Employee felt pain in his stomach and groin area as he attempted to lift a drill head in 4 south left section. Continued to work. *** Diagnosed with Hernia 10/28/2009.
EE was under the 4 South No. 2 belt, at about 24 X cut, changing bottom rollers. The area was wet and muddy. A roller fell about 24" and struck the little finger of the right hand, resulting in an "open fracture", requiring 5 sutures and a splint.
Employee was removing crib block from in front of #11 Voest Alpine Miner with a co-worker in the 3 North Right Section. Both employees picked up a crib block to toss aside when the crib blocks collided catching the tip of injured's right middle finger between the two crib blocks. Employee sustained a fracture to the right middle finger. Employee was wearing Meta Miner gloves.
crew of 4 men were turning over Dosco beams that were stacked upside down, EE did not let go as others were dropping the beam and the tip of his right middle finger was caught between the beam s as he pulled away. the end of his finger was severed and remained in his glove, no bone fragments were in the glove.
Employee was throwing a belt roller under the bottom belt during a belt move in J tailgate section and felt pain in his left groin. Employee has continued to work but was diagnosed with a hernia on 10/5/2009.
Employee threw a track switch on bailey bottom. The switch throw spung back and struck the employee in the right leg. He continued to work with pain in his leg for several days. He went to see a doctor and a blood clot was detected in his calf. Lost work day began 9/30/2009
EE was installing a bottom belt roller on wide side of the 3 north #2 belts at #36 walls when the roller slipped and caught the tip of his left index finger between the end of the roller and frame of roller hanger. EE sustained a laceration to the tip of is left index finger requiring 7 sutures. EE was wearing Meta miner gloves.
While moving roof support supplies, employee was throwing lincoln logs through a man door. While throwing a lincoln log through the door, his glove caught on a splinter on the log causing his glove to be pulled off and then he struck his right hand on the frame of man door.
The shop mechanic was cutting a piece of plexiglass with a circular saw. The dust from cutting accumulated onto his safety glasses. He took off his glasses to clean them and a piece of dust went into his right eye.
13-I-The miner operator was sumping the miner out. The right side miner bolter was beside the miner. Some bolts were stored on top of the miner. One bolt fell, struck a stack of augers bits that were also being stored on the miner. The augers then struck the right side bolter operator on the lip. This resulted in a laceration to his lower lip.
EE was checking the charge on a set of batteries when the lid fell and hit his left hand.
Helping to lift miner cable off of the boom of the miner in 4 South left and felt a pain in his left knee. Lost workdays began 10-28-2009.

Employee claims that while he was taking down water line a piece of the line fell and struck him on the top of the head. ***Lost work days began July 1 2009
The miner was removing rollers and frame. The frame slipped and caught two fingers of left hand between frame and bed rail, cutting 2nd and 3rd finger of left hand.
EE was holding the 16" slider tube with both hands while tow other EE's were sliding the 18" vent tube over it in #5 entry, 3 North Left Section, when the 18" tube caught his left thumb between the slider and vent tube. Ee was wearing gloves.
Employee was picking a 2 inch air pump out of the mud in 13H can line and felt a strain in his groin. ***Diagnosed with hernia and began missing time on 6/19/2009***
EE was sliding a top center belt roller across the top assembly frame at #86 wall, 3 North #3 Belt, when the roller fell and bounced off the bottom belt, catching his left little finger between the assembly frame and belt roller. EE was wearing Meta Miner Gloves. EE sustained a fracture to the left little finger.
Employee was cleaning up at drive 57 building, Bailey Overland. He picked up a 30 pound shoe for a 555 bender and pulled his groin.

Employee was reaching for bedrail being used to hold belt in place while changing out the F-16 belt tail roller. The tail roller swung over and caught the tip of his left little finger between the bedrail and tail roller resulting in a fracture to the tip of the left little finger.
While handling a bit sub, employee lost control of the balance. The sub fell over and pinched his right index finger between the sub and the bumper of the trailer. He required 14 sutures to close the laceration to his finger.
While adjusting the torque on the right side miner bolter, the relief valve struck him in the right hand. Sutures to close the laceration.
While picking up belt structure, employee experienced pain in his groin. **Diagnosed as hernia 9-22-08.

The injured stated he was cleaning safety glasses and mandoseal got in left eye, scratching it.
