EARLY MORNING AND THE SUN WASN'T COMPLETELY UP HOWEVER IT WAS LIGHT ENOUGH FOR GOOD VISIBILITY WITH OR WITHOUT LIGHTS BEING ON. BACK UP ALARM WAS FOUND TO BE IN GOOD WORKING CONDITION. The victim was struck by a road grader when it backed up. The accident occurred in the staging area where the victim and other coworkers were checking their equipment for the shift.

Climbing down from scraper and as reached the ground, right ankle rolled & right knee twisted and popped. Possible partial ligament tear to right knee.
Employee was operating a 657 scraper and hit a small rough spot on the haul road, resulting in back and shoulder pain. EE stopped the equipment, and when the scraper was inspected it was found that the threaded yoke and linkage rod threads stripped out at the cushion hitch valve. The employee is currently on restrictions.
Injured employees right thumb lodged between a 1/2 inch winch cable and a AWJ drill rod causing a compression injury.
Trespassers suspected of poaching were spotted by FWC on mine property. When FWC officer activated the emergency lights, the suspects took off on an ATV at a high rate of speed, traveling on the crest road of a dam. The ATV left the roadway traveling down the dam coming to rest on it's roof on the dam's toe road. Both suspects were taken to the hospital.

While ascending spillway stairs employee slipped and fell causing EE to twist EE's ankle. Sprain was treated conservatively. Employee's personal Physician ordered X-ray due to discomfort. Company Physician had X-ray repeated which revealed a fracture. Employee was placed on restricted duty on 2/11/20.

6 man crew was planting vegetation in 18"" of water. Worker was bit on left side by an alligator. Pre-job inspection showed no alligator presence. Spotter in place did not see gators until incident occurred. Employee taken to emergency room treated for injury on left hand, significant damage to thumb. Also cuts on torso under left arm and abrasions on back and stomach.

While performing inspections/PM EE felt something in left eye. EE did not report to supervisor and went to a walk in clinic the next day. A foreign object was found and removed from left eye. EE reported when returning for normal shift 12/11, Restrictions - No driving CMV - other accommodations have been made.
A front-end loader traveling on a mine road broke through into a water conveyance ditch and overturned. A contractor noticed the overturned equipment and responded to the site. Contractor employees immediately notified Mosaic supervision who activated the site ERT. ERT contacted 911.
Employee was dismounting off of the payloader when they fell, landing on the left wrist. Employee misjudged the distance from the bottom step to the ground and fell to the ground. The ground was uneven where the employee was dismounting off of the payloader.
Walking to the water jack, the employee's right leg slipped out from under EE and caught self by planting left leg. Emp was seen by company nurse on 2/13 given ibuprofen and ice packs. On 2/16 company nurse re-evaluated emp placed employee on Restricted duty due to the swelling and complaint of pain.

Employee exited the truck and closed the door on right hand ring finger. Resulting an approximate 2 inch avulsed area on the finger.
The sealing water hoses for the pit pump had been disconnected and were being stowed on the pump sled by 2 members of the float crew. As the flanged end of the hose was being rolled up on the sled the employees gloved left hand was caught between the flange and the pump gearbox, causing an avulsed area on the left index finger.

Right hand caught between end of pipe wrench and helpers stand. Resulted in broken index finger & laceration of index, middle and ring fingers - 9 stitches total.
Employee A and B were working on water line tank. Employee B stopped swinging the hammer to let Employee A use the impact to loosen the bolt. Employee A reached for the spinning nut and Employee B swung the hammer at the same time striking Employee A.
In the process of bolting pipe together the Employee wiped the flange off with his hand a metal splinter stuck into his right ring finger. A local anesthetic was administered and the fragment was removed. The employee was given a written prescription for antibiotics and sent for an x-ray at a local hospital to ensure all metal from the splinter had been removed.
Employee was sliding a fork on the forklift and pinched his right thumb between the cage and the fork.

