Employee was installing a man door in a previously built stopping. He placed his left hand on the bottom row of block and a block fell from the top row of block causing a laceration to his index finger.
EE 1 break inby #2 belt head helping recover rail. Standing next to rail being lifted by fork-lift. Rail fell due to mud on it, struck EE's right foot. Mine worked out, no one scheduled to work there after 2/18/09.
Employee was replacing right cutter motor guard, the guard fell onto and cut the right ring finger. He was taken to the e.r. for treatment. He had stitches inserted to close the wound, and returned to work.
Employee was helping two others hang miner cable. Area was muddy; while lifting and positioning it on the hangers, he developed pain in his lower back. He ceased work to let the pain subside but it increased. Employee was taken to the hospital for treatment. X-ray was negative; diagnosed with low back muscle strain.

INJURED ASSISTED THREE CO-WORKERS INSTALLING A 6' X 7' METAL DOOR AT THE PORTAL ENTRANCE. WEIGHT SHIFTED AND EXTRA EXERTION CAUSED THE BACK INJURY.
MEMBER WAS ADJUSTING FORK ON A FORKLIFT WHEN IT CAME OFF AND FELL STRIKING HIS RIGHT FOOT.

MEMBER WAS LOADING BAGS OF REDI MIX CONCRETE ONTO A TAG ALONG TRAILER AND FELT PAIN IN HIS BACK.
EMPLOYEE STARTED MISSING WORK 10/26/04. HE IS SEEKING DIAGNOSTIC TREATMENT FOR BACK PAIN. WE ARE FILING THIS REPORT AS AN OCCUPATIONAL INJURY. COPIES OF DR.'S REPORTS/NOTES WILL FOLLOW WHEN WE RECEIVE THEM. Revision per misc. mail 1/10/05 #17: Member was helping carry a filing cabinet up a set of steps, hurting his lower back.
Employee stated he was shoveling coal with a #4 aluminum shovel from the location of the 'land mine' under the belt guard at #6 belt head. He shoveled a load of coal and mud pulling the shovel load out to place it on the belt, twisted around, and as he lifted the shovel, he experienced a pulling sensation in his back.
EE HAD MADE A BELT SPLICE AND WAS REMOVING THE OLD PIN FROM THE OLD SPLICE. HE TOOK HIS SAFETY GLASSES OFF TO WIPE HIS EYES OFF WITH A RAG AND THEN HE FELT SOMETHING IN HIS RIGHT EYE.
EE WAS UNLOADING BELT STRUCTURE FROM THE SUPPLY CAR. A BOTTOM STAND FELL OVER, HITTING HIM ON HIS RIGHT HAND, CAUSING A FRACTURE IN HIS RIGHT THUMB.

