The final shuttle car was being loaded from the face of the place, when a piece of roof bolted rock measuring about 71"" long X 39"" X 18"" thick fell, striking the victim. The injuries received from the occurance were fatal.

Employee was moving rocks out of roadway and strained left rotator cuff.
Employee was operating a scoop and said hit a hole and jarred EE, causing pain in EE's lower back. Didn't tell the foreman until the end of shift. Worked the shift without saying anything to the foreman and never returned to work.
Employee was bolting top and went to remove drill steel from the hole and a piece of draw rock hit EE's left hand.
While changing out a hydraulic pump on the feeder, EE was pulling the pump out when it slipped and landed on EE's right middle finger requiring 4 stitches.

A NON injury roof fall occurred @ Cross Cut 17 in the #2 entry (Belt Line) on the Mudlick Mains. After investigation,it was determined that the cause of the fall was due to age deterioration of the area.
While bolting in the No#5 heading, the drill operator was swinging in to put up inside bolt when a piece of rock fell off the canopy striking EE's left hand. The rock was round in diameter 2"" thick x 10"".

This was a roof bolter when EE let EE's canopy down the pin broke letting the canopy fall down striking EE in the shoulder and broke EE's finger.
A Non-Injury accident occurred on the MMU. No. 003-0 Section during second or retreat mining. The accident occurred in the # 3 entry, 50'ft. inby Survey Station 4274 on the 1 Rt. Panel which caused the # 1 MRS to be caught by the mine roof which had fallen above anchorage of the primary roof support. MSHA was immediately notified about this accident or event that occurred.
A Non-Injury Roof Fall occurred in the Pillar Line on the MMU. 003-0 Section on the 1 Rt. Panel off the 1 North Submains. A section of the mine roof approximately 20' long x 4' thick set down on the No. 1 Mobile Roof Support while the continuous miner was cutting the No. 3 Pillar Block on the run No. 3 between X-cut # 20 and X-cut # 21. ( MSHA was immediately notified.)
A Non-Injury Roof Fall was found in an outby area of the mine during an exam. MSHA was notified about the Roof Fall. The Roof Fall is approximately 30'feet long x 20' feet wide x 12' feet thick. The Roof Fall is located in the No. 6 entry at c-cut # 66. The roof Fall area has been blocked off to prevent entry to this area.
Employee turning a crosscut in No. 3 entry. When EE bent over to move the continuous miner cable, EE felt a sharp pain in EE's lower back.
Employee was squatting down and hanging a shuttle car cable when EE felt pain in EE's right hip and leg. The employee stated that EE's hip locked, and EE could not move.
The injured employee and co-workers had installed a new pump in a outby area of mine. The employees were hanging the 3"" discharge line to the mine roof when the discharge line came off the pump and sprayed water and mud into the face and eyes of the employee after the safety glasses were knocked off. This resulted in a small tear to the retina of the left eye.

Employee was changing a broken training roller when they struck their right index finger with a hammer resulting in two stitches.
On the evening shift of 08/17/2018, a roof fall above anchorage occurred at the outby end of the Mobile Roof Support, the fall was in the #6 entry, 80' inby spad #3640. It measured 6' thick- 20' wide -23' long.
Employee was operating a diesel mantrip, Damascus Mac 3D, in secondary travel-way when the ride hit a piece of rock in travel-way bouncing employee's head upward into the roof.
Foreman was conducting weekly exam in the Mud Lick Mains. The roof fall was discovered in the right return #6 entry 2 breaks inby #5 head drive at spad 260. The fall was 20' wide, 20' long and 6' high. The fall was dangered off with timbers and danger tape and called in to the hot line local MSHA and State officials.
An unintentional roof fall occurred on the # 18 belt drive inby the 002-0 section. The fall was from 0 to 12' high, 22' wide at the widest point and approx. 20' long. The roof fall is measured from the roof ""not the floor"" to the apex of the fall.
Employee was cutting a skirt board and cut downward slipped and cut forearm.
Employee was adjusting curtain while roof bolter was tramming into entry. The drill hit gob/rock & shifted striking employee in chest with the T Bar. Employee has broken ribs & collapsed lung.
EE was ready to pin curtain together #2 entry & a rib rolled out striking EE & knocked EE to the ground. EE was taken to ER, Xrays negative. EE is following up with an MD. EE had bruising on head, back, hip & knee.
EE was helping change a speed reducer on No.8 Drive using a bar to remove the chain from sprocket. The bar slipped & caught EE's finger in chain causing a laceration to right ring finger. EE received stitches.
An unintentional, non-injury roof fall occurred on the 002 Section #5, one break in by spad 3862 that measured 25' wide x 25' long & 10' high. Area was timbered off, bolts have been installed, jacks & beams set & 16' roof bolts installed in adjacent entries.
EE was bolting top in #1 entry, a piece of rock broke & fell out of the top when EE was pushing up a bolt. EE's back & leg were injured.
While installing new miner conveyor, beating links together, sledge hammer came apart & metal pierced EE's leg on shin near calf.

