Employee was building cribs; had right hand on scoop bucket and crib rolled off on his right finger. Finger is broken. Splint on right pinkie and ring finger.
Employee states, injury and other ee were loading the roof bolter, when he cut the bands on the bolts the band srung back striking him on the left hand. He was wearing gloves.
Trying to pry belt clamp loose from top structure, structure belt broke. Belt and C clamp struck employee in the face, near the right eye.

6/25/08 EE called to say he was unable to work. He said on 6/23 he was loading the TC sprayer (plaster machine) & liquid shot into face & eyes. I asked about safety glasses. He said he broke them, they were on his head but not on his eyes. His eyes were washed at work. He returned next day though his eyes were a little swollen. Today they're swollen worse & he is going to dr.
Employee was hanging cable inby feeder and his right hand slid across a pizza pan and sliced his fourth knuckle on right hand, causing laceration. Action to prevent recurrence is to watch the surroundings.
While laoding rock dust into scoop bucket; tied chain to metal straps turned around and the straps broke hitting EE on right side of head; making a one and a half inch cut on right side of head. Action taken in preventing reocurrence is do not hang rock dust from straps.
Employee was rock dusting by hand and packing rock dust; pulled something in rt. side rib area.
Employee was scattering belt structure for a belt move, threw a piece of structure under the belt. His hand got caught between roller and frame pulling something in right shoulder. Complained of pain going down arm. ***Employee did not start missing work until 07-13-2007.
He was lifting oxygen bottle; felt pain in lower left stomach. *****Employee did not miss work until surgery was scheduled 05-16-2007******
Was taking bearing off shaft of feeder crawler frame puller bolt broke housing of bearing and flew off and cut right index finger
Hurt both shoulders on 04-13-07 pulling miner cable. Reinjured on 04-18-2007 when stepping off man bus. Did not report accident until 04-18-2007, left work and went to Dr.
The coal hauler had a sharp piece of metal; employee brushed his right hand against it. Went to hospital rec. 5 stitches
While taking apart style jack, I had it standing in the up right position. When I turned around to get a hammer, the style jack fell striking me in the right lower leg.
EE was getting structure out of structure car a piece of structure fell into his light cord and jerked his head
Moving roof bolter out of the way when his left wrist came in contact with roof plate causing plate to cut his left wrist

A PIECE OF ROCK FELL, HITTING EE IN THE HEAD AND BACK - KNOCKED EE TO THE GROUND. EE SUFFERED STRAIN WHEN REMOVING ROCK FROM SELF. EE STARTED MISSING WORK 04/01/05 (DATE HERNIA SURGICALLY REPAIRED).
EMPLOYEE WAS LAYING ON THE GROUND, MINER MAN WAS RAISING RIPPER HEAD; CONVEYOR TORQUE SHAFT FELL OFF MINER HITTING HIM ON THE RIGHT KNEE.
Loading scaffolding on scoop, caught finger between scaffolding and fender of scoop.
Employee was lifting a piece of rail & felt something pull in lower back.
EMPLOYEE WAS HANGING CURTAIN IN #1 ENTRY AND FELT PAIN IN LOWER BACK AND LEFT HIP.
EMPLOYEE WAS GETTING INTO BUS TO SIT IN A SEAT THAT HAD BEEN PUT IN FOR COMFORT. THE SEAT WAS LOOSE AND WHEN THE EMPLOYEE GOT IN THE SEAT IT ROCKED BACK, CAUSING THE EMPLOYEE TO THROW HIS HAND BACK TO CATCH HIMSELF AT THE SAME TIME ANOTHER EMPLOYEE WAS CLOSING THE LID ON HIS TOOL BOX & CUT FIRST EMPLOYEE'S LEFT RING & BABY FINGER.
EE WAS PUTTING JACK ON SCOOP AND STRAINED HIS BACK. EE DID NOT START MISSING WORK UNTIL 11/11/04.
LIFTING OMEGA BLOCK & FELT SOMETHING TEAR IN HIS RIGHT SHOULDER.
EMPLOYEE TOOK SHIELD LOOSE ON MANTRIP TO PUSH CONTACTOR IN TO SET OUT OF SWITCH; SHIELD HIT RIB AND CAME BACK AND HIT RIGHT HAND. HE RECEIVED 5 STITCHES IN RIGHT PALM.
LIFTING MINER CABLE, MINER MOVED FELT PAIN IN BACK AND SCROTUM.
EMPLOYEE WAS LOADING BOLTS ONTO BOLTER; HAD A PIE PAN SECURED TO PREVENT BOLTS FROM FALLING OFF. HE BRUSHED HIS RIGHT WRIST ON THE PIZZA PAN; HE DIDN'T THINK IT WAS BAD UNTIL HE CLEANED IT UP LATER AT HOME THAT EVENING, REALIZING HE MIGHT NEED STITCHES. WENT TO THE HOSPITAL; RECEIVE FIVE STITCHES.
