A trained longwall shearer operator was removing stone off the shearer and stone rolled onto his ring finger. 5 stitches
Employee stopped to pick up a roll of air line. As he bent over he felt something in his back.
Employee was hanging canvas on the rib when he felt a pop in his back.
Employee was standing beside a roll of inch and a quarter plastic slick line while it was being unrolled. The employee attempted to put marking tape on the end of the roll when it finished unrolling from a hanging support and the end of the plastic line struck his safety glasses and he received a laceration to his face at the right eyebrow. The employee received sutures.
A fully trained loader operator bent over to pick up the loader cable and felt a pop in his back.
IE was installing a piece of 12 gauge "V" deck panel in the steel sets that had been previously installed. A piece of rock came dislodged and was laying on the "V" deck panel pinching IE's right hand between decking and beam resulting in laceration and fracture to right ring finger
A fully trained miner was unloading an air drill used for bolting with the help from bolt partner upon lifting the drill he felt pain in his right shoulder and back he needed to have better body positioning while lifting he was wearing all required PPE
A fully trained miner was mudding a wall in 15 west the b-bond dripped inside of his gloves and irritated his right arm to his wrist the miner was not wearing the proper gloves for this job and was not wearing long sleeves either.
A fully trained miner was wearing his metatarsal gloves and was loading 4" waterline onto a supply car. While loading the waterline his right ring finger got caught between the waterline and bulkhead of the car resulting in a fracture and laceration.
A fully trained miner was moving up the snub for the shuttle car, the employee bent over to pick up the buggy cable and twisted his lower back. The area was dry and level all PPE was not a factor.
A fully trained miner was repairing a omega block wall while putting a 2"x12" block on top of the wall fibers from the block fell in behind his safety glasses and getting into his left eye.
A full trained miner had replace the switch at 10 drive and was picking up old piece of 15'-20' rail to load on car set one end of rail on the car with help and the rail bounced off the car landing on the left foot of the miner . He was wearing leather metatarsal boots.
A fully trained miner was lifting a 10 ton jack that weighs approximately 25 lbs. out of a box to put on a mantrip and strained his lower back. The area was dry and flat level surface .Employee did not use proper lifting technique proper PPE was not a factor.
Fully trained miner was loading belt stands onto a flat car in the 14 east section. The employee caught R middle finger causing a laceration he was wearing the proper gloves and required PPE. The area was dry and level and accident was due to poor hand placement and concentration.
A fully trained miner was clearing out gob from the head gate in 13 east at #4 shield and was removing a piece of metal from the walkway of pan line. While he was bent over a 2'x2'x6" stone came out of the pan while clearing the chain landing on his right foot causing a contusion. He was wearing Rubber metatarsal boots at the time. The pan was not in motion

A miner while carrying a Z-drill down the 5n 2 belt. While carrying the drill through the steel sets at 40 wall he twisted and felt a muscle pull in his lower back.
While laying rail at the top of 4 track in dry level conditions a fully trained miner had a piece of 85 lb rail rolled on of his right foot by the other track man. The proper tools were provided this was a miscommunication between the workers. The injured miner was transported by ambulance to the hospital where he was diagnosed with a broken bone in his right foot.
While using a rail roller to move 85# rail, the miner positioned himself in the middle of the rail 60' in length on dry level surface. He then squatted down to roll the rail over. When the rail rolled, the ends of the rail was not on the ground the rail flopped and jarred his back. This also resulted in the rail landing on another track man's foot.
A fully trained electrician strained his lower back while hanging highline from the 12 East VCB which was causing the power to knock at the power center at 156 wall. The employee did not report this accident until he went to the hospital on 7/12/15. Employee started losing time on 7/13/15.
A trained miner was re-railing a bus at 26 break on 7 belt and had hooked a chain from 1 bus to another bus to try to get the bus back on the rail the chain had became the grounded between the 2 buses and got hot. The employee grabbed the chain to assist himself up and was not wearing gloves and resulted in his right hand and fingers to be burned.
A fully trained miner was helping transport another employee on a stretcher and when he bent down to help lift the stretcher the employee heard a popping sound in his left knee and had a stinging sensation in his knee.
A fully trained employee was tossing a crib block under a shield toe and pinched his right index finger between the crib block and shield toe
A fully trained employee was shoveling spillage from underneath the #2 refuse belt to the #1 refuse belt. With a loaded shovel he turned and felt a pop in his right shoulder.
A fully trained longwall mechanic was repairing a hose for dust control at 55 shield . When the employee was knocking out the staple from the shield the pressure released and he was struck by the emulsion lacerated the tip of pinky finger. Employee had removed the staple from the wrong Tee .
A employee was dropping his basket in the blake ridge locker room and there was a burr on the chain cutting his left hand resulting in 5 stitches to the ring finger.
Employee was completing training as a center bolter helper 20 16' bolts had been installed. The operator had drilled 15' in this hole as EE dropped the mast the injured said raise the mast back up. When the mast was raised his pinky finger and ring finger got caught between the drill steel and mast.
A fully trained employee was moving rockdust hose to second setup from 46 wall to 41 wall pulling hose along beltline for a 1/2 block long. Employee struggled to pull hose and the employee strained his lower left back.

Employee was hooking up 6 empty cars in 9 west spur coupled cars hooked up safety ropes. Employee bent down to remove wheel skids cars rolled and caught finger between wheel and skid.
Employee was shoveling on the belt conveyor. As he pushed his shovel into the pile of sluffage his wrist and forearm began to swell. Employee continued to work until 1-31-15 when he began to miss time.
A trained rock dust motorman was throwing a track switch and he felt pain in his abdomen.
Employee was removing a draw bar from a locomotive and dropped it on his foot. Employee continued to work until 1-30-2015 and then began losing time.
Employee was working on the longwall face as a fully trained shieldman and picking up stones in the shield line. He felt pain in abdomen. He continued to work and while shoveling felt more pain. Started losing time as of 1-17-15.
Employee was removing a pin from a locomotive and pinched a finger. Contusion.
employee was installing legs on a pan at the belt, legs had not been tighten and pan rolled mashing his left hand between a leg and the bedrail. (fractured left middle finger)
Employee was stacking Omni-Props and had his finger in between 2 props when he sat one down. The sharp flat edge caught his index finger. Required 4 stitches.
A shuttle car operator got a ventilation tube stuck on his shuttle car. He got out of the shuttle car and grabbed the tube by the nylon strap. Stored energy released causing the nylon strap to lacerate his index finger. Stitches.
A trained belt operator was pulling belt rollers across the cat walk in the bunker. As he was dragging it he felt a pain his shoulder. Began losing time on 11/30/2014.
A laborer was unloading steel beams off a mine car. The beams shifted catching his thumb.
A trained general inside laborer was carrying belt bed rails from the belt to the track and placing them on a mine flat car. The injured and another employee was laying a bed rail on the car when the injured finger was caught between two bed rails. He was wearing gloves.
A fully trained roof bolter operator was roof bolting when the drill became stuck in the roof. He pulled on the drill steel and felt pain in his back. He continued to work and after 2 hours requested medical treatment. He complained of back pain.
moving steel beam from track to beltline, two beams slide together and smashed the employees' left middle finger. (fracture)
