Employee was working on bucket teeth and adapters on a shovel bucket. When attempting to remove the G.E.T. lock out the keeper pin fell out and pinched employee's hand between the keeper and a 2 inch plate. The plate was being used to press the wedge up using a ram and hydraulic pump. Injury required stitches to close the wound.
Hydraulic ram broke free from pressing pin out of bores on CAT 011R Dozer and struck employee on the head.
Employee was removing an oil pump from a dozer with a chain hoist. As EE was holding it steady with EE's hand, the pump slipped from the chain hoist, pinching EE's finger. This caused a laceration that required 9 stitches to EE's ring finger
Employee stated while changing a drill bit in a hand held drill, while tightening the drill chuck the employee stated he felt something snap in his upper right arm. an accident report was filed and the employee continue to work. The Employee went to Med Express on 6-27-14 and put on restricted duty. employee Notified his employer of seeking medical attention on 7-1-14
Employee was operating equipment, was in the process of adjusting the width of the forks on a JLG forklift. The employee attempted to push the fork from atop the vertical divider by using his foot. The employee placed his right hand in the crossbar support area to steady himself. As force was applied, the crossbar seated, smashing the employees thumb.
Employee was pulling cable when a loop in the cable caught his right foot, pulling him down and injuring his right knee and leg. He was seen in the ER later that evening and diagnosed with a strained muscle and small fracture of the lower right leg. He has been excused from work.
Employee was loading bolts and glue into a scoop bucket when he twisted with materials in hand and heard a "pop" and felt pain in his back. He continued to work his shift but the following day, he went to the ER and was excused from work.
Injured employee was pulling on a beam when it became fouled and when it came loose it struck employees left knee. He was treated at the ER and released to return to work. Employee returned to work the next day but due to continued discomfort, employee went back to the ER on 11/23/12 and was excused from work.
Injured employee was opening a metal storage container when he smashed his left middle finger between the handle and container causing a fracture to the distal tip of his left middle finger. He was seen and treated at the ER.
Injured employee was climbing into the coarse ref screen when he began to experience sharp pain in his back and neck. He continued to work but went to the ER later that day. The ER physician excused him from work.
Injured employee and co-worker were carrying long piece of metal. Co-worker dropped his end, causing metal to slip from injured's hands resulting in laceration to right index/middle fingers.
Employee was assisting with the installation of a piece of pipe and apparently twisted suddenly, causing him to feel pain in his low back.
Employee was lifting a bottom roller when he began to experience lower abdominal pain. He was seen by the ER physician and diagnosed with a Abdominal Hematoma and excused from work.
Employee was traveling down the slope belt entry on a rubber tire 4-wheeler when the chains broke on the suspended belt structure causing one side of the structure to fall. The structure struck the roll bar of the 4-wheeler and the employees left shoulder and head causing swelling and bruising. He was transported to ER by EMS.
While lifting material at the stage loader, employee began to experience lower back pain. He completed his shift and was seen at the ER later that day. He has been excused from work.
Injured employee tossed a piece of material onto the belt when it struck a slate bar that was on the belt and the slate bar flew up and struck employee over the left eye causing a laceration. He was transported to the ER where he received 5 sutures and was released to return to work.
Injured employee was setting a beam when he injured his left shoulder. While setting the prop, the bottom of the jack slipped causing the beam to fall striking him on the left shoulder. He refused EMS and finished his shift. He was then transported to the ER by a Safety staff member where he was diagnosed with a Left Shoulder Contusion and excused from work for 7 days.
Injured employee was using B-Bond/Mine Sealant to plaster a set of doors when he got the sealant in his left eye. The eye was immediately flushed and he was transported to the ER. He was treated for a chemical injury to the left eye, prescribed medication for his eye and referred to Optometry. He was released to return to work.
Injured employee was lifting belt structure when he suddenly began to experience lower back pain. He was transported to the ER. The ER physician excused him from work and referred him for a MRI due to the possibility of a herniated disc.
Employee was installing pipe stubs between floor beams. Picked up on end of pipe section and felt pain in low back.
Injured employee was replacing a piece of tin when the corner of the tin punctured his glove causing a small laceration to the top of his left hand. He received 3 sutures.
While cleaning the dozer the injured employee scraped left forearm. He continued to work his shift but the following day he was seen by his physician and excused from work for two days. He was excused from work due to the risk of infection secondary to history of cellulites. He returned to work on 10/10/11.
Injured was checking broken piece of angle on de-watering screens when right middle finger was smashed between screens and metal frame. Received 5 sutures.
