Employee injured servicing GM Autocar fuel truck when an unplanned movement of the truck occured. Employee was struck by the rear bumper of the fuel truck while walking between the rear of the truck and partially closed shop door. Employee sustained a broken pelvis

Employee was standing about 10'away observing a track tourch that was cutting metal. He was wearing both a cutting face shield and safety glasses when he felt something in his eye. This was later found to be metal that was imbedded in his eye and had to be removed by an eye Doctor.

EE was operating a locomotive door with no safety features to prevent inadvertant closure. The EE was holding a clip board and lost controll of the door as his hand attempted to clear the door. The door closed on his left ring finger post to the begining of the nail bed, causing a laceration and parial oblique amputation. No loss to use of finger.

EE was cleaning a weld on a table leg when the grinder kicked back and lacerated left fore arm. Employee was taken to hospital and recieved 8 stitches. Returned to work on next scheduled shift.

Employee was climbing down a ladder on a loader and lacerated his right middle and ring finger on a burr. He needed sutures to close the wound. He returned to work on his next regular scheduled shift with no restrictions.

Employee twisted right knee mounting his haul truck. Employee returned to work performing normal duties driving truck. On 6/8/2008 employee worked in shop and right knee swelled up. Employee went to doctor on 6/13/08 where he was given a restricted duty release.

Todd Pisciotti was stepping over the Cable Reel tow cable behind the P&H Shovel and fell onto the ground dislocating his left ring finger. He was taken to the hospital to have it re-aligned causing a Medical Treatment Case. He returned to work on his next scheduled shift. (Misc Mail 5/30/08 #1-he went out for surgery on 5/15/08 and was placed on restricted duty 5/16/08)

EE was carrying a hydraulic cylinder over his right shoulder. He was on his way to the next floor. As he apporached the first step, he slipped on ice and the cylinder landed on the metal floor, crushing the tip of his right middle finger between the cylinder and the floor.

Employee was changing a blade on a sawsall, did not unplug the tool first.

Employee was working out of a bucket truck swinging a sledge hammer. When he swung the hammer to strike a pin he accidently pinched his left index finger between the handle of the sledge hammer and the lip of the bucket truck at the same time he struck the pin with his bottom hand holding the handle to the sledge hammer. He fractured his left ring finger.

Employee attempted to climb up the ladder of her haul truck, when her left leg slipped off of the first step of the ladder and she slid through to her knee. She felt pain in her left knee. She was seen by a doctor and given pain medication the next day resulting in a medical treatment case.

Scrapping floor with scrapper (hand-held putty knife), had hand in front of scrapper - no gloves on - scrapper slipped, cut skin off tip of finger (index) and cut middle finger.

Employee stepped on the battery box and slipped straining his right knee on the 4008 drill. He continued to work and the pain did not ever go away. On November 28th he had orthoscopic surgery and will miss work.

Employee was closing the door to the 16g motorgrader and accidently closed it on her right middle finger that was in the door jam. She fractured the tip of her middle finger and received 8 sutures to the laceration. She returned to work on her next regularly scheduled shift.

Empty haul truck was heading back to cut, when met by oncoming truck, which was taking up too much of the road. Moved over too far and got into muddy ditch. This made him slide sideways, jackknifing truck on left hand side of road slamming him around inside cab. Caused broken arm, ribs and head laceration.

The employee involved was helping other employees move a conveyor table. The conveyor table shifted and part of the table fell on the employee's foot just above the steel toe.

EE was placing a chock block back into holder on Haul Truck #2043 when a piece of mud fell off underside of the truck and struck her. She was given a full duty work release in the emergency room & returned to work. One week later she went back to doctor on 9/7/07 and was given restricted duty until MRI results come back.

Employee was training on the 4008 drill and was dismounting the drill when he stepped onto a lump of coal. His knee twisted and he felt pain in his left knee. He was diagnosed with a torn meniscus. He had surgery on his days off and returned to his normal truck driving duties on his next regular scheduled shifts.

While hauling topsoil to dump area employee noticed something inside his air conditioner (a piece of paper). He took the louver off of the A/C unit & reached inside the unit & put his finger into the fan, stopping the fan & shorting out unit. Caused laceration to right ring finger requiring medical assistance.

