MS EMPLOYEES WERE MOVING CRIBBING AT THE CF HARDEE BUCKET YARD. EE'S WERE WORKING TOGETHER, STACKING CRIBBING ON A FORKLIFT. WHILE STACKING THE CRIBBING, INJURED EE GOT THE PINKY AND RING FINGER ON HIS LEFT HAND CAUGHT IN A PINCH POINT.

A mechanic was using a cutting torch to split a 20 pipe for a wear band. He cut thru the pipe & stored energy in the pipe caused it to spring open striking his left ring finger was struck by pipe. He was evaluated by facility Nurse & sent to local clinic for x-ray, which confirmed a fracture. His finger was set & splinted. He was released to return to work with restrictions.

Employee was stepping over a matrix pipe line carrying a small aluminum ladder. Employee stated that he felt a pain in his lower back. A subsequent examination and evaluation by a physcian resulted in the diagnosis of a muscle strain. Employee was allowed to return to work on modified duty with work limitations pending full recovery.

Employee was attempting to empty a trash drum when he heard a popping sound in his right elbow. The initial doctor evaluation reccomended immobalization of arm until further testing. A subsequent examination by orthopedic specialist revealed a damaged right bicep muscle.

EE was picking up power cable to move and slide over pipe when cable slipped pinning right hand between the pipe and hot stick. Contusion to top of r/hand. Treated & released to full duty w/MMI @ 0% PPI & dscharged from care. No meds prescribed. One time visit, first aid status. Conduct a JSA and retrain for the task as necessary to prevent reoccurrence.

While removing a 20" blind from between two flanges on a pipeline, the bucket on the PC 400 excavator struck employee on his left middle finger and smashed it between the bucket and the pipe.

EE WAS IN THE PROCESS OF CONNECTING TWO 100' JOINTS OF 24" PIPE. ONE JOINT WAS LYING ON THE GROUND WITH ONE END CRIBBED WITH A WOODEN BLOCK. THE OTHER JOINT WAS BEING MOVED INTO PLACE WITH A L OADER WHEN IT STRUCK THE FLANGE OF THE ONE ON THE GROUND. THIS CAUSED IT TO SLIDE ONTO THE EE'S FOOT.
EMPLOYEE WAS PLACING BOLTS INTO A PIPE FLANGE. WHEN HE HIT ONE OF HTE BOLTS WITH HIS HAND HIS RIGHT HAND HIT THE PIPE FLANGE AND KNOCKED OFF THE FINGRNAIL ON HIS PINKY FINGER.
