Injured had driven a pin out of a conveyor chain when a piece of the pin broke off striking injured in the arm. Injured had no lost time or medical treatment until 6-19-2012 at which time he went to the doctor for infection and was x-rayed.

Employee was hanging the Feeder cable and hurt his left shoulder
The crew was moving power up. EE was pulling Crawler cable slack, when he pulled something in his back.
EE was dragging a 18 H.P. Pump that weighs approx. 300lbs. off # 6 panel approx. 800 feet. EE hurt the lower part of his stomach.
He was hanging a curtain at the miner in #3 heading. The conveyor chain hung up on side board. When it freed up it threw a piece of metal and struck him on the elbow on his left arm. A med treated him and took him to Dr. where he received 5 stitches to his left arm.
EE stated that he was hanging his tie off to a bolt anchored in the roof. He twisted his body when he reached up to hang the shuttle car cable tie off, he felt pain in his neck and right shoulder.
The EE was spreading rockdust from the rockdust bag with one hand, while holding the bag of rockdust with the other hand. He stated that he felt a sharp pain in his lower back. He left work at that time, driving himself home, he went to his doctor the next day.
The EE stated that he was pulling on the cable and the cable stopped he then jerked the cable and he felt pain in his back. He left the job site but returned to work the next day and worked the next 4 days. EE didn't come to work on 4/19/10 and didn't call his foreman. The next day the foreman called him, EE stated that he would be off for a week or two requested by a Doctor.
Employee was flushing out water lines and was hit in the face with debris.
The injured miner was pulling a 4/0 crawler from beltline to the section power center when he felt pain in his lower back. The miner also stated that he had been injured at a former employer that was still paying his medical bills on his back. The foreman should have asked for help when dragging the cable.
The employee was changing rollers out on a conveyor belt in the prep plant when the chain fell from the I beam letting the roller and the chain fall catching the injured persons left hand between the roller and the electrical box.
The injured employee was putting guard back on the No. 1 conveyor belt tail pulley and dropped the guard striking his little finger (pinky finger) on his left hand.
The injured person was removing a piece of rock out of a Long-Airdox bridge conveyor. When he let the rock fall, he didn't move his right foot out of the way letting the rock fall on his right foot.
The injured miner stated that he & 2 other miners were hanging a 6 inch discharge plastic water line. They had it hung 3 times & when they picked it up to hang the 4th time the other 3 hangers broke. The water line fell and hit him on the elbow on the left arm and bruised his arm and caused pain in his elbow.
The injured miner stated that he picked up an acetylene tank and felt back pain.
The EE allegedly strained his lower back trying to lift a scoop charger and put it in a scoop bucket.
The employee claimed he was picking up crib blocks on the outside of the mine and allegedly strained his upper back.
The employee claimed he was getting a rock out of one of the bridges and allegedly strained his lower back.
Lifting to much weight in low coal Alleged strained upper back.
Employee was helping the electrician put a lid on the power center. The lid dropped on employee's left hand index finger. It injured the area from the last knuckle up. The doctor later removed the tip of his finger from the last knuckle up.
Injured was constructing a brattice when a block that he had stacked fell and struck him on the left index finger, fracturing it near the tip. He left the job at 3:00 pm that day and has not returned.
Injured stated he was shoveling loose coal from under the conveyor belt and when he mised a shovel full to empty on the conveyor belt, he felt a sharp pain in his lower back
INJURED STATED HE WAS PULLING THE CONTINUOUS MINER TRAILING CABLE AND PULLED A MUSCLE IN HIS ARM. HE REPORTED THE INJURY ON 8-22-03.
INJURED STATED THAT HE WAS CHECKING THE OIL IN THE RD800 GREASE TRUCK AND AS HE WAS LIFTING THE HOOD HE FELT SOMETHING PULL IN HIS LOWER BACK. INJURED HAD NO LOST TIME UNTIL 7-28-03.
INJURED STATED THAT HE WAS LIFTING A SPEED REDUCER WHEN HE FELT A SHARP PAIN IN HIS LOWER STOMACH. INJURED HAD NO LOST TIME UNTIL 7-29-03.
INJURED STATED HE WAS MUCKING LOOSE COAL FROM UNDER THE CONVEYOR BELT WHEN HE FELT A SHARP PAIN IN HIS BACK, CAUSED FROM SHOVELING.
INJURED WAS APPLYING ROCKDUST BY HAND TO THE CONVEYOR BELT ENTRY. WHEN HE SLUNG A HANDFULL OF DUST HE STRUCK HIS HAND ON A BELT ROLLER, CUTTING HIS SMALLEST FINGER ON HIS RIGHT HAND REQUIRING STITCHES. INJURED HAD NO LOST TIME UNTIL 3-14-03
INJURED SAID HE WAS PULLING ON THE CONTINUOUS MINERS CABLE AND FELT A PAIN IN HIS BACK.
INJURED STATED THAT HE RECEIVED CHAPPING WITH A BURNING SENSATION AS HE WAS APPLYING SEALENT TO A MINE SEAL. INJURED WAS WEARING RUBBER GLOVES THAT COVERED THE AFFECTED AREAS.
