EE was operating a locomotive door with no safety features to prevent inadvertant closure. The EE was holding a clip board and lost controll of the door as his hand attempted to clear the door. The door closed on his left ring finger post to the begining of the nail bed, causing a laceration and parial oblique amputation. No loss to use of finger.

Employee twisted right knee mounting his haul truck. Employee returned to work performing normal duties driving truck. On 6/8/2008 employee worked in shop and right knee swelled up. Employee went to doctor on 6/13/08 where he was given a restricted duty release.
Employee was closing the door to the 16g motorgrader and accidently closed it on her right middle finger that was in the door jam. She fractured the tip of her middle finger and received 8 sutures to the laceration. She returned to work on her next regularly scheduled shift.
EE was placing a chock block back into holder on Haul Truck #2043 when a piece of mud fell off underside of the truck and struck her. She was given a full duty work release in the emergency room & returned to work. One week later she went back to doctor on 9/7/07 and was given restricted duty until MRI results come back.
The employee involved was helping other employees move a conveyor table. The conveyor table shifted and part of the table fell on the employee's foot just above the steel toe.
The employee was working on a conveyor hood cover, when the wind blew some sand underneath his safety glasses and into his right eye.
Employee was assisting an oiler take an oil sample on an 854 Catapillar RT dozer. The oiler asked him to shut the machine off and lock it out. When he returned, he stepped into the engine compartment and lacerated his left lower leg. He was transported to the hospital and received 6 sutures to close the wound. He returned to work on his next scheduled shift.
EE WAS CLEANING UP SHAVINGS IN THE TRAY OF THE LATHE MACHINE AND LACERATED HIS LEFT INDEX FINGER. EE RECEIVED 4 SUTURES TO CLOSE THE WOUND AT THE HOSPITAL. EE RETURNED TO WORK FULL DUTY ON HIS NEXT REGULARLY-SCHEDULED SHIFT.
Employee stated he had a sore back prior to the day he was moving CO2 bottles on 2/4/04, but alleges his back happened to get worse on this date. Diagnosis is low back strain. Given prescription medication and restricted duty. Employee did not make supervisor aware of medical treatment until 2/8/05.
EE STRAINED BACK WHILE LIFTING WHEEL CHOCKS OUT OF THE BRACKET ON THE FRONT OF A HAUL TRUCK
EMPLOYEE WAS PERFORMING A PRE-SHIFT WALK AROUND INSPECTION ON HAUL TRUCK. WHEN SHE LOOKED UP TO INSPECT THE TRUCK FRAME SOMETHING ENTERED HER RIGHT EYE BEHIND HER GLASSES. SHE WAS TAKEN TO THE HOSPITAL AND DIAGNOSED WITH DIRT IN HER EYE. IT WAS SWABBED OUT AND SHE WAS GIVEN A PRESCRIPTION ANTIBIOTIC TO PREVENT INFECTION. SHE WAS RELEASED TO FULL DUTY THAT DAY.
While lifting trash bag out of trash can, employee also lifted the trash can. The trash can fell on her toe. At the time, employee did not seek medical attention. She went to the Dr. on July 21st and had the toe nail removed.
While installing lock guide on a Cat 854 Dozer employee got his left hand middle finger pinched between the guide and the wheel causing an open fracture.
Pulling parting 70 bls chunk preparing to replace a 2300 p&h shovel track pad felt pain in right shoulder. continuing to work and perform all functions of job. 7/2/4 asked to see orthopedic surgeon. prscribed steriods and physical therapy to enchance healing of rotator cuff ligaments.
A SHEET OF SCRAP METAL TIPPED OVER AND DROVE A SHARP POINT INTO THE EE BOOT BEHIND THE SAFETY TOE AND CUT THE SECOND TOE, THE CUT REQUIRED STITCHES TO CLOSE IT.
THE EE FELT SOMETHING POP IN HER LEFT CALF AS SHE WAS CLIMBING THE BOARDING LADDER TO HER HAUL TRUCK. SHE APPARENTLY RUPTURED HER ACHILLES TENDON.
A CRANE WAS BEING USED TO LOWER A MOTOR ONTO A SHOVEL AND THE EE WAS GUIDING THE MOTOR BETWEEN THE HOIST ROPES WHEN IT SWUNG AGAINST A HOIST ROPE AND SMASHED THE TIP OF HIS LITTLE FINGER.
EE STRAINED HIS SHOULDER CARRYING A SHOVEL TOOTH. HE CONTINUED TO DO HIS NORMAL JOB. AN MRI BY THE DR SHOWED A TORN ROTATOR CUFF. HE STILL CONTINUED DOING HIS NORMAL JOB UNTIL 4-7-03 WHEN HE D EVELOPED SHOULDER PAIN AND COULD NOT RUN EQUIPMENT. HE WAS ASSIGNED LIGHT DUTY.
EMPLOYEE WAS CLIMBING A LADDER ON A HAUL TRUCK ENGINE COMPARTMENT WHEN HIS RIGHT KNEE GAVE OUT, RESULTING IN POSSIBLE TORN LIGAMENTS.
EE GOT A FORIEGN BODY IN HIS EYE WHILE PUTTING ON HIS HARD HAT. THE WINTER LINER FLIPPED INTO HIS FACE AND CAUSED A PARTICLE TO GO INTO HIS EYE. THE EYE GOT SCRATCHED FROM THE OBJECT AND THE DR. GAVE THE EE A PRESCRIPTION ANTIBIOTIC ASPRECAUTIONARY MEASURE.
EE SMASHED HIS LITTLE FINGER BETWEEN TWO HOIST CYLINDERS, WHILE ROLLING ONE OFF THE FORKS OF A FORKLIFT. THIS CAUSED A FRACTURE OF THE FINGER TIP AND A LACERATION REQUIRING STITCHES.
EE WAS PULLING QUICK PLUG OUT OF DRILL PIPE. HE STARTED TO HAVE PAIN IN LEFT ELBOW AND COULDN'T LIFT HIS ARM.
EE WAS MARKING GLASS TO CUT NEW WINDOWS. WHILE PICKING UP THE OLD WINDOW THAT WAS BEING USED AS A PATTERN, IT SLIPPED AWAY. HE REACHED FORWARD QUICKLY TO MAINTAIN HIS GRIP & CUT HIS KNUCKLES ON A SHEET OF GLASS LAYING UNDER THE PATTERNS. HE RECEIVED SIX STITCHES AT THE HOSPITAL.
A CRUSHER CHAIN SPROCKET FELL OFF, STRIKING EE IN THE SHOULDER, NECK AND BACK. IT THEN FELL TO THE FLOOR AND TIPPED ONTO HIS FOOT CAUSING A SMALL FRACTURE.
EE'S EYE BECAME IRRITATED APPROXIMATELY FOUR HOURS INTO THE SHIFT. A SMALL PIECE OF METAL WAS REMOVED. THE CAUSE OF THE METAL IS UNKNOWN.
EMPLOYEE PLACED HER FINGER BETWEEN A TRAILER HITCH AND END OF THE TONGUE ON A LIGHT PLANT. WHIEL MANEVUEVERING THE TONGUE ITNO PLACE, HER FINGER WAS PINCHED.
