I was getting my toolbox on the bed of my truck after picking up the toolbox I turned to get down when I tripped on a jack which caused me to fall off my truck and I landed on the concrete surface head-first and I tried to break my fall with my left arm but I still hit my head. I received a cut on my head.

Showing 50 of 61Employee was adjusting the two lengths of poly water pipe into the fusing machine when EE's left index and middle fingers got caught between the poly water pipes ends, which pinched the tips of the two fingers.
While employee was lifting 4 foot cribbing. EE felt a sharp pain in right shoulder. IE reported it to Supervisor and was taken to and seen at the Mine Clinic. IE was referred to another Clinic for further evaluation where Doctor determined it was a rotator cuff tear.
After employee finished replacing troughing idlers on beltline, ee was in the process of removing used idlers off the platform on the catwalk. To clear the walkways and got left hand caught in between handrail and idler frame, pinching the side of left thumb, receiving a laceration which required 7 stitches.
Trainee was moving cable at 2570. Trainee twisted as they were lifting cable and felt lower back pain. Reported to supervisor.
IE severed the left middle finger tip while installing a push arm assembly on a D-11 dozer.
The IE was lowering a tow pin weighing 25 lb. on the bumper of a water truck. As the IE was trying to lower the pin, it got hung up and wouldn't move. As the IE applied more force, the 25 lb. pin fell 3" to 4" inches and struck the IE's hand, causing a fracture to the left thumb.
The EE was in the process of removing a hydraulic filter from drill 812 when EE injured a middle finger. EE first removed the bracket that secures the filter housing assembly. EE then held the filter housing assembly with left hand and began to remove four bolts located on top. When EE removed the last bolt, the filter housing dropped and crushed the left middle finger.
The dragline operator and oiler were in the process of inserting a shaft roller on top of the dragline house. The operator lost hold of the shaft roller and it slipped from hands, left middle finger was pinched between the shaft roller and housing causing a laceration. Over the weekend the wound became infected.
EE in an attempt to remove broken bolts from a dozer track pad, lacerated the heel of his gloved left hand. EE had positioned his left hand along the outer edge of the pad, then manhandled the freed pad towards him, when it slid into the adjacent pad pinching his hand. The EE was taken to the Kayenta Health Clinic where the laceration was sutured.
The injured employee sustained laceration to his left pinky when a metal catwalk fell on his left hand after losing his grasp. He was initially seen at the mine clinic and later taken to Kayenta for further evaluation. The finger was not fractured but sutures were needed to close the cut.
Employee while assisting two other individuals in replacing a troughing idler, suffering injuries to his left wrist/hand. The employee's hand was pinned between the idler and the conveyor belt frame. The crew had managed to nearly upright the idler when a wrench used in process slipped off the idler causing it to fall on the injured limb, possible navicular fx.
Mechanic was removing and replacing bit and sub on drill steel when it shifted. Thumb got caught between steel and nylon sling. Mechanic sustained broken and laceration to his thumb.
Employee was removing crusher liners when he pinch his finger.
Employee was cleaning trash in scraper parking lot. Lifting a 55 gallon barrel full of trash. Was taken to hospital for further evaluation, and was diagonis with lower back injury. Was given one week off and to get more treatment.
The injured employee's left hand was caught in between the frame and the pivoting idler base in a scissor like fashion pinching the left ring finger between the 2 pieces of steel. The finger tip, about 1/4" was amputated. The IE was wearing gloves at the time of the incident.
Mechanic was in a sitting position when a co-worker walking by threw a piece of rock at him striking his right. Right hand sustained a simple fracture of a metacarpal of little finger.
Injured was trying to turn the swivel of a portable light plant into position and when it came loose, injured's right little finger struck the light plant mast stopper causing a cut to the finger tip.
The Prep-laborer reached with his right hand to remove a steel choker while a splice house was lowered onto a lowboy trailer. The cross brace of the splice house and surface of the trailer pinched his right mid-finger. The finger sustained an avulsion requiring sutures to close the wound.
Injured dropped a 4"x32" bolt on his gloved right index finger while bolting the dragline hoist case together. Lacerated finger required 3 stitches at a local hospital and subsequently returned to work completing his shift.
The mechanic was removing the dust filter collector cone when it slipped from his grip and dropped on his left ring finger.
Hand crushed between actuator rod and park brake actuator.
The mechanic was holding the sides of the planetary cover of wheel assembly and putting it in place onto the housing when his right mid-finger was pinched.
The dragline oiler was holding a pry bar as the dump rope wedge was being hammered into the socket when a lift chain holding the wedge came loose. The loose chain struck the oiler's skull when his hard hat fell off.

The loader operator was closing the cab-window door when his right little finger was pinched between the door and jamb of the door way.
The mechanic was closing a fuse panel door on a loader after he made the necessary repairs when it slipped out of his left hand. The panel built-in rotating fans of air condition struck his left thumb lacerating it.
The welder was changing out oxygen bottles gauge in the back of his welding truck. When he lifted a protective bottle gauge cover-door to perform the task, the cover-door came back down striking his left hand.
The technician was greasing on the sample system when his right hand was caught between tertiary blade cutter and a limit switch bracket; lacerating the palm of his right hand.
Reparing a Drill Tech Drill, the employee descended a ladder and caught his left hand on railing, cutting his left finger, requiring stitches
The welder was prying out a shovel cable that was caught between the crowd bull-gear and it's guard when it broke loose. The cable struck the welder's left eye, face and his left little finger. The impact from the cable also caused him to fall backwards onto the crowed track below.
