Employee was helping to change ram jack on scoop and had set it in place with a lo-trac on 2 crib blocks. A repairman was putting hoses on it when he decided to move it to give more room to hook the hoses and it tilted off the crib block mashing his right hand resulting in a fracture to his ring finger.

Employee was taken to a doctor on 2/25/2015. As a result of repetitious lifting of his job duties, a doctor has diagnosed him with osteolysis. As a result of the occupational illness he started working restricted duty as of 3/3/2015. **It has been determined that the EE had a detached bicep tendon. Changed from an occupation illness to an injury.

While installing 20' long panels on an overcast 4-breaks outby #9 drive, all five crew members were picking up panels and sliding them over the overcast walls. Two contractors were pushing a panel into place and employee could not get out of the way. The panel caught his left hand and middle finger. This resulted in ee left middle finger being amputated just below the knuckle.

EE had installed a bottom roller on offside of the belt. While crossing back over the belt he placed his left hand on the structure to step off the belt. EE felt a sharp pain in his elbow when stepping off of the belt.

The injured and another miner were setting roof jacks along the haulway at SS #888. When the other worker took a jack from the jacks piled along the rib, another jack fell on the left foot of the injured.

EE was in the process of installing a roof bolt. He drilled a hole, inserted the resin and bolt into the hole. Due to mining height, the bolt had been bent prior to inserting it in the hole. He inserted the top part of the bolt in the hole and as he pushed on the bolt to straighten it out, He strained his back.

THE INJURED WAS HAULING SUPPLIES WITH A BATTERY POWERED SCOOP. HE STOPPED AND GET OUT OF THE SCOOP TO OPEN A SET OF DOORS. WHEN HE RETURNED TO THE SCOOP DECK IT APPEARS THAT HE ACCIDENTLY STRU CK THE STEERING CONTROLS WHICH CAUSEN THE MACHINE TO PARTICULAR THE INJURED'S LEGS WERE CAUGHT BETWEEN THE MACHINE AND THE DOOR.
