EE had been shoveling on LF 1 belt as he twisted to put material on the belt with a shovel he felt pain in lower back and his left leg went numb. EE had back surgery in the past and his symptoms were the same as before the back surgery. I ask this be classified as an occupational illness because of his past history with his back.
Employee was removing a 60" top roller 2 bks out by #3 tailpiece. He had raised the belt with a chain ratchet and was pulling the top structure out when his back started hurting. He finished his shift then called in the following day stating he was going to the doctor who prescribed him physical therapy for 2 weeks because of a strain.
Member cut band on bundle of bolts and plate's, band flew back and struck his right arm. This required stitches to close his wound.
EE was cleaning on tailpiece on his knees out of sight of foreman. EE told foreman to throw him a crib block when he threw the crib block it struck ee in the lower back. EE wasn't scheduled to return back to rockhouse, accident occured on 7/31/09 he was to return back to solid his next scheduled shift. He suffered broken vertebra tips and hasn't returned to work.
Employee was picking up a timber in the #4 heading when he felt pain in lower back. He was given off work until 4-26-09 because of a back strain.

EE WAS MOVING A WHEEL FOR A 15 TON MOTOR, ATTACHED TO THE HOIST. THIS IS A MANUAL HOIST THAT WILL ONLY LIFT OBJECTS. IT HAS TO BE MOVED BY PULLING ON A STEEL CABLE THAT IS ATTACHED TO THE HOIST. EE WILL HAVE TO HAVE THE HERNIA REPAIRED BY SURGERY.
WHILE MOVING APPROX 20 PIZZA PANS, HE CUT HIS LITTLE FINGER ON LEFT HAND, RECIEVING STITCHES TO LITTLE FINGER.
EE was removing a piece of drawrock from the top of the sheer when his left hand slipped allowing it to go into the drawrock causing a small laceration to palm of hand.
EE WAS WORKING ON FIRE SUPPRESSION ON SHEAR. HE WAS CLEANING SHEAR OFF WITH A WASH DOWN HOSE WHEN HE SLIPPED ON A ROCK CAUSING HIM TO FALL ALLOWING WASH DOWN HOSE TO GET LOOSE FROM HIM, STRIKING HIM IN THE RIGHT HAND. REQUIRED SUTURES.
WHILE UNKOOKING A ROPE HANGER THE MINER CABLE FELL CAUSING HIS LEFT ARM TO BE PULLED DOWN, HURTING EE SHOULDER. EE HAD CONTINUED TO WORK UNTIL 11/01/2004 AT WHICH TIME EE WAS UNABLE TO WORK.
EE was removing a rock out of the britby tray. other was assisting when other raised the rock from his side if flipped over. EE's hand was in the other it pinched his ring finger on his left hand. he had a fllow up with a specialist to correct his finger or 6/23/04. he returned to work on 6/24/04.
HOSE THAT IS USED TO LET SHIELDS DOWN WITH CAME UP AND HIT EE UNDER CHIN CAUSING A SMALL LACERATION TO CHIN.

Employee was building a brattice, he was removing a block off of a pallet when another block fell over striking him on his right thumb, resulting in a fractured thumb.
MEMBER WAS RAISING A LID ON THE SHEAR TO CHECK THE TRANSHAFT WHEN HIS HAND SLIPPED OFF THE LID CUTTING IT ON A CARTER KEE THAT WAS HOLDING A PIN. MEMBER REQUIRED STITCHES IN RIGHT HAND.
EE WAS ROOF BOLTING WHEN DRILL STEEL BECAME STUCK AND TRIED TO FREE IT, HE HIT HIMSELF IN HEAD, CAUSING LACERATIONS.
EE WAS MOVING ROOF BOLTS ON TOP OF THE ROOF BOLTER. AS HE WAS HE STUCK A METAL BAND IN HIS WRIST CUTTING IT. HE WENT TO HOSPITAL WHERE HE RECEIVED STITCHES TO HIS LEFT WRIST.
EE STATED HE WAS INJURED AT QUACKER COAL IN THE PAST. HIS BACK HAS CONT'D TO HURT SINCE. EE STARTED MISSING WORK ON 3/27/03 SOUGHT MEDICAL ATTENTION FOR HIS BACK. HE TOLD 3 SUPERVISORS THAT HI S BACK PROBLEMS BEGAN FROM AN INJURY THAT HAPPENED AT QUAKER COAL COMPANY
EE WAS MOVING HIS SHUTTLE CAR ANCHOR & REHANGING IT WHEN HE FELT PAIN IN HIS LOWER BACK.
BREAKING BAND FROM APLLET OF CRIB BLOCKS. THE METAL BAND BROKE SLICING THRU HIS GLOVE AND CUTTING FINGER ACROSS KNUCKLE REQUIRING 4 STITICHES. MEMBER DID NOT NOT TEASE WORK.
EE WAS TAKING DOWN THE MINER CABLE WHEN HE UNHOOKED THE ROPE HANGER THE FORCE OF MINER CABLE CAUSED THE END OF THE ROPE HANGER TO CATCH HIS LEFT HAND BETWEEN THE THUMB AND FIRST FINGER CAUSING LACERATION & REQUIRING STITCHES.
EE WAS MOVING A PIECE OF DRAW ROCK FROM THE TOP OF THE PANLINE ON THE LONGWALL WHEN THE ROCK HEADED OVER INTO THE LINE. THE BACK END OF THE ROCK CAME UP CATCHING HIS LEFT THUMB AGAINST THE SHI ELDS CAUSING A LACERATION AND BROKEN THUMB REQUIRING STITCHES.

EE WAS HELPING TO MOVE A MINER CABLE WHEN HE FELT PAIN RUNNING DOWN HIS LEG. HE WAS TRANSPORTED BY AMBULANCE TO HOSPITAL.
TA LING DOWN MONRAIL WHEN HE UNHOOKED IT A PIECE CAME DOWN CATCHING. EE RIGHT HAND INDEX FINGER. CAUSING A LACERATION AND POSSIBLE BROKE BONE. HE WAS WEARING HIS META-PRO. GLOVES.
EE WAS INSTALLING TIMBERS IN THE HEADGATE ENTRY. WHILE INSTALLING A TIMBER A PIECE OF DRAW ROCK MEASURING 18" WIDE BY 21" LONG AND 2"THICK FELL FROM THE SIDE OF THE BOLT STRIKING EE ON THE FOR EHEAD. EE SUSTAINED TWO LACERATIONS REQUIRING STITCHES.
