While trying to connect EE's drill steels together, the bottom drill steel cross threaded with the starter steel and popped the contractor on the left hand resulting in a fracture to EE's left index finger. EE began missing work on 5/14/2026

While dragging a piece of line curtain, employee twisted right knee and felt something pop. Employee was seen at OA and was diagnosed with a torn meniscus. They continued working for several weeks with no restrictions. However, it was later determined that surgery would be required to repair the knee. They began missing work on 7/7/25.

EE got out of EE's shuttle car to throw the miner cable over on in entry 4 on #2 unit. As EE bent over EE felt a sharp pain in EE's lower back. EE finished EE's shift and was seen the next morning at OA. EE missed work on 11/28/2023 and 11/29/2023 but was able to return to work on 11/30/2023 and was able to perform EE's normal job duties.

EE finished the shift and worked the next regularly schedule shift. On 09/13/23 EE was seen by a doctor and diagnosed with a shoulder strain. EE started missing work on 09/13/23 due to restrictions.

X-rays revealed a small fracture. EE is expected to make a full recovery without requiring surgery. EE began missing work on 4/6/2023.

While carrying an 8' timber to set under a bridge, employee slipped on the slick edge they had just scooped. On 7/25/2022 employee was seen by a doctor and was diagnosed with an inguinal hernia and was given no restrictions. They went off work on 11/3/2022 to have the hernia surgically repaired.

A contractor was bending a stamped roof bolt prior to installation. As the contractor pushed down on the roof bolt, it rolled in the contractor's tray causing wrist to hyperextend. X-rays later revealed a small chip fracture in left wrist. The contractor is expected to make a full recovery without surgery being required. The contractor began missing work on 4/7/2022.

An MRI later revealed a tear in left shoulder. EE continued to work until 9/17/2021 when EE was taken off work for surgery. EE is expected to make a full recovery.

The employee was placing the belt bar back onto the belt winder. The mini track operator went to pick up the roll of belt and as EE did the belt rolled off the forks into the employee's legs and pinched EE's legs between the roll of belt and the belt winder bar. The employee began missing work on 05/27/2021 due to possible torn ligament in the right leg.

An MRI later revealed a partial tear in EE's hamstring. EE is expected to make a full recovery without requiring any surgery. EE began missing work on 01/26/2021.