Employees were tasked with changing out the paddles on the C Log washer. Employee was attempting to step over a spray water brace on top of the log when he stumbled. He put his left arm against the guard on the side of the log washer to catch his fall. Employee then reported that he felt like his shoulder was dislocated and he was in pain.
Employee was changing out a ballast in wall mount light turned to set it down and felt a pinch in his stomach. He though it was the harness pinching him, and keep right on working.
While the 852 drag line was walking, an operator was walking in front of the machine to ensure power cables were clear of the tub. Due to recent rainfall, the area was saturated and muddy. As the operator was walking thru the mud, he felt a pain in his lower back as he pulled his foot out of deep mud.

employee cleaning tools & materials off bed of maint truck. Emp bent over to pick up choker off ground, had hand resting on edge of storage box and lid fell on his left hand, causing a fracture of the 3rd metacarpal of left hand.
Employee was installing a section of 20 inch steel pipe to a 90 degree fitting. Two employees utilized hand tools to attempt to free a frozen nut. Employees hand was rotated toward the pipe causing his right ring finger to be caught in between the open end wrench and the pipe. Employee received a laceration requiring sutures to correct.
While Mechanic's Helper was swinging sledge hammer, he missed intended target; causing a metal fragment to chip off.

Employee was exiting the float truck bed area, utilizing manufactured steps for egress. Instead of stepping straight down, the employee stepped to the left corner of the bumper step. EE WAS USING A 4' PRY BAR TO SLIDE A PIECE OF DECK PLATE THAT HE WAS WORKING ON IN THE CF MAINTENANCE SHOP. THE PRY BAR SLIPPED AND THEN SMASHED HIS LEFT INDEX FINGER (THROUGH HIS GLOVE) BETWEEN THE PRY BAR AND A PIECE OF ANGLE.
Miner was hammering a keeper pin into place on a dragline bucket tooth when he missed the pin, striking the tooth itself with the hammer. A sliver of metal chipped off of the tooth and struck the miner between the first and second finger of his left hand. It required three stitches to close the laceration and the employee was released back to work with no restrictions.
While trying to loosen bolts on plate; IW felt sharp pain in right shoulder
While operating pan on dusty haul road; dust entered IW eye.
EE was attempting to insert a spike puller to remove a spike, when the action of hitting said spike puller dislodged a piece of an old weld that had adhered itself to the spike. When this piece became dislodged, it flew up & hit him in the neck causing a laceration that required two (2) stitches to close. EE was transported to ER for treatment. Released, no duty restrictions.
Mechanic was using a lathe to fab part for a track torch. Metal shavings spun around & caught his left index finger, causing a laceration. He was treated by the facility Nurse & sent to local clinic where he received 11 sutures and was returned to work with restrictions.
A mechanic was using a cutting torch to split a 20 pipe for a wear band. He cut thru the pipe & stored energy in the pipe caused it to spring open striking his left ring finger was struck by pipe. He was evaluated by facility Nurse & sent to local clinic for x-ray, which confirmed a fracture. His finger was set & splinted. He was released to return to work with restrictions.
While attempting to turn disconnect switch off; plastic casing came off release handle causing hand to slip.
A Maintenance employee was working in the Washer structure. Employee was standing on the ground directing a crane into place when he was struck in the mouth by a falling piece of rock. The employee was taken to Occupational Health Partners where he received 13 sutures. The employee was released to return to work with no restrictions.
EE was dismounting machine when his foot slipped causing his leg to hit track pad growser.
While attempting to check final drive on D8 Dozer #1702; IW was using a ratchet to pull up causing pain to in middle of back.
While using chipper hammer to clean rails off; hammer hit metal causing a piece of metal to come off corner of hammer and lodging into middle finger of right hand.
Employee was stepping over a matrix pipe line carrying a small aluminum ladder. Employee stated that he felt a pain in his lower back. A subsequent examination and evaluation by a physcian resulted in the diagnosis of a muscle strain. Employee was allowed to return to work on modified duty with work limitations pending full recovery.
A contractor employee was in the process of removing electrical zip ties from bundled electrical cords using a pocket knife. The knife slipped and the employee was cut on his left thumb through his cotton work glove. The laceration required three sutures to close. The miner was released back to work with no restrictions.