The employee was walking down from the second level of the truck dump when a gust of wind blew something under his safety glasses and into his right eye.

We lost power to the bottom of the silos which also runs the elevator. An employee was 3 feet from the top on the elevator. Electrician troubleshooted for 25 minutes and a decision was made to put a harness on EE & have her step out the top of the cage onto the silo deck with a rope tied onto her by the response team instead of hand brake the cage 217 feet. MSHA was notified

The employee was working on a conveyor hood cover, when the wind blew some sand underneath his safety glasses and into his right eye.

Employee fell onto ice while attempting to move shovel cable in pit #1. Employee dislocated right shoulder and strained right ribs & neck. Employee returned to work on next scheduled shift.

Work: Remove drag hitch extension pin using welded pulling eyes on bucket and pin with a 2 ton come-a-long. Employee was cleaning dirt from pin while it was under tension. Weld broke and he was struck in the face by the hook, clevis and eye. Laceration of upper lip required 8 stitches, with cleaning of abrasions to nose and cheek.

While a mechanic was performing the task of cutting safety glass, methanol vapors ignited burning the mechanic's left arm and hand. The mechanic incurred 2nd degree burns to his left hand and forearm and 3rd degree burns to the finger tips of his left hand. The mechanic was transported to a medical facility and did not return to his next scheduled shift.

Employee was assisting an oiler take an oil sample on an 854 Catapillar RT dozer. The oiler asked him to shut the machine off and lock it out. When he returned, he stepped into the engine compartment and lacerated his left lower leg. He was transported to the hospital and received 6 sutures to close the wound. He returned to work on his next scheduled shift.

A P&H shovel bucket jarred haul truck #2037. The employee was seen at the hospital and given a work release. One week later, returned to the doctor and received another work release. On 3/26/06 he again went to the doctor and received 3 weeks of physical therapy. This employee is presently under JRM investigation for falsifying pre-employment medical history.

EE was loosening a hydraulic filter housing on an 830E haul truck company # 2039 when his chain wrench broke and the chain grazed (lacerated) his left cheek. He was taken to the campbell County Hospital & received 9 sutures to close the wound. Returned to work next reg. scheduled shift.

Employee stepped from DRAGLINE roof into manlift & caught his right foot on a pedal, bending his foot causing pain to his ankle. He was treated & released to return to work. Transferred to another job. 3rd day in complained of pain to ankle.Taken to clinic 3/25/06, referred to ortho who found that he had a broken ankle. Taken off work for 3 days & released to modified duty.

Employee was grinding when received a foreign body to both eyes.

EE BACKED HAUL TRUCK #2082 UNDER THE 1214 XPB SHOVEL & RECEIVED THE 1ST BUCKET LOAD OF MATERIAL (120 TONS). ON THE 2ND PASS, THE OPERATOR WAS JOLTED BY THE MATERIAL BEING LOADED INTO THE HAUL TRUCK AND SUSTAINED LOWER BACK INJURY. SECOND BUCKET OF MATERIAL WAS ALSO 120 TONS OF OVERBURDEN. OPERATOR WAS WERAING HER SEATBELT.

Employee was walking down the stairs and turned to step off the last step. Employee stepped off and scraped (L) thigh on the corner of the ledge requiring stitches.

Operating a 773, belly hit a rut in the road and he was jarred hard enough for head to hit the right door handle.

During the 2005 wellness testing, a severe hearing loss (3, 4 6K range) was detected in EE's left ear. Sent to hospital for hearing re-test. Evaluated by an audiologist & recommended a follow up with a doctor. Examined by doctor and given MRI which found no medical evidence to suggest the hearing loss. Therefore, we are reporting the loss.

Employee was cutting tape off a cable when the box cutter he was using slipped and cut his (L) hand requiring stitches.

Employee unexpectedly placed finger in a hole between two pieces of steel while steel was struck by a 10lb sledge hammer, cutting off a portion of the index finger on the left hand.

Employee alleges wrist pain from her moderate grinding duties, but has been allowed to return to regular duty by the doctor. On 6/23, she was prescribed medication and physical therapy which now makes this injury reportable. She can still work her regular duties.

