The Company was notified that EE qualified for an occupational pneumoconiosis claim.

Struck against a moving object

EE steam cleaned equip. until 3:30 pm fueled equip. and was returning to maint. area, EE stated he started down grade when truck jumped out of gear he applied brakes no brakes. The truck continued down the hill struck the berm and overturned coming to a stop on its top. EE was taken to hospital with injuries to right arm and several bruises. Released 06-21-10

Accident type, without injuries

Stockpile of coal located in maintenance yard for a long period of time eventually became hot on the southeast side of the pile. The area, although small in comparison to the over all size of the pile, will be isolated by removing non-hot material, then removing the hot coal.

EE was driving up to mine site doing pre-shift, when fire occurred under frame. Truck was engulfed and a total loss. Unit just recently had had major maintenance. Truck had 190,000 miles on it.

PW-15, a point augered several years ago developed a fire in the #2 gas seam. This fire from time to time becomes noticable with smoke etc. Due to the need to travel by truck close to the seam it is important to eliminate by smothering with overburden.

Injured was replacing a leaf spring on a school bus. While positioning the spring on the front axle, the spring slipped off the axle and caught his finger betwen the spring and a crib block lacerating the little finger which required stitches and medical treatment.

DRIVING AN EMPTY FUEL TRUCK, TRUCK POWERED OUT ON HILL. HE TRIED TO RESTART THE TRUCK AND IT STARTED GOING BACKWARDS DOWN HILL. TRUCK HIT BANK AND ROLLED OVER. CUT FOREHEAD AND FRACTURED SHOULDER BLADES.

INJURED AND COWORKER WERE CHANGING A LATCH ARM ON A SUPERIOR HIGHWALL MINER BEAM, INJURED WAS HOLDING NEW LATCH IN PLACE WHILE COWORKER ATTEMPTED TO INSERT THE HOLDING PIN. THE PIN WAS NOT PROPERLY ALIGNED AND WHEN THE PIN WAS REMOVED THE LATCH FELL CATCHING INJURED'S FINGER BETWEEN THE LATCH AND THE BEAM CAUSING INJURY.

INJURED WAS USING 6 POUND HAMMER TO POUND A DRIVE SHAFT INTO POSITION ON A MACK WATERTRUCK. THE END OF THE DRIVE SHAFT DROPPED AND WHEN INJURED SWUNG HE CAUGHT HIS THUMB BETWEEN THE END OF THE DRIVE SHAFT AND THE HANDLE OF THE HAMMER CAUSING A INJURY TO HIS RIGHT THUMB, EMPLOYEE IS A CONTRACTOR WORKING FOR AJM CORP.

INJURED WAS HOLDING A TORQUE MULTIPLIER UNDER A DOZER BLADE ON A NUT, WHILE THE MECHANIC APPLIED THE RATCHET ON THE INPUT SIDE. THE HANDLE OF THE MULTIPLIER WAS BRACED AGAINST A STEEL RIB UNDE R THE BLADE. THE HANDLE SLIPPED PFF THE RIB CATCHING THE INJURED'S THUMB BETWEEN THE HANDLE AND THE DOZER BLADE.

EE WAS DRIVING A CATERPILLAR 777 ROCK TRUCK & SHE CLAIMS SHE WAS BEING JOLTED WHEN THE EXCAVATOR WAS LOADING HER. SHE STARTED TO GET PAIN IN HER LOWER BACK & DID NOT FINISH THE SHIFT.

EE WAS CLIMBING DOWN LADDER OUT OF 988G LOADER WHEN HE STEPPED ONTO SOME UNEVEN GROUND & HIS LEFT KNEE TURNED OUT. HE FELT PAIN IN LT KNEE. HE FINISHED THE SHIFT & WORKED THE FOLLOWING MONDAY. WENT TO EMERGENCY ROOM ON TUESDAY 3/12/02.

EXCAVATOR WAS LOADING A LARGE ROCK INTO INJURED'S TRUCK, FROM THE SIDE. SINCE EXCAVATOR WAS LOADING FROM THE SIDE, HE COULD NOT PLACE THE ROCK INTO THE BED, HE HAD TO DROP IT. THE WEIGHT OF TH E ROCK CAUSED TRUCK TO ROCK. INJURED ATTEMPTED TO HOLD ON AND HE FELT PAIN IN HIS BACK.

INJURED AND CO WORKER WERE WELDING TWO 30' JOINTS OF 6"" STEEL PIPE TOGETHER. AFTER WELDING TOP SIDE OF PIPE, BOTH MEN USED SLATE BARS TO ROLL PIPE OVER. WHILE ATTEMPTING TO LIFT THE SLATE BAR, INJURED FELT PAIN IN LOWER BACK. INJURED FINISHTHE SHIFT AND REPORTED FOR WORK ON MONDAY 2-25-02 BUT LEFT AT 100 PM WAS TREATED AND RELEASED FROM HOSPITAL ER.

INJURED AND 3 OTHERS WERE LOADING BAGS OF ANFO EXPLOSIVE. EACH MAN CARRIED TWO 25LB. BAGS FROM INSIDE OF BOX TRAILER AND DROPPED INTO BED OF A P.U. TRUCK. INJURED THE STEPPED DOWN ONTO THE BUMPER OF THE TRUCK AND THEN DOWN TO THE GROUND.INJURED FELT PAIN TO LOWER BACK AND HAD NUMBNESSIN RT. LEG. X-RAYS AND MRI INDICATED A BULGING DISC.