Miner was tightening bolts on a piece of equipment when a bolt snapped. The sudden decrease in resistance caused the Miner's hand to fly back, mashing his right ring finger between the wrench handle and a nearby pipe. This resulted in the Miner receiving 5 sutures on the affected finger.
Employee was engaged in cutting, grinding, and welding activities when he noted discomfort in his right eye. A subsequent examination later the same day by an Optometric Physician revealed the presence of a paint chip. It was removed and the employee was allowed to return to work with no restrictions.
Operator hit bump in haul road causing him to lose control of truck. Dump truck came to rest in ditch after hitting guard rail.
Employee did not have proper foot placing and twisted left ankle. In an attempt to break fall, employee injured left hand. Employee suffered sprain/strain to ankle and two fingers. No fractures.
Employee was driving a vehicle alongside of a materials pipe line, when a stud on the right front tire struck a pipe flange, causing the wheel to suddenly jerk. This in turn caused the steering wheel of the vehicle to unexpectedly spin, yanking the emploee's left thumb downward and causing a ""Bennett Fractue"".
Employee was attempting to empty a trash drum when he heard a popping sound in his right elbow. The initial doctor evaluation reccomended immobalization of arm until further testing. A subsequent examination by orthopedic specialist revealed a damaged right bicep muscle.
Laborer was moving tools & reports tweaking back muscle while pulling on rope. Laborer was immediately removed from jobsite to office to report injury and taken for medical evaluation. Local MSHA office and mine company personnel notified immediately.
Employee was operating an open cab tractor when he was struck on the right temple by a piece of PVC pipe that was lying in the back of a parked truck. Employee was briefly knocked unconscious and was taken to the hospital for treatment and evaluation. Employee was released and allowed to return to work the evening of the incident.
Employee was driving wooden wedges under the tub of a 1370 dragline to tighten the cribbing under the tub. On his backswing, he hit some adjacent cribbing, altering the path of the sledge hammer and hit his left foot.

He had also placed a 30"" spud bar into the grating with the intention of applying leverage to the low spot. The weld and clip holding the grating broke loose causing the spud bar to be thrown upward and strike the employee on the nose.

Employee was operating a Genie Z-80 Manlift on the North side of the washer. While attempting to scope in the basket, a pin retainer made contact with a section of conduit and caused the manbasket boom to become jammed. Upon trying to unjam the boom, the boom suddenly thrusted upward and caused the employees head to strike a pipe.

DOZER OPERATOR CAME UP FROM BEHIND EE ON PAN, HIT HIM TOO HARD JARRING HIS BACK AND NECK.
EE WAS BENDING OVER TO PICK UP MATERIALS WHEN HE FELT A STRIN IN HIS LOWER BACK.
EE WAS FABRICATING AN ALUMINUM SLIDE GATE BOX FOR THE PEBBLE BIN. WHEN EE ATTEMPTED TO REPOSITION THE BOX, WHICH WAS RESTING ON METAL SAW HORSES, THE BOX SLIPPED AND PINCHED HIS FINGER BETWEEN THE BOX AND SAW HORSE.
EMPLOYEE WAS LOOSENING BOLT, WRENCH SLIPPED, HIS HAND HIT METAL SPUR CAUSING SMALL LACERATION ON RIGHT HAND.

