Struck by falling object

The injured miner stated that he & 2 other miners were hanging a 6 inch discharge plastic water line. They had it hung 3 times & when they picked it up to hang the 4th time the other 3 hangers broke. The water line fell and hit him on the elbow on the left arm and bruised his arm and caused pain in his elbow.

Accident type, without injuries

The RB #10 mine was on retreat mining. The continuous miner was in the 2nd lift in the 3 entry left pillar about 30 feet into the pillar. A piece of rock fell onto the Emico miner that was 5 feet thick on the miner cutter head and 3 feet thick on the miner tail. The measurement was 5' to 3' thick, 12' wide and 25' long. The rock did not fall into the entry or pull any bolts.

The employee was shoveling the #3 head drive when his shovel became hung in the belt splice and he allegedly twisted his back.

Injured was in the bridge conveyor with a come-along tied to the bridge pulling it over to reload it onto the carrier when the bridge was accidentally started causing injured to fall injuring his shoulder.

INJURED WAS CARRYING A BOX OF GLUE FOR HIS ROOF BOLTS FROM THE BACK TO THE FRONT OF THE ROOF BOLTING MACHINE WHEN HE SLIPPED & TWISTED HIS KNEE.

AN UNPLANED ROOF FALL(40' L X 20' W X 0'-5' H) OCCURRED IN ONE OF TWO INTAKE ROADWAYS BEING # 4 ENTRY OF FIVE. THE FALL WAS LOCATED 30 CROSSCUTS OUTBY THE WORKING SECTION AND BETWEEN THE #25 & 26 CROSSCUT INBY # 4 HEAD DRIVE. NO EQUIPMENT WAS INVOLVED. THE FALL HAD NO AFFECT ON VENTILATION. THE AREA WAS DANGERED OFF WITH CRIBBS AND TIMBERS.

INJURED STATED THAT HE WAS SHOVELING FROM UNDERNEATH A BELT TAKE-UP AND BUMPED HIS KNEE ON EITHER THE METAL PART OF THE TAKE-UP OR THE MINE FLOOR. INJURED FINISHED THE SHIFT AND NEVER RETURNED . INJURED VISITED A DR. ON 11/6/03 THEN REPORTEDTHE INJURY AS WORK RELATED BY TELEPHONE THE SAME NIGHT. INJURED FILED A FORMAL ACCIDENT ON 11-7-2003.

INJURED STATED THE HE WAS PULLING THE CONTINUOUS MINERS CABLE WHEN HE SLIPPED IN THE MUD AND TWISTED HIS KNEE. INJURED HAD NO LOST TIME UNTIL 2-7-03. AT THIS DATE HE TOOK OFF FOR MINER KNEE SU RGERY. HE IS SCHEDULED TO BE OFF WORK 4-6 WEEKS.

INJURED SAID HE WAS HANGING THE CONTINUOUS MINERS CABLE FROM THE MINE ROOF AND AS HE LIFTED THE CABLE, HE FELT A SHARP PAIN IN HIS BACK.

INJURED STATED HE WAS REMOVING A PLUG TO CHECK THE OIL IN THE HEAD OF THE CONT MINER AND THE WRENCH SLIPPED CAUSING INJURED'S HAND TO STRIKE A BIT ON THE HEAD OF THE MINER. INJURED HAD NO LOST TIME UNTIL 8-7-02 AT WHICH TIME HE WENT TO THE DR AND DISCOVERED HIS HAND HAD BECAME INFECTED.

INJURED WAS ADDING BRAKE FLUID TO MASTER CYLINDER OF SCOOP AND GOT SOME FLUID IN HIS EYE.

Over-exertion in pulling or pushing objects

INJURED WAS HELPING TWO OTHERS PUSH A 3-WHEEL MANTRIP BUGGY THE LAST FEW BREAKS TO THE SURFACE & INJURED HIS BACK. HE TOLD NO ONE OF HIS INJURY UNTIL 3/25/02 AT WHICH TIME HE NOTIFIED THE COMP ANY OVER TELEPHONE THAT HE HAD A CRACKED BACK. INJURED'S FIRST DAY OFF WAS 3/21/02.

Caught in, under or between a moving and a stationary object

HE WAS ATTEMPTING TO DRAG A ROLL OF CONVEYOR BELT OUT OF THE SCOOP BUCKET & HAD HIS TOES HOOKED ON THE EDGE OF THE BUCKET FOR LEVERAGE. THE SCOOP OPERATOR LET THE BUCKET DOWN & CAUGHT TOES OF HIS RIGHT FOOT UNDER THE EDGE OF THE BUCKET. HE SUFFERED BRUISES TO HIS TOES.

INJURED STATED THAT HE WAS MUCKING THE CONVEYOR BELT AND HIS FOREARM STRUCK THE BELT AT THE SAME TIME A SPLICE WAS PASSING BY AND THE PIN IN THE SPLICE CUT INJURED APPROX. 2"" REQUIRING 4 STITC HES.

EE WAS TRAMMING THE SCOOP WHEN HE RAN OVER A ROCK IN THE ROADWAY AND JARRED HIS BACK. HE WENT TO THE HOSPITAL WHERE HE WAS CHECKED & RELEASED. NO LOST TIME UNTIL 3/12/02.

INJURED HAD HELPED REMOVE ROCK FROM THE MOBILE BRIDGE CONVEYOR. HE WAS POSITIONED LOOKING TOWARD THE OUTBY DUMPING POINT OF THE MBC WITH HIS HAND LOCATED ON TOP OF THE MBC, JUST ABOVE THE CONVEYOR THROAT. A LG. ROCK CAME DOWN THE CONVEYOR AND STRUCK INJURED ON THE TIP OF SMALLEST FINGER OF THE RT. HAND.

INJURED'S DRILL STEEL HAD BECOME CAUGHT IN THE DRILL HOLE. INJURED WAS TRYING TO GET THE STEEL FROM THE HOLE WHEN THE FINGEROFHIS GLOVE GOT CAUGHT WHILE HE WAS USING THE TOTATIONOF THE ROOF BO LTING MACHINE. HIS FINER WAS DISLOCATED. INJURED HAD TWO LOST WORK DAYS.
