow contusion) in the process.
The Operator was loading a truck out of 8 silo. EE attempted to hold the hose and ladder simultaneously. Employee shoe slipped and gate sprung back into its original position. This resulted in a strained shoulder. EE was given instruction to take over the counter medication, and not to lift anything over than 10lbs.

Operator inspected the mill tank for plugage. During inspection EE found a plugged area around the discharge of the tank. EE then retrieved a cleaning tool from the milling area and began prying product away from the discharge area. While clearing the product away from the discharge, the tool slipped from EE's left hand and struck EE's right index finger
Employee was cleaning out the #5 Mill feed bin with a push bar to clean the material that had plugged up inside the tank. When the employee pulled to remove the push bar employee's left index finger became caught between the push bar and a guard rail.

Mechanic got hand caught in a conveyor chain resulting in a right hand laceration.
On April 21, A Bulk Loader bumped elbow on safety railing while putting on harness, it resulted in swelling to the right elbow. EE was seen by a doctor on May 4th, EE was given a restriction of lifting no more than 20 lbs. Therefore it became reportable on May 4th.

Employee was stung by an insect and had an allergic reaction. Saw a doctor and received medication to reduce swelling. No impairment from EE's daily life.
The accident occurred on 9/25/19, but it didn't become reportable until EE went to doctor on 10/03/19. EE first noticed pain while pulling wrenches on the Haver bag placer, one in each hand, to either loosen or tighten. Then moved to hammer to help move a bolt. Moved back to wrenches and pain got worse. Pain present along forearm to palm near thumb on right hand.
Walking in reclamation and tripped on uneven ground resulting in a fall. Rt shoulder was hurt from the fall.
Slide gate was removed from an incline auger during operation. The slide gate was not bolted closed at the time of the incident. Lockout/tagout was not followed prior to removal. As they pulled the slide gate open, the fingers of their right hand contacted the rotating auger. This caused a hand laceration and a finger dislocation on the right hand.

The employee was restacking pallets that had fallen onto the ground by hand. When the employee picked up the third pallet, EE felt a pop in the lower back with pain traveling down the right leg. EE notified the supervisor about the issue, received medical treatment and returned to work on restricted duty after being diagnosed with a lower back strain.
After performing various electrical and maintenance tasks on July 16th, 2018, a mechanic working on site indicated that lower back was stiff. The employee could not identify a single instance or incident that caused the stiffness/soreness. The employee reported the incident at the end of the regularly scheduled shift.

Employee was moving barricades to line them up for installation when one of the beams slipped out of the holder and cut their palm.
Moving some bundles around maintenance shop, miner stumbled over own feet. In catching balance, left thigh was strained.
Miner was performing a gear change out on the Raymond Mill inclined grit auger. Miner was using a small sledge hammer to assist removal of gears when their hand struck an adjacent guard constructed from expanded metal. The miner received a laceration on the back of their hand that required sutures.

Employee was assisting in the replacement of the Blunger pump. Employee was guiding one end of the pump and lifting on that end while another employee was pushing from the other end. Employee's hand was guiding end of pipe attached to the pump. The pump moved suddenly catching the employee's finger between two pipe ends
Employee was cleaning out hard built up clay/mud out of Imp Mill Slicer. While employee was trying to remove clay/mud from the back walls using a heavy bar in a prying motion. The bar slipped causing a quick and hard motion resulting in a sharp pain and burning sensation in the left forearm.

Employee was loading a bulk truck and the clay had backfilled into the discharge pipe. To free the discharge the employee had to lean over and apply extra force to maneuver the discharge pipe free. Upon doing this the employee felt as if something had slipped in his back or he pulled something.
Employee states that while moving a pallet of stacked poly bags to the stretch wrapper the bags fell off the pallet. Employee stopped the forklift and proceeded to reload the pallet with the poly bags. Employee states he had already lifted 10 bags and was on the 11th bag when he lost his footing. While regaining his footing employee felt a pain in lower back.

The employee was putting some washers under a motor. He had the motor raised approximately 1 inch in order to get the washers under the motor.
