The miner put hammer handle through the strap, the miner had hand on the hammer head when the cable became tight the miners hand was caught between the hammer and the car deck. While removing transmission from D9L fastened transmission to the boom of the mechanic truck. After attempting to loosen the transmission with the boom, employee stepped forward to see why transmission would not come loose. The transmission "popped off" striking the employee in the chest and knocking him against the side of the mechanic truck, resulting in six fractured ribs.

Employee removed the bottom steel from the bolter pot to check the suction. As he placed his left hand over the pot, the remaining steels fell from the bolthole he was drilling and struck his left hand, fracturing his middle finger.

The employee had wrapped a thin cable around the brake handle on a piece of equipment and was going to pull the cable but, while he was trying to hang onto the cable it caught on something and pulled tight and caught his left hand and broke his middle finger. The nature and location of the injury caused the employee's finger to be amputated by the doctor.
Employee had been under a doctors care on his right shoulder since May 2007 for repetitive shoveling. He re-aggrevated the shoulder on 12/4/2007 while shoveling on a belt tail roller. He went off work for shoulder surgery on 2/13/2008.