It has been determined on this date (11/02/2011) that the employees injury resulted from an incident at work prohibiting him from working. On 07/27/2011 the injured employee was pulling on a wash down hose causing him to strain his left shoulder and neck. First day away from was 07/29/2011. He remains excused from work as of 11/02/2011.
Employee was helping change out coal screen and thought a metal shaving might have gotten in his finger which became infected. Employer not aware of treatment until 7/22/11, at which time, employee prescribed antibiotic.
Injured member was lifting supplemental roof support timbers when he twisted his left knee. He did not cease work and continued to work his regularly scheduled shifts until May 25, 2011. Due to continued discomfort, injured was evaluated and excused from work by his doctor and awaiting an MRI on his knee.
While attempting to pull gate shield the hook slipped off and struck member on left side of his face requiring 7 sutures.
While loading belt structure into mantrip he mashed his left little finger between two rollers requiring 7 sutures
Injured was shoveling the off side of the belt near the rock box when he began to experience lower back pain. He finished his shift but later went for medical evaluation and was diagnosed with a lower back strain and he was excused from work.
Injured member was changing the bits on the head end of the shear when a small piece of metal from the bit or hammer broke off and became embedded in his left hand. The metal was removed on 03/14 and he received 4 sutures. He was excused from work on 03/14 & 03/15. Returned to work on 3/16 with a full release.
Injured employee was adjusting the tail roller bearing when it slipped smashing the employees right little finger between the bearing and tailpiece frame. ER diagnosis: Fracture of right little finger and referral to specialty care. ER physician excused injured from work.
Employee was moving an extension ladder and pulled a muscle in his lower back.
Employee was using a torch to cut a piece of pipe. While handling the pipe, it slipped out of his hand and he moved suddenly to try to catch it and it hit his hand.
Employee was lifting a 5 gallon bucket with some concrete in it and felt something pull in his lower left side.
Injured employee was bent over picking up a belt rail as he raised up with the rail he began to experience lower back pain. He finished his shift but did not return to work the next day. He was seen at the ER and diagnosed with a lower back strain and excused from work for two (2) days.
Injured member was lifting a pump at the bottom of the slope, when he felt a sharp pain in his abdominal cavity and he noticed a small discharge of blood from his rectum. He has not returned to work.
Injured employee was loading materials into the back of a pick-up truck when the employee's left middle finger was smashed in the tailgate causing a laceration. Employee received 11 sutures.
Part 50 Audit. Employee was repairing an A/C Unit on a D9 Dozer when he felt and heard his right shoulder "pop" causing a strain. Employee was referred and received physical therapy treatments post accident.
Injured employee was hanging a curtain when his arm contacted a roof bolt plate and caused a laceration. Injured received 4 sutures.
Employee was pulling a piece of handrail into the plant when his hand slipped and bumped against a piece of decking or sheeting, causing a laceration.
Injured was hanging a curtain when he lacerated a finger on his right hand on the curtain hook. Injured received 2 sutures.
Injured was loading kennedy panels in the scoop bucket when his left hand was smashed between the scoop bucket and the panel. Diagnosis: Fractured hand. Injured has been placed on restricted duty.
He was removing fly pads to move up to the next line. He cut the back of his left hand on a pizza pan, requiring 5 stiches.
Part 50 Audit. Employee was removing the pump barrel from the vessel pump when he smashed his left ring finger causing a laceration. Employee was transported to the ER by EMS. EMS reported that the employee lost consciousness sometime after the incident.
Member was picking up a 6x6 timber and felt pain in his back, MRI on 3-8-2010 reported a strain muscle and was placed on restricted duty.
Part 50 Audit. Employee was installing belt rollers when he began to experience back pain. Employee was excused from work for 4 days.
EE was carrying a door for a scoop and felt pain in his lower back, this door estimated to weigh about 40 pounds. He called in on 3-3-2010 and stated that the doctor had removed him from work and he was referred to a neuro-surgeon for futher evalation.
Member states he was lifting belt structure and felt a pain in his back, he worked until 3--13-2010 when he went to doctor and was removed from work.
Stated that while lifting belt structure he felt a pain on his left side of lower back.
Injured employee was shoveling the 4C tailpiece when he began to experience lower back pain. He was transported to the ER by EMS where he received a CT Scan and referral for an MRI. MRI revealed inflammation and swelling causing pressing against the sciatic nerve causing lower back and left leg pain. He remains excused from work.
