The injured and another person were installing conveyor belt structure. The two men lifted a bottom roller and were putting it in position when the injured's left knee buckled.

The individual was cleaning the water filter at the end of the hard line. While working on the filter, he caught his finger between the water filter and the conveyor belt tail piece resulting in a laceration to his left middle finger.

THE EMPLOYEE WAS HANGING CABLE FOR THE ROOF BOLTING MACHINE WHEN HE FELT PAIN IN HIS RIGHT SHOULDER. HE CONTINUED TO WORK HIS NORMAL JOB BUT CONTINUED TO EXPERIENCE PAIN IN HIS SHOULDER. HE STARTED MISSING WORK ON 12-5-2005.
