Employee was looking for the end of a hydraulic hose. He placed his hand on a pipe, his hand slipped and caused him to get against the hot hydraulic pump.

EE was helping put a cutter chain back on the sprocket when the chain slipped and pinched his left hand between the links. This resulted in a fracture to the top of his hand behind his little finger.

Employee was lifting old "h- bracket" with boom when brace got hung. He then lowered the brace back down when a chain came unhooked, dropping the brace down approx. 12 inches on his left foot.

mine mechanic was assisting, Injured mechanic install a steel bushing in a link using a metal hammer, when mine mechanic struck the bushing it fragmented creating a projectile which penetrated EE's lower left calf. The fragment was to deep to be removed locally, required trip to emergency room for removal.

Employee was standing between two pump motors. He and another employee was attempting to move them. Each motor weighed approx. 800 pound each. As the first one was lifted, the second one became unstable and fell, pinning EE's leg under it.

A pin keeper that holds the tooth on the adapter was being removed when a piece of the adapter broke off and flew through the air approximately 5 to 6 feet. An employee was walking past and was struck in the nose.

HWM pad man was washing the walkway off when a piece of coal became dislodged and flew from the belt, striking him above the left eye. This cut required 4 stitches.

EE was loading the powder truck, standing on top, when the ANFO at the top of the bin broke loose causing the bin to shift abruptly hitting him on the right shoulder. At the same time he felt pain in his lower back.

Employee was assisting another employee with lifting a piece of plate (1"x6"x10'). He felt a sharp pain in his Rt. Shoulder and was sent to the doctor. The doctor released him back to work, but set up further testing. On 8/18/2009, the specialist put him on light duty restriction and set up an appt. with a surgeon.

Employee was doing his preshift exam on his 930E haul Truck, where he found glass in the cab of his truck, due to a window installation. He then began to clean it up when he felt a slight burn in his right eye. He then reported to his foreman and was transported to Boone Hospital. He was treated with eye drops & returned to work on his next shift.

Injured employee was replacing dump ropes on the 8050 dragline bucket. The rope had formed a kink in it and the employee overexerted himself while trying to pull the kink out, resulting in a sprain/strain his right shoulder, arm & neck.

EE states that the lights went out on his dml 50 drill, he got out to check the alternator, cut his right hand on a metal band that holds the exhaust insulation in place.

Employee was removing a jack stand from under the machine and as he tilted it to where the jack could be removed his hand slipped from the stand. The jack stand fell over and caught the employee's hand between the jack stand and the track pad.

The employee was working with the dump ropes on a dragline. He was working on the rigging sheaves while trying to thread the dump ropes through the sheave. He completed what he was doing and noticed he had a puncture wound on his right elbow. He was sent to a clinic where they cleaned the wound, removed a sliver of steel and closed the wound with 1 stitch.

Employee was changing out the dump ropes on the right side of a dragline bucket. They had the rope threaded through the socket and were getting ready to install the wedge, when another employee lost his grip on the rope and it fell out of the socket, striking the injured employee on the right side of his neck and collar bone.

Employee stated that while removing the air box on a 930 Komatsu rock truck, he was putting pressure on the lifting eye to remove the air box. The lifting eye broke and cut left ring finger. He received 10 stitches .

Employee was lowering the boom on the light plant and the crank slipped from his hand. He tried to hold the crank back; while doing that the crank hit his right hand middle and ring fingers. He found that he had fractured the fingers when he went to the hospital.

EE was cleaning out the bed of a service truck, when a piece of steel plate fell off the tailgate and landed on his left foot. The EE was wearing meta-tarsal boots, but they lacked sufficient coverage, resulting in a fractured left pinky toe. A contirbuting factor was that there was a power outage, due to lighting, that resulted in poor visibility.

Employee states that he shut down #7 stacker belt to remove a rock from snub rollar. Once belt stopped, he began removing rock when rock slipped and caught his right index finger, cutting it. He was wearing his gloves.

While trying to hook a chain to the bucket of a 988F Loader to move a 495 dipper door, the loader bucket curled down pinning employee's foot 9 inches into the ground.

Employee was uncoiling 1" wire cable. The cable end recoiled due to tension, striking him in face. Transported to local E.R. Treated for minor cuts and abrasions. Returned to work next scheduled shift.