Employee was bent over walking up #3 entry, low area. EE raised up & struck neck on a roof bolt. EE received 2 sutures & released for full duty. EE returned to regular job on the next scheduled shift.
EE was tightening a nut on a D3 EE was operating with pliers, both hands using full grip. EE said EE's chest popped & EE let go & drove ride outside & reported to another EE. EE did not see a M.D. until 7 days later, 11/27/17. EE missed 1 day of work, 11/28/17 and returned to EE's normal job 11/29/17.
Employee was unloading belt structure. While turning with a top belt cradle, EE stepped on a rock which caused EE to twist lower back. EE missed one day of work and returned to work on 09/19/2017.
An unintentional roof fall occurred in the #4 entry @ crosscut #360 on the MMU 001/002 section at spad #3684. The fall was approximately 9' high & 45' x 30' in the intersection. The area was supported with 5' resin bolts and 10' cable bolts. The material that fell was shale and absent of any sandstone.
Employee was holding pry bar with left hand and was hitting it with 8 lb hammer when they missed the bar & struck their left hand. EE finished the shift but went to physician on 07/16/2017, next day. EE was diagnosed with a bruised radial nerve. EE was taken off work for 3 days and returned to work on 07/20/2017.
EE was hauling supplies in diesel scoop when hit a hole in bottom bouncing them into the top with their head, EE was wearing a hard hat. EE felt pain in neck/back region. EE went to doctor & diagnosed w/muscle sprain. EE continued to have pain, followed up with dr. and has loss time starting on 07/20/2017.
Employee picked up miner cable & water line to move it out of the way. When they turned & pitched the cable they strained their lower back causing muscle spasm. Employee went to local ER and has strained the muscle in their back causing spasms. Employee is off work and has lost time.
Employee setting tailpiece, while turning turnbuckle w/hammer the hammer broke loose striking EE in the upper lip. This resulted in laceration requiring 2 stitches to the lip. EE went to ER @ Lonesome Pine Hospital in Big Stone Gap, VA. EE had no loss time and has full release to RTW.
While loading belt take up jack onto forklift, load shifted onto employee. Employee caught the left ring finger between jack & fork resulting in crushing injury to the tip of the left ring finger, requiring 2 sutures from local ER.
EE was installing roof bolts, EE swung drill head out,a hose caught one of the bolts, EE reached across drill head when piece of rock (2'x2'x2"")fell striking the right forearm against the boom, rock slid down & struck the hand. EE was taken to ER,received 3 sutures in forearm,3 in the hand. EE fractured the long bone in the right hand just before the knuckle of the pointer finger.

The section foreman was walking the section when a piece of draw rock fell between two bolts and hit his right forearm causing a laceration.
Employee was installing roof bolt when a coal rib (2' long, 18"" wide, and 2"" thick) rolled off and hit his left ankle. Coal fell in loose pieces and accident occurred in #6 entry 150' inby SS# 2055
While making a splice in the #1 shuttle car cable, struck his finger with hammer when he was hitting the anvil.
The EE was operating a C.M. in the face of the #2 heading, he was in the process of setting the miner over when a piece of draw rock fell from between the installed roof bolts and struck him on his left foot and resulted in a laceration which required stitches.
He was putting up a roof bolt the end of the bolt broke. He drilled another hole beside the bolt that had broke the drill steel drilled into the broke bolt, when he let the head down the drill steel hit him on the left side of his neck.

Employee was on work deck. Holding concrete trunk hose. Received jolts from the hose as concrete was being poured and shut off. Caused pulled muscle in back.
Working on deck, making concrete pour. Employee was pushing work deck off form, strained knee.
While servicing the miner, the individual was carrying a 5 gallon bucket of gear oil. When he stepped in a water hole, he slid on a slick rock and his left knee buckled.
The belt move crew were preparing to move the conveyor belt. As the hydraulic jacks which secured the belt tailpiece were being released one of the jacks suddenly dislodged from the mine roof and fell. The jack struck the EE on the right arm as it fell.
The EE was hanging the miner cable by himself when he felt pain in his right testicle. He did not go to the doctor at that time. EE went to the Dr. on 9/22/10 and was diagnosed with a hernia. Surgery to repair the hernia was performed on 10/27/10.
While putting cutter back on miner, the jack slipped and caught the employee's right index finger between the jack and the CLA.
The individual was walking beside the low-trac when he fell and struck the low-trac
The injured EE reported that he hurt his back working on shuttle car. EE went to the E.R. on 1/24/10 and was admitted suffering from pneumonia. EE has not returned to work and has been on sick leave. An investigation is being conducted to determine when and if EE has missed work as a result of the injury.
While 2nd mining in the end cut #5 block while taking 1st cut a piece of sandstone 2.5' X 12' wide X 15' fell across the cutter head.

A non injury. High angle slip mainly on the 001 MMU. The #2 shield was caught. There was damage to the hyd system. A retreiver was used to pull shield out.
While moving MRS the roof began working and the EE started to run and fell down hitting his forehead above right eye.
While pulling cable, the individual had pain in lower left abdomen. Individual did not miss any work days until 12-7-09 at which time he had surgery to repair the hernia.
The individual was loading bolts and plates when a bundle of plates came apart and one plate fell on right hand causing a laceration. The individual was wearing gloves. The laceration required stitches. This is medical only injury report.
The individual was drilling a roof bolt hole and was removing the drill steel from the drill hole when it struck him above the right eye. The individual received stitches and returned to work on next scheduled shift. (Medical only).
The individual was using a chain ratchet to work on a tail piece. The hook slipped hitting him in the forehead. The individual received stitches and saw doctor on following day. (10-1-09). This is medical only.
The individual was using reverse rotation to remove a bent roof bolt from the bolt hole, when the bolt rotated abruptly and struck him on the right forearm.
The employee was operating a 3-wheeled rubber tired personnel transporter when a mechansism in the steering gear box broke and caused the vehicle to go out of control. The vehicle struck a belt conveyor structure rail and the sudden stop forced the individual into the steering wheel. This resulted in a fractured rib to his right side.
EE was kneeling down and when he stood up and turned around he twisted his knee when he turned to get back in shuttle car.
The EE was installing a roof bolt next to the rib, when a piece of rock fell from the rib and struck his right ankle. This resulted ni a bruise to his ankle.
