ON FEBRUARY 3, 1997 EMPLOYEE WAS EXPERIENCING PAIN IN HIS MID SECTION NEXT TO HIS BELLY BUTTON. EMPLOYEE STATED THAT HE HAD NOT SUFFERED AN INJURY PRIOR TO THE PAIN AND THAT HE COULD NOT RELAT E IT TO ANY SPECIFIC OR INSTANTANEOUS SINGLE INCIDENT. HE ALSO STATED THA THE DID NOT FEEL ANY STRAIN THAT MIGHT HAVE CAUSED THE PAIN.

EMPLOYEE WAS ATTEMPTING TO SECURE CYLINDER ON FORKS. WHEN EMPLOYEE PUSHED CYLINDER TO LINE UP WITH FORKS, THE CHAIN SECURING CYLINDER ON SECOND FORKLIFT SLIPPED OFF FORKS AND CYLINDER FELL AGA INST EMPLOYEE. THE OTHER EMPLOYEE WORKING WITH INJURED PUSHED CYLINDER OFF CAUSING IT TO FALL AGAINST EMPLOYEE'S HAND AND FRONT PART OF FORKLIFT.