EE WAS CLEANING UP SHAVINGS IN THE TRAY OF THE LATHE MACHINE AND LACERATED HIS LEFT INDEX FINGER. EE RECEIVED 4 SUTURES TO CLOSE THE WOUND AT THE HOSPITAL. EE RETURNED TO WORK FULL DUTY ON HIS NEXT REGULARLY-SCHEDULED SHIFT.

EE was attempting to dump #2044 haul truck into circuit #1 and #2 coal hopper. The bed went up approx 6 ft and stopped. He attempted to feather the bed down with the controls. The bed slammed onto the frame and he injured his lower back. He returned to work full duty for three shifts, but the pain continued. He requested the Dr and missed his next regular shift 3-9-05.

Employee stated he had a sore back prior to the day he was moving CO2 bottles on 2/4/04, but alleges his back happened to get worse on this date. Diagnosis is low back strain. Given prescription medication and restricted duty. Employee did not make supervisor aware of medical treatment until 2/8/05.

Employee was grinding in the fan house area and felt a foreign object in his (R) eye which was embedded in his cornea.

EE WAS DRIVING A PIN INTO A SHOVEL TRACK USING A LARGE SLEDGE HAMMER AND STRAINED HIS BACK.

EE STRAINED BACK WHILE LIFTING WHEEL CHOCKS OUT OF THE BRACKET ON THE FRONT OF A HAUL TRUCK

Ee was riding on a mine pit bus when something entered his right eye. He was taken to Campbell County Hospital & diagnosed with a granular substance in his eye. It was swabbed & he was given a prescription antibiotic to prevent infection. He then returned to work that night with no restrictions.

EMPLOYEE WAS IN THE LEAD SCRAPER PUSHING THROUGH AN OVERBURDEN REGRADE AREA. THEY HAD FINISHED LOADING AND WERE GOING DIRECTLY TO THE FILL AREA TO BEGIN DUMPING THEIR LOADS. EMPLOYEE WAS BUMPED BY THE REAR SCRAPER AS THEY WERE COMING UNHOOKED. HE WAS LOOKING BACK TO ASSESS THE SITUATION AND DID NOT RECOGNIZE A ROCK IN HIS PATH. HE DROVE OVER THE ROCK WHICH IMPACTED HIS BACK.

EMPLOYEE WAS PERFORMING A PRE-SHIFT WALK AROUND INSPECTION ON HAUL TRUCK. WHEN SHE LOOKED UP TO INSPECT THE TRUCK FRAME SOMETHING ENTERED HER RIGHT EYE BEHIND HER GLASSES. SHE WAS TAKEN TO THE HOSPITAL AND DIAGNOSED WITH DIRT IN HER EYE. IT WAS SWABBED OUT AND SHE WAS GIVEN A PRESCRIPTION ANTIBIOTIC TO PREVENT INFECTION. SHE WAS RELEASED TO FULL DUTY THAT DAY.

While lifting trash bag out of trash can, employee also lifted the trash can. The trash can fell on her toe. At the time, employee did not seek medical attention. She went to the Dr. on July 21st and had the toe nail removed.

EE WAS ATTEMPTING TO PULL A METAL CHOKER OFF A RIPPER SHANK ON A 854 DOZER WITH A PRY BAR. THE PRY BAR SLIPPED OUT OF THE LOOP WHEN HE WAS APPLYING PRESSURE AND HE ROLLED HIS RIGHT ANKLE OVER AS HE FELL. EE FRACTURED HIS RIGHT ANKLE. HE WAS TRANSPORTED TO THE HOSPITAL AND RECEIVED SURGERY TO REPAIR THE ANKLE.

EE was training a truck driver who lifted the bed of the haul truck approximately 2-3 feet short of the hopper. EE tried to feather the bed down fully loaded. The bed slammed down, causing pain in his neck. He continued to work restriction free until we received a doctors restriction on 9/13/04 to not drive haul truck due to a disc lesion diagnosed in an MRI.

While installing lock guide on a Cat 854 Dozer employee got his left hand middle finger pinched between the guide and the wheel causing an open fracture.
