Employee was working on bucket teeth and adapters on a shovel bucket. When attempting to remove the G.E.T. lock out the keeper pin fell out and pinched employee's hand between the keeper and a 2 inch plate. The plate was being used to press the wedge up using a ram and hydraulic pump. Injury required stitches to close the wound.

Employee was removing an oil pump from a dozer with a chain hoist. As EE was holding it steady with EE's hand, the pump slipped from the chain hoist, pinching EE's finger. This caused a laceration that required 9 stitches to EE's ring finger

The Employee went to Med Express on 6-27-14 and put on restricted duty. employee Notified his employer of seeking medical attention on 7-1-14

Employee was operating equipment, was in the process of adjusting the width of the forks on a JLG forklift. The employee attempted to push the fork from atop the vertical divider by using his foot. The employee placed his right hand in the crossbar support area to steady himself. As force was applied, the crossbar seated, smashing the employees thumb.

Employee was pulling cable when a loop in the cable caught his right foot, pulling him down and injuring his right knee and leg. He was seen in the ER later that evening and diagnosed with a strained muscle and small fracture of the lower right leg. He has been excused from work.

Employee was loading bolts and glue into a scoop bucket when he twisted with materials in hand and heard a "pop" and felt pain in his back. He continued to work his shift but the following day, he went to the ER and was excused from work.

Injured employee was pulling on a beam when it became fouled and when it came loose it struck employees left knee. He was treated at the ER and released to return to work. Employee returned to work the next day but due to continued discomfort, employee went back to the ER on 11/23/12 and was excused from work.

Employee was traveling down the slope belt entry on a rubber tire 4-wheeler when the chains broke on the suspended belt structure causing one side of the structure to fall. The structure struck the roll bar of the 4-wheeler and the employees left shoulder and head causing swelling and bruising. He was transported to ER by EMS.

Injured employee was setting a beam when he injured his left shoulder. While setting the prop, the bottom of the jack slipped causing the beam to fall striking him on the left shoulder. He refused EMS and finished his shift. He was then transported to the ER by a Safety staff member where he was diagnosed with a Left Shoulder Contusion and excused from work for 7 days.

Injured employee was using B-Bond/Mine Sealant to plaster a set of doors when he got the sealant in his left eye. The eye was immediately flushed and he was transported to the ER. He was treated for a chemical injury to the left eye, prescribed medication for his eye and referred to Optometry. He was released to return to work.

He was excused from work due to the risk of infection secondary to history of cellulites. He returned to work on 10/10/11.

It has been determined on this date (11/02/2011) that the employees injury resulted from an incident at work prohibiting him from working. On 07/27/2011 the injured employee was pulling on a wash down hose causing him to strain his left shoulder and neck. First day away from was 07/29/2011. He remains excused from work as of 11/02/2011.

Injured member was lifting supplemental roof support timbers when he twisted his left knee. He did not cease work and continued to work his regularly scheduled shifts until May 25, 2011. Due to continued discomfort, injured was evaluated and excused from work by his doctor and awaiting an MRI on his knee.

Injured employee was shoveling the 4C tailpiece when he began to experience lower back pain. He was transported to the ER by EMS where he received a CT Scan and referral for an MRI. MRI revealed inflammation and swelling causing pressing against the sciatic nerve causing lower back and left leg pain. He remains excused from work.
