EE WAS MANUALLY ADJUSTING THE PADDLES INSIDE A CONDITIONER. PADDLES TURNED AND PINNED LEFT HANDBETWEEN PADDLE AND CONDITIONER FRAME. INJURY WASA CONTUSION OF THE LEFT HAND.
WHILE DISASSEMBLING THE BLISS 2000 PALLET MILL, THE CAP THAT HOLDS THE ROLLER IN PLACE WAS FOUND TO BE STUCK. THE MECHANIC AHD TO USE EXTRA FORCE TO REOVE IT CAUSING IT TO COME LOOSE SUDDENLY , SLIDIN GOFF THE END OF THE SHAFT STRIKING THE SIDE OF THE MECHANICS FOOT, BREAKING THE LITTLE TOE.
EE WAS CLIMBING UP LADDER ON SIDE OF COAL TRUCK BED TO GET COAL SAMPLE. AS HE WAS CLIMBING BACK DOWN THE LADDER, HIS RING ON HIS LEFT HAND CAUGHT ON ONE OF THE RUNGS, CUTTING HIS FINGER.
