While trying to connect EE's drill steels together, the bottom drill steel cross threaded with the starter steel and popped the contractor on the left hand resulting in a fracture to EE's left index finger. EE began missing work on 5/14/2026

An unplanned roof fall occurred on Unit #3 in the last open crosscut in between entries 4 and 5, crosscut #9 on the 1st NW Submain. The fall measured approximately 12 feet in length, 17 feet in width and 7 feet in height. The fall landed on the unoccupied roof bolter #6038.

EE was driving EE's shuttle car in #9 entry when EE hit a hole causing EE to jar EE's back. EE was seen at OA and placed off work beginning 5/1/2026 for a possible pinched nerve.

Employee was helping their pin partner reattach the suction hose on a bolter when a rock fell from the edge of the installed screenwire and struck employee in the back of their left hand resulting in a small fracture. Employee is expected to make a full recovery without surgery being required.

EE was throwing miner cable over out of the way when EE felt a sharp pain in EE's lower back. EE was seen by a doctor on 4/1/2026 and placed off work for a possible pulled muscle.

While using a cutting wheel to remove a brace from a metal chute, the brace broke free and fell towards EE inadvertently causing EE to contact EE's right leg with the cutting wheel resulting in a laceration. (8) sutures were required to close the wound and EE was placed off work due to EE's restrictions.

Employee was making the turn out of the belt line when EE's shuttle car struck the inby rib causing a chunk of the rib to roll off onto EE's car and cab. The rib contacted the employee on EE's left arm and left knee resulting in 2 lacerations. EE received (8) sutures in EE's arm and (15) sutures in EE's knee.

Employee was walking an old beltline counting waterline joints when EE lost EE's footing and caught self by grabbing a belt chain causing a sharp pain in EE's shoulder. EE was first seen by a doctor on 11/06/2025 and continued working without restrictions until 02/04/2026 when EE was placed off work for an exploratory surgery to have EE's shoulder repaired.

Employee was loading a shuttle car in the first turn of 7 right. They went to take a step back and lost footing and fell backwards striking their head, neck and upper back against the shuttle car resulting in an acute fracture of the T6 vertebrae. Employee is expected to make a full recovery without surgery being required. They started missing work on 08/27/2025.

Employee inadvertently contacted the corner of the rib with the mast of the mini-trac causing a large chunk of coal to fall, striking the employee in the right hand. The employee sustained fractures and lacerations to EE's right hand. EE began missing work on 08/04/2025.

EE rolled EE's right ankle on a piece of loose coal while walking to the face to help the miner move. EE was taken to OA the next day where EE's x-rays revealed a hairline fracture. EE is expected to make a full recovery without surgery being required. EE began missing work on 6/24/2025.

Employee was using an axe to separate a coupler from a waterline when a piece of the coupler broke off and struck employee in the safety glasses resulting in a bruise to their left eye. Employee was seen by a doctor and prescribed eye drops.

While dragging a piece of line curtain, employee twisted right knee and felt something pop. Employee was seen at OA and was diagnosed with a torn meniscus. They continued working for several weeks with no restrictions. However, it was later determined that surgery would be required to repair the knee. They began missing work on 7/7/25.

While waiting on the miner, the roof bolter operator went to reposition the bolter in the cross cut so that the opposite operator would have room to load the tray. Unaware of the other operator's position, the RB operator slewed the bolter which pinned the opposite operator's leg between the bumper and the rib resulting in a fractured tibia. EE started missing work on 5/7/24.

While changing out belt on the 1st SE, EE was reaching into the diesel scoop bucket to re-attach the belt and asked the operator of the scoop to lower the bucket. Unaware that the foot was under the bucket, EE's foot was pinned to the mine floor resulting in a fracture to the left foot. EE began missing work on 4/30/24.

EE was changing a tire on an outby scoop using the hoist winch on the back of EE's service ride when the winch rope broke. The scoop tire sat down on EE's right ankle causing a fracture. EE started missing work on 4/9/2024.

EE got out of EE's shuttle car to throw the miner cable over on in entry 4 on #2 unit. As EE bent over EE felt a sharp pain in EE's lower back. EE finished EE's shift and was seen the next morning at OA. EE missed work on 11/28/2023 and 11/29/2023 but was able to return to work on 11/30/2023 and was able to perform EE's normal job duties.