EMPLOYEE WAS HELPING UNLOAD RAILS FOR THE NEXT INSTALLATION PROJECT. HE WORKED UNTIL 3/31/03 AT WHICH TIME THE PAIN BECAME SO BAD HE STARTED MISSING WORK. THE ACCIDENT WAS CAUSED BY IMPROPER L IFTING AND TWISTING.
EE WAS CHECKING COAL HAULER BATTERIES A ROCK KICKED OUT CAUSING HIM TO HYPE EXTEND HIS KNEE. ALSO FALLING & TWISTED KNEE EE STARTED MISSING WORK 3-20-03
MINER WAS SET UP IN #2, EMPLOYEE WAS THROWING MINER CABLE OUT OF WAY IN XCUT WHEN PICKED UP ON TIGHT MINER CABLE, THE MINER MOVED CAUSING THE CABLE TO PUT STRESS ON EMPLOYEE AND HIS KNEE GAVE OUT.
EMPLOYEE WAS DOWN UNDER ROCK BOX 8 TAILPIECE CHANGING EOLLERS WHEN HE PULLED MUSCLE IN HIS RIGHT/ MIDDLE LOWER BACK.
EE TELEPHONED MINE FOREMAN 12/16/02 AND SAID THAT HE HAD HURT HIS BACK ON 12/12/02 WHILE PULLING MINER CABLE. EE DID NOT NOTIFY ANYONE ABOUT THIS INJURY UNTIL 12/16/02.
EE DID NOT REPORT ACCIDENT UNTIL 10/3/02 (ON THE JOB ACCIDENT) THE EE FINISHED HIS SHIFT ON 9-23-02, SAID HE GOT UP THE NEXT DAY AND WAS SORE. WENT TOT HE DR AND WAS TOLD HE HAD A STRAINED MUSCLE. THIS WAS A WORK RELATED ACCIDENT.
EE WAS THROWING CRIB BLOCKS IN SCOOP WHEN HE FELT SOMETHING POPE IN HIS LOWER BACK.
EMPLOYEE WAS SHOVELING ROCK DUST INTO DUSTER AND TWISTED HIS BACK.
EE WAS SLIDING STRAPS ON ROOF BOLTER, A JAGGED LEDGE WAS STICKING OUT FROM THE STRAP; THE EE DIDN'T SEE IT WHILE IT PENETRATED LT. INDEX GLOVE & FINGER.
EEWAS UNLOADING PLATES FROM SCOOP, THE BANDS WERE CUT AWAY FROM THE PALLET. A PIECE OF METAL WAS LACED IN THE PALLET, THE EE DID NOT KNOW THIS, AND WHEN HE PULLED THE PLATES FROM THE PALLET, A PC. OF METAL CUT THROUGH HIS GLOVE INTO THE PALM OF HIS LEFT HAND.
EE WAS TRAMMING MOTOR AND ROCK DUSTER TO DRIF TODUST BELTS. HE THOUGHT THAT HE NEEDED TO OPEN BOTH SETS OF DOORS TO LET THE DUST OUT QUICKER. HE HAD ONE SET OPEN, ON THE SECOND SET HE HAD ONE DOOR OPEN AND GRABBED THE OTHER ONE WHEN THEDOOR SWUNG CLOSED HITTING HIM ON THE LEFT ARM & LEG, CUTTING THE BACK OF HIS LEFT ELBOW.
EE WAS TRYING TO LIFT A ROCK OFF OF THE MINER. A KNOT CAME UP IN HIS LOWER RT. ABDOMEN.
EE WAS BUILDING BRATTICE & CRIBS ON SECTION. LIFTED 50# CRIB & KNOT POPPED UP IN STOMACH. ON 10//19/01 WENT TO DR. AND WAS TOLD HE HAD A HERNIA & WOULD SCHEDULE SURGERY LATER. SURGERY WAS SCHE DULED FOR 3/1/02. EE CONTINUED TO WORK UNTIL 2/28/02. FIRST DAY OF WORK MISSED WAS 2/28/02.
EMPLOYEE WAS HELPING PULL THE BELT IN PLACE BY MEANS OF A PULLEY WHEEL AND A STEEL ROPE. THE ROPE BROKE AND FLEW ACROSS THE ENTRY STRIKING EMPLOYEE ACROSS THE FACE.
EE WAS UNLOADING OXYGEN TANK WHEN ONE HAND SLIPPED CAUSING TANK TO FALL ONT HE OTHER HAND SMASHING FINGER.
EE WAS RECOVERING BELT HEAD CABLE TWISTE RT KNEE
PACKING MINER BITS-INJURED BACK.
EE WAS PUTTING 4" WATERLINE ACROSS BELT LAYED WATERLINE ON TOP OF RAILING AND WENT TO PUSH IT ACROSS, PULLED SOMETHING LOOSE IN LEFT SHOULDER.
