Employee allegedly strained a muscle in his back while hanging scoop charger cable. * Employee did not begin to miss work until 03-31-11.

Shuttle car cable pulled tight and at an angle. Employee tried to pick it up and hang it and a pain shot through left shoulder.

Employee was shoveling belt line when he pulled a muscle in his right shoulder.

Employee was taking mantrip off charge when it started to roll and the power cord was pulled out of power center. Employee tried to stop it by grabbing the cord with his hand, jerking his arm and shoulder.

Employee was carrying beams & jacks. The beam slipped from his hand causing an abrasion on his right wrist & a laceration on right upper leg.

The employee was moving timbers.

Employee was setting timbers. Timber fell and hit employee under his right eye and across the bridge of his nose lacerating his jaw & nose.

Employee was working on #3A Head. When he moved a guard it fell striking his left hand middle finger.

Employee was setting breaker post timbers. When he went to split a half header it slipped causing it to kick back & lacerate his left middle finger, ripping nail off.

EE cut his left arm on a metal roof bolt binding strap while he was unloading roof bolts from a scoop bucket and placing them on the bolting machine.

Employee was reeling cable up on MRS when a cable broke loose from breakaway hangers striking employee in the neck.

Employee was picking up a timber out of the scoop bucket, when the timber hit him in the left knee.

Employee was taking rub rail off miner. The rub rail fell on right hand.

Employee was pulling a roof bolt off the bolter, when he jerked the bolt it was hung up causing his left shoulder to be pulled.

Employee was under belt line and feeder when employee felt hamstring pull in left knee. Didn't begin missing work until 1/12/09.

The employee was helping to hang a miner cable when he felt a burning sensation in his shoulder.

Employee was climbing up ladder to start stacker belt when he hurt his leg.

Duck bill slipped off belt, striking left hand. Employee did not report until 6/12/08.

The employee was helping bolter operator back out of #3 face. The operator turned the reel on, when the cable pulled down, it struck employee on the right knee.

EMPLOYEE WAS LIFTING A BLOCK & FELT SOMETHING PULL IN HIS ARM. MISSED WORK 9/10 & 9/11 RTW 9/12 THEN BEGAN MISSING 9/13/07 TO CURRENT. QUESTIONABLE INJURY.

EMPLOYEE WAS WORKING ON A WHEEL UNIT. WHEN HE LIFTED THE WHEEL UNIT HE FELT HIS HAND POP.

EMPLOYEE WAS PUTTING BELT CLAMP ON BELT. WHEN HE LIFTED THE CLAMP, HE FELT SOMETHING PULL IN LOWER RIGHT SIDE OF BACK.

Helping tighten a binder down, he bent over and pulled his back. Went home about 8:00 pm, returned to work next day, worked 2 days and then went to doctor on 6/10/06.

Employee was putting a torkshaft in on miner when he mashed his hand between metal.

Employee was carrying buckets filled with rock dust. He bent over to pick up buckets and strained his lower back.

Belt line broke - Employee was picking up structure to level it when he pulled something in the left side of his back.

Employee was lifting a box and turned when he felt a pain in his back.

Employee was squatting on knees while working on panel box, when rub rail fell striking his left leg.

EE WAS TAKING 4" WATER LINE DOWN WHEN HIS HAND GOT CAUGHT BETWEEN WATER LINE AND BELT STRUCTURE.
