Employee reached behind a tool box to retrieve some welding rods. As employee pulled the welding rods out the employee lacerated left forearm on a sharp edge of the toolbox. Employee received 10 sutures to the wound.

Employee was putting a split header on a roof bolt while standing on a ladder. Another employee handed a header and as employee raised the header ee felt something pop in left shoulder. EE began missing work on 8/16/2017.

Employee was lifting two five gallon oil cans when EE felt something pop in left elbow. EE continued working and later sought medical attention. A medical procedure was performed on the elbow on 1/17/2017 making this incident reportable.

While employee was pulling a piece of top belt structure out of the belt, the structure became stuck. EE pulled harder and when the structure released, EE fell backwards with the structure hitting right leg below the knee causing a laceration requiring staples. EE returned to normal duties the next working shift.

While employee was hanging a shuttle car cable, he missed the hanger causing he cable to pull his arms forward straining his right shoulder. He continued working but began missing work on 1/18/2016 when he had surgery to repair a tear in the shoulder.

Employee was taking a belt spreader our during a belt pull back when he felt pain in his right bicep. He continued working and sought medical treatment on 11/03/2015 for a surgical procedure on a tendon in the arm. He returned to work his next scheduled shift.

Employee was moving a sump pump. The pump line was tied off at the pump and lo-trac forks. While the operator was pulling the waterline with the lo-trac, the waterline got caught on the feeder causing the rope hangers to break allowing the pump to strike the operator on the left side of the face. He suffered fractures to the left side of his face.

While employee was tramming a roof bolter from #3 to #2 entry, he engaged the cable reel take up on the roof bolter which caused the cable to pull a rope hanger attached to the cable from a rib bolt. The hook on the hanger struck the employee in the right eye causing an eye injury that required medical treatment and lost days. He was wearing eye protection.

While pushing a skirt at a tail piece, the employee experienced pain in his right arm. It was later determined that he had suffered a torn tendon in his elbow. He had surgery on the elbow on 1/30/2012 and missed two shifts of work.

His regular job is shoveling belts and he began having pain to his lower back and said it became worse as the morning progressed. He complained to his supervisor and was taken outside. He was able to drive himself for medical evaluation and treatment.

When the employee took a bolt out of the rear end of a scoop, the rear end dropped down and caught the employee's left hand between the impact wrench he was holding and the floor of the shop. The employee suffered a laceration to the palm of his left hand.

While changing out a scrapper, the scrapper fell catching employee's right little finger between the scrapper and crossunder guard. The finger was lacerated and fractured just behind the fingernail. The employee returned to work his next scheduled shift.

EE reported that he bent over to pick up one end of a jack to set it for roof support. When he raised up, holding the end of the jack, he said he felt a "pop" in his mid lower back area. He finished his shift and sought medical treatment the following day, due to continued pain to his lower back.

Employee had been lifting belt structure during his shift on 8-18-09. At approx. 5:00 A.M. he complained of severe pain to his lower back and was taken to Lonesome Pine Hospital for evaluation. He was treated and released to the care of his regular physician.

Employee was near the end of a power center, watching a bridge cable being pulled through a sheave wheel attached to the power center. The cable hung up, causing the power center to move onto the employees hand. He suffered a dislocated little finger and a laceration, requiring sutures on his ring finger on his right hand.

The EE was taking down a kennedy stopping panel next to the coal rib. As the panel was removed, a piece of draw rock fell, striking the EE and caused him to fall to mine floor. The panel then fell, striking him on the wrist, causing a laceration that required sutured. The rock measured 17" X 14" and was 2" in thickness.

EE has made a claim under our worker's comp alleging that he injured his back while lifting a 50 lb bag of block bond on 2/24/07. He had not made any mention of injury that day to his supervisor or to his co-workers. It was 12 to 14 days before we learned he was seeking treatment. I then had to wait to hear from him to get details as he had had his phone number changed.

HE STATED THAT HE WAS IN THE PROCESS OF MOVING A PINCH ROLLER FOR A BELT STORAGE UNIT USING A DIESEL LO-TRAC. AS HE WAS LOOSENING THE C.M. PULLERS THAT HAD THE ROLLER SECURED, THE WEIGHT SHIFTED AND CAUGHT IN HIS LEFT MIDDLE FINGER BETWEEN THE CHAIN & FORK OF THE LO-TRAC, MASHING IT. THIS RESULTED INTO A FRACTURE TO END OF HIS FINGER & STITCHES.