Employee was changing gauges on oxygen cylinders. He removed a cap & set it on another cylinder when it slid off & caught his thumb between the cap & oxygen bottle. Received a fractured (L) thumb. Returned to regular duties
12"IBEAM ROLLED ONTO EMPLOYEE'S R FOOT WHEN SLACK OCCURRED IN RIGGING RESULTING IN A FRACTURE.
Injured was trying to remove a barrel-plug by hitting it with a piece of metal strap when he missed it. He struck his right thumb against the sharp edge of plug head; lacerating it.
The welder was setting a pear-link Metal on a table when it slipped off. It caught his left hand between the linke and load line of an overhead crane. He was operating the crane controls with his right hand and wearing protective gloves which minimzed the laceration on the dorsum side of his left hand.
EE WAS WORKING ON A FENCE PROJECT. EE WAS PULLING THE BARB WIRE AND JERKED THE BARB WIRE WHEN IT HUNG UP ON A SHRUB. THE BARBED WIRE PENETRATED THROUGH THE GLOVE AND CUT EE RIGHT HAND INDEX FINGER. THE CUT WAS ABOUT 1/2" IN LENGTH ON SKIN
BUILDING A TIN STRUCTURE, THE TOP "Z" CHANNEL SLIPPED OUT OF POSITION WHEN THE MIDDLE CHANNEL WAS CUT LOOSE. SO TO PUT TOP CHANNEL IN POSITION, WHEN TOP CHANNEL FELL AND HIT EMPLOYEE ON THE HEAD. CHANNEL FELL 12 INCHES. HE DIDN'T HAVE HIS HARD HAT ON. ANOTHER MAN WAS HOLDING THE OTHER END OF THE "Z" CHANNEL UP. CHANNEL WAS 18' LONG AND WEIGHED ABOUT 80 POUNDS.
THE EMPLOYEE WAS UNROLLING A FIELD FENCE WHEN HE WAS INJURED. HE WAS PUSHING ON FIELD FENCE TO GO OVER A BIG SHRUB AND HE SUDDENLY SLIPPED. THE FIELD FENCE ROLLED BACK TOWARDS HIS FACE AND HIT HIM. THE INJURED SUSTAINED AN ABRASION, A CUT ON HIS INNER LIP AND A CHIPPED TOOTH.
Loading brake assemblies on service truck, employee was kneeling down to place cribbing under the brake assembly so that it could be slung to load. Hook slipped from the track pad and hit the employee in the left eye area.
THE EE WAS DIGGING IN HIS LOCKER FOR SOME TOOLS. A LOADED, HEAVY TOOLBOX FELL FROM THE TOP SHELF OF THE LOCKER AND LANDED ON HIS RIGHT MID-TARSAL OF THE FOOT. HE FELT PAIN THE NEXT DAY WHEN HE STOOD OR WALKED ON IT.
THE WELDER WAS PERFORMING HOUSEKEEPING DUTIES IN THE SHOP & AS HE WAS MANEUVERING & PLACING A CRUSHER SLIDE PLATE IN AN UPRIGHT POSITION, THE INNER ADJUSTABLE PLATE SLID DOWN CATCHING HIS LEFT RING FINGER.
WHILE PULLING HIMSELF OUT OF BACKHOE SEAT HE WRENCHED HIS ARM POSSIBLY PARTIALLY DISLOCATING ELBOW. HE ALSO HIT HIS WRIST/HIS ARM WAS CHECKED AT HOSPITAL AND IT IS FINE.
THE EE WAS REPLACING A DOZER RIPPER SHANK GUARD. EE'S RIGHT INDEX FINGER WAS CAUGHT BETWEEN THE RIPPER SHANK AND ITS GUARD WHEN THE GUARD WAS RELEASED INTO ITS PLACE FROM ITS SUSPENDED POSITION BY A COWORKER.
THE WELDER WAS REMOVING AN EXPANDED METAL GUARD OFF A FLAT BELT WHEN IT HANG-UP WHEN HE TRIED TO REMOVE IT. HE PULLED ON IT & IT CAME LOOSE ON HIM CATCHING HIS LEFT RING FINGER BETWEEN THE GUARD & IT'S BRACE.
While handing a hydarulic hose with co worker, miner strained right leg ham-string.
EE & COWORKER WERE CLOSING TEMPORARY FENCE GATE & SECURING IT W/SMOOTH WIRE ATTACHMENT OF WOODEN TREE BRANCH. EE WAS PULLING ON WOODEN TREE BRANCH TO SECURE IT TO WOODEN POST & HIS COWORKER WA S PUSHING & PULLING ON T-POST & FENCE GATE THAT ARE TIED TOGETHER. AFTER HIS COWORKER REMOVED HIS HAND FROM PULLING TO ASSIST EE IN SECURING TREE BRANCH ON WOODEN POST, THE GATE FENCE JERKED
The welder was moving a pallet box full of quipment parts by hand when he felt backwards, after the box he was holding tore off during the pull. he strucked his head against an argon bottle rack directly behind him.
EE SUFFERS FROM CHRONIC KNEE CONDITION. STRAINED KNEE WHILE HANDLING POWER CABLE.
THE INJURED WAS PULLING DOWN & CLAMPING BARBWIRE FENCE. THE TENSION ON THE BARBWIRE FENCE CAUSED IT TO BREAK & SPRING BACK TO WHERE HE WAS CLAMPING. THE BARB CUT HIS RIGHT HAND.
THE WELDER WAS INSTALLING TROLIGHING IDLER DURING BELTLINE REPAIR WHEN HIS RIGHT RING FINGER WAS PINCHED BETWEEN AN IDLER FRAME AND THE BELT MODALE.