HYDRAULIC OIL SPRAYED ON HIM WHEN HOSE BURST. HE WASHED IT OFF WITH MECHANIC SOAP. THIS CAUSED AN ALLERGIC REACTION.
EE WAS BACKING A MANLIFT WHEN THE LEVEL INDICATOR CAUSED THE LIFT TO ABRUPTLY STOP. HE WAS CARRYING A 1/2 X 4 X 7' PIECE OF ANGLE IRON ON TOP OFTHE BASKET WHICH STRUCK HIM IN THE CHEST AREA WH EN THE MANLIFT STOPPED.
EMPLOYEE WAS STEPPING INTO A JON BOAT WHEN HIS RIGHT FOOT BECAME STUCK IN WET CLAY, AS HE PULLED HIS LEG HE FELT A POP IN HIS RIGHT KNEE.
EMPLOYEE WAS REMOVING BOLTS WITH A 1/2 INCH IMPACT WRENCH WHEN THE SOCKET FLEW OFF AND STRUCK HIM ON THE BACK OF THE RIGHT HAND.
EE WAS SECURING A MOTOR WITH A COME-A-LONG WHEN THE CHAIN SLIPPED AND CAUSED HIS RIGHT ELBOW TO STRIKE THE TAILGATE OF THE PICKUP TRUCK.
EE WAS AIR GOUGING ON A DRAGLINE BUCKET WHEN A SMALL PIECE OF SLAG FELL INTO HIS WORK GLOVE AND BURNED THE MIDDLE FINGER ON HIS LEFT HAND. THIS ACCIDENT REPORTED UNTIL 6-19-01 AFTER THE BURN B ECAME INFECTED.
EE REPORTED TO THE NURSE ON 5/15 WITH WHAT APPEARED TO BE A SLAG BURN ON HIS RIGHT WRIST. HE STATED THAT HE WAS WORKING IN THE MAINTENANCE SHOP ON 5/11 AND WASN'T SURE WHEN IT HAPPENED. THE BU RN BECAME INFECTED AND NEEDED MEDICAL ATTENTION.
WHILE TURNING WRENCH TO LOOSEN OIL FILTER, HE FELT PAIN IN BACK. PULLED MUSCLE.
EE WAS PLACING A PIECE OF CRIBBING UNDER AN OUTRIGGER WHEN HE SMASHED HIS RIGHT RING FINGER BETWEEN THE CRIBBING AND THE OUTRIGGER.
EE STANDING ON PUMP BEARING ASSEMBLY WHEN HIS FOOTING SLIPPED AND CAUSED HIM TO FALL INTO A PUMP SKILL DISCHARGE FLANGE.
DURING THE DAY EE WAS USING A HAND HELD PORTABLE GRINDER IN A CONFINED SPACE. SOMETIME DURING THE DAY A PIECE OF METAL ENTERED HIS EYE BUT IT DID NOT EFFECT HIM UNTIL HE GOT HOME THAT NIGHT. E E WAS WEARING A GRINDING SHIELD AND SAFETY GLASSES DURING THE GRINDING TASKS.
EE WAS IN THE PROCESS OF CONNECTING TWO 100' JOINTS OF 24"" PIPE. ONE JOINT WAS LYING ON THE GROUND WITH ONE END CRIBBED WITH A WOODEN BLOCK. THE OTHER JOINT WAS BEING MOVED INTO PLACE WITH A L OADER WHEN IT STRUCK THE FLANGE OF THE ONE ON THE GROUND. THIS CAUSED IT TO SLIDE ONTO THE EE'S FOOT.

EMPLOYEE WAS CONNECTING TWO 20"" FLEXIBLE PIPES WHEN THE ONE BEING GUIDED BY A SYNTHETIC SLING SLIPPED ON THE SLING AND STRUCK HIS RIGHT KNEE.
EMPLOYEE WAS USING A WEED EATER WHEN HE CONTACTED POISON IVY VINE.
THE GRATING LOCK DOWN PINS CAME LOOSE. AS THE EE STEPPED ON THE GRATING, IT GAVE WAY. HE FELL TO NEXT LEVEL, APPROX. 10-12 FT.

WATER TRUCK HAD JUST MADE A PASS AND THE DIKE WAS WET. THE DUMP TRUCK DRIVER DID NOT ADJUST SPEED AT CURVE AND THE TRUCK SLID OVER THE DIKE AND LANDED ON ITS SIDE.