Employee was in #7 entry working on the miner cable by himself. He said as he was pulling on the cable trying to get it out of the conduit, he twisted his left knee.
Employee was running the Roof Bolter in the 5R section crosscut. He noticed a crack in the roof running across the entry. He prepared to support the crack by laying a strap across the ATRS. When he did, the strap slid off the ATRS and struck him in the top lip causing a small laceration. He sought medical attention and recieved 2 stitches to the upper lip.
THE EE WAS SUPPLYING HIS BOLTER WITH ROOF BOLT PLATES. THEY HAPPENED TO BE WET AND SLICK. HE HAD A BUNDLE OF PLATES IN HIS HAND WHEN THEY BEGAN TO SLIP THROUGH HIS FINGERS. HE TRIED TO CATCH THEM AND CUT HIS FINGER ON THE EDGE OF ONE OF THE PLATES. HE HAD GLOVES ON AT THE TIME OF THE ACCIDENT. HE RECEIVED 7 STITCHES TO HIS LEFT MIDDLE FINGER.
EE WAS OPERATING THE CONTINUOUS MINER. HE STATES THAT HE THREW THE MINER CABLE AND WATER LINE OVER TO THE RIB AS HE NORMALLY WOULD DO AND HE FELT PAIN IN HIS BACK - SHOULDER AREA. (RIGHT SHOUL DER).
EE WAS CROSSING #5 BELT TO REPAIR A SPLICE. CO-WORKER HAD WALKED TO THE MAN BUS TO GET NAIL BOARD, WHEN INJURED YELLED AT HIM. WHEN HE ARRIVED ON THE SCENE, HE WAS LYING ON THE OPPOSITE SIDE O F THE BELT. HIS RIGHT LEG/KNEE WAS OVER THE BELT RAIL WITH HIS FOOT PINNED UNDER THE TOP BELT.
EE WAS ATTEMPTING TO PULL A ROOF BOLTER CABLE OFF WHEN THE REEL GOT, FOULED, HE TRIED TO PULL A SECOND TIME AND FELT A PULL IN HIS TORSO.
INJURED WAS STANDING NEXT TO A 4" VICTOLIC WATER LINE. THE LINE WAS CONNECTED BY A GRIPPED TYPE COUPLING. WHEN LINE PRESSURE WAS APPLIED, THE COUPLING FAILED. THE FORCE OF THE LINE STRUCK INJU RED'S LEFT KNEE.
EE PULLED ON A PIECE OF STRUCTURE THAT WAS AMONGOTHER STRUCTURE. WHEN THE STRUCTURE FREED, IT PENETRATED HIS LEFT THIGH(UPPER). HE WAS TRASPORTED TO THE HOSPITAL BY AMBULANCE.
WHILE SHEAVING THE SECTION BELT TOGETHER THE HOOK PULLED THROUGH THE BELT HANGER PLATE IT WAS ATTACHED TO. THE SHEAVE BLOCK STRUCK INJURED ON ARM.
EE WAS HANGING MINER CABLE ALONG WITH HIS CO WORKER EE STRUCK. HE EXPERIENCED PAIN IN HIS BACK AS A RESULT OF HANDLING THE CABLE.
EE WAS USING A JACK BAR TO CONNECT A FLAT CAR & A 12 TON MOTOR TOGETHER. WHEN HE FINISHED LININGUP THE TWO VEHICLES TO PUT IN THE LOCK PIN, HE LAID THE JACK BAR ON TOP OF THE MOTOR. THE BAR RO LLED OFF AND CAUGHT HIS LEFT THUMB.
EMPLOYEE DISMOUNTED THE SCOOP TO MOVE A ROCK IN THE ROAD WAY. AS HE MOVED THE ROCK HE FELT PAIN IN HIS LOWER BACK.
EE WAS CARRYING TACK TIES WHEN HE SLIPPED ON A ROCK, AND THE TIE HE WAS CARRYING, FELL AND STRUCK HIM IN THE BACK. HE WORKED UNTIL FEB. 26TH AND HAS NOT RETURNED TO WORK.
WHILE IN NORMAL TRAVEL TO POWER CENTER, EE EXPERIENCED PAIN IN HIS GROIN AREA THIS WAS 2/12/01 EE CONT TO WORK UNITL 2/20/01 AT THIS TIME HIS DR RECOMMENDED 8-10 DAYS REST
EMPLOYEE WAS COMPLETING AN OVERCAST. ONE OF THE METAL BARS SLIPPED OFF THE BRATTICE CAUSING IT TO FALL. EMPLOYEE WAS STRUCK BY THE METAL BEAM AND BLOCKS IN NECK AND LOWER BACK.
EE WAS HELPING TO ATTACH A FLAT CAR TO A MOTOR. THE ENDLOADER OPERATOR WAS PSOITIONING THE FLAT CAR SO THAT EE COULD INSERT A PIN INTO THE HITCH. THE FLAT CAR SLIPPED, CRUSHING EE'S LEFT MIDDL E FINGER.
EE WAS PULLING CABLE OFF THE ROOF BOLTER ROOF. AS HE PULLED, HE FELT PAIN IN HIS LOWER BACK. HE FINISHED HIS SHIFT, BUT WAS UNABLE TO RETURN TO WORK THE FOLLOWING DAY.
EE WAS TAKING THE CRIB BLOCK OUT FROM UNDER THE FEEDER - PREPARING TO MOVE THE SECTION BELT, WHILE THROWING A CRIB BLOCK ON THE FEEDER, HE CAUGHT HIS FINGER BETWEEN THE EDGE OF THE FEEDER AND CRIB BLOCK.
EE WAS GETTING H IS BELT TOOLS OUT OF A LARGE TOOLBOX WHEN THE LID FELL ON HIS RIGHT HAND BREAKING HIS FINGER.
