On 1/11/10, the employee was helping to lower a pump into a jeep using a come-a-long. The pump started to tip on the jeep floor and the employee grabbed hold of the top of the pump to keep it from falling over,and felt pain in right shoulder. The employee has continued to work at his regular job until 8/2/10, when he started missing to have surgery on his right shoulder.

Employee was helping to remove 2 joints of concrete pipe while pouring cement for belt pad. One of the pieces fell on his right foot. He started missing work on 7/5/2008 due to pain in right foot.

1/16/05 EE DID NOT SPECIFY A SPECIFIC INCIDENT. HE STATED THAT AFTER WORKING INSTALLING TROLLEY WIRE OVERHEAD, P1/SHIFT THAT HE FELT TIGHTNESS IN SHOULDER. HE STATED HE FELT IT WAS FROM WORKING OVERHEAD FOR 10-12 YRS AS A WIREMAN. WAS DIAGNOSED WITH A TORN ROTATOR CUFF.
Employee was helping to remove monorail from 3-B beltline. as he was dropping a section of monorail off the belt the flange struck his left wrist.

As ee was advancing slider tube towards face he felt something pull in left shoulder.

EMPLOYEE PICKED UP A ROCK FROM WALKWAY ALONG BELTLINE. AS HE TURNED TO THROW THE ROCK ONTO THE BELT, HE STRUCK HIS LEFT HAND ON A BELT ROLLER FRAME.

EE WAS HELPING TO REINSTALL THE BATTERY LID ON THE SCOOP BATTERIES. AS THE LID SLID INTO PLACE,IT PINCHED HIS RT. RING FINGER BETWEEN THE BATTERY CASE AND THE LID.
EE WAS GREASING THE BEARING OF THE SLOPE BELT BEND PULLEY. HE WAS HOLDING THE GREASE GUN WHEN SLACK FROM EXTENSION HOSE CAUGHT ON THE PULLEY LUG. HE PANICKED AND TURNED TO RUN WHEN HE STRUCK H IS HEAD ON A SUPPORT BEAM. ALSO RIGHT SHOULDER.
