Employee stated that he was stacking tires when as he was rolling a tire, it rolled on a bearing and began to fall. As he tried to catch it, grabbing it with his left arm, he felt something pop in his arm. Employee began missing work on 01/12/2011.

EE was handling a clevis for 2" steel cable when the clevis dropped catching the tip of his ring finger on the right hand between the clevis and a rock resulting in a fracture to the tip of the finger.

EE stated that he raised the hood to check the fluids, when hsi shoulder popped, resulting in a "fractured" collar bone. This hood weighed approx 25's maximum.

Employee stated that he was picking up a crib block when he hurt his back. Stated that it was an old injury from a non-occupational motor vehicle accident.

Injured was replacing a leaf spring on a school bus. While positioning the spring on the front axle, the spring slipped off the axle and caught his finger betwen the spring and a crib block lacerating the little finger which required stitches and medical treatment.

Employee stated that while attempting to open tailgate, he was using the bullpin out of the bumper to try to pry the tailgate open. The pin slipped and mashed his thumb between the pin and bed.

Employee was loading a tank of oxygen into his service truck and felt a pain in his lower back. The employee continued to work until 12/25/04 at which time he obtained medical attention and started missing days worked, at this time the incident became reportable.

EMPLOYEE HAD ONLY ONE DAY OF RESTRICTED DUTY ONLY.

THE EE SECOND EFFORT TO MOVE THE FLOC CREATED A SHARP PAIN IN HIS LOWER BACK. THE EE CONTINUED TO WORKHIS REGULAR JOB AT FULL CAPACITY UNTIL 10-2-00, AT THIS TIME THE OCCURRENCE BECAME REPORTABLE DUE TO WORK DAYS MISSED.