THE INJURED WAS LOADING BELT STRUCTURE ON A FLAT CAR. AS HE WAS THROWING THE STRUCTURE ON THE FLAT CAR, HE FELT A SHARP PAIN IN HIS BACK.
INJURED STATED THAT HE WAS LIFTING A WELDER INTO A PERSONNEL CARRIER AND FELT A SHARP PAIN IN HIS BACK. HIS FIRST LOST WORK DAY WAS 10-8-02.
INJURED SAID HE WAS HANGING THE CONTINUOUS MINERS CABLE FROM THE MINE ROOF AND AS HE LIFTED THE CABLE, HE FELT A SHARP PAIN IN HIS BACK.
INJURED HAD OPENED A DOOR THAT RAISED FROM THE BOTTOM AND PROPPED IT UP WITH A ROOF BOLT. AS SOON AS HE PASSED UNDER THE DOOR, HE KNOCKED THE PROP LOOSE, CAUSING THE DOOR TO FALL AND STRIKE HI S BACK. THE DOOR WEIGHED APPROX. 70# & FELL APP. 2 FEET.
THE INJURED WAS REPLACING A LID ONTO A SHUTTLE CAR AFTER SERVICING IT WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.
INJURED WAS REMOVING THE DRIVELINE FROM A MACK TRUCK AND AS HE PULLED THE DRIVELINE LOOSE IT FELL AND STRUCK HIS FOREFINGER REQUIRING 1 SUTURE. HE HAD ONE LOST SCHEDULED WORKING DAY.
THE INJURED WAS MOVING CONTINUOUS MINER CABLE OUT OF THE ROADWAY WHEN HE FELT A PAIN IN HIS LOWER BACK.
INJURED STATED THAT ON 5-15-02 HE WAS MUCKING ON THE BELTLINE & HIS BACK STARTED HURTING. HIS FIRST DAY OFF DUE TO THE INJURY WAS ON 7-11-02. THE DATE OF INJURY WAS DISCOVERED BY COMPANY ON 7- 18-02 WHEN INJURED WENT TO THE OFFICE & FILE AN ACCIDENT REPORT.
THE INJURED WAS THROWING BELT STRUCTURE INTO THE SCOOP AND AS HE DID HIS FINGER GOT CAUGHT BETWEEN A PIECE OF STRUCTURE ALREADY IN THE SCOOP. LACERATION TO INDEX FINGER.
INJURED WAS ADDING BRAKE FLUID TO MASTER CYLINDER OF SCOOP AND GOT SOME FLUID IN HIS EYE.
INJURED WAS HELPING TWO OTHERS PUSH A 3-WHEEL MANTRIP BUGGY THE LAST FEW BREAKS TO THE SURFACE & INJURED HIS BACK. HE TOLD NO ONE OF HIS INJURY UNTIL 3/25/02 AT WHICH TIME HE NOTIFIED THE COMP ANY OVER TELEPHONE THAT HE HAD A CRACKED BACK. INJURED'S FIRST DAY OFF WAS 3/21/02.
EE WAS PULLING ON ROOF BOLT MACH CABLE IN ORDER TO HAVE ENOUGH CABLE TO REACH OUTSIDE ENTRY WHEN HE FELT A PAIN IN HIS WRIST.
EE WAS REMOVING BELT STRUCTURE ON A BELT BACK & THE AREA WAS VERY MUDDY. AS HE ATTEMPTED TO THROW A PIECE OF STRUCTURE, HIS FOOT STUCK IN THE MUD CAUSING HIM TO TWIST HIS LEFT KNEE & LOWER BAC K.
INJURED STATED HE WAS LOADING SUPPLIES IN THE SCOOP BUCKET. HE PICKED UP A BUCKET OF PLASTER ANDAS HE TWISTED AND THREW IT INTO THE SCOOP BUCKET HE FELT A SHARP PAIN RUN UP HIS BACK.
THE INJURED WAS INSTALLING A CONVEYOR MOTOR ON A SHUTTLE CAR WHEN HE FELT PAIN IN HIS LOWER BACK.
THE INJURED WAS MOVING MINER CABLE OUT OF THE ROADWAY WHEN HE FELT A PAIN IN HIS LOWER BACK.
INJURED WAS LIFTING A METAL DOOR TO GAIN ACCESS TO THE FAN ENTRY AND FELT SOMETHING PULL IN HIS BACK.
THE INJURED WAS HAULING SUPPLIES WITH RAILRUNNER AND HAD HIS HAND ON TOP OF RAILRUNNER WHEN ELBOW CAUGHT AGAINST THE ROOF OF THE MINE IN A LOW PLACE CAUSING A LACERATION OF HIS ELBOW AND CONTU SION TO HIS WRIST.
THE INJURED WAS REPAIRING A HYDR. LEAK WHEN HE BRUSHED AGAINST A FRAYED HYDR HOSE AND THE WIRE WHEN INTO HIS UPPER ARE.
THE INJURED WAS WORKING ON START BOX ON ROOF BOLTER WHEN THE START BOX LID FELL OFF BOLT MACHINEAND STRUCK HIM ON THE LEFT HAND.
