On 1/11/10, the employee was helping to lower a pump into a jeep using a come-a-long. The pump started to tip on the jeep floor and the employee grabbed hold of the top of the pump to keep it from falling over,and felt pain in right shoulder. The employee has continued to work at his regular job until 8/2/10, when he started missing to have surgery on his right shoulder.

Employee was removing a modulating valve from rock dust tank car. The valve fell off pinching his left index finger between the valve and a reflector bracket that was mounted on the car. Fractured left index finger.

Employee was helping to remove 2 joints of concrete pipe while pouring cement for belt pad. One of the pieces fell on his right foot. He started missing work on 7/5/2008 due to pain in right foot.

Employee was getting out of compartment after putting motor on charge. He struck his left ear on a fire suppression spray in the process, which required 5 sutures to close the laceration.

Employee was operating a trolley jeep on main haulage. The pole jumped off wire and struck overcast. As employee reached to get pole, it bounced back striking him in mouth and knocking out front tooth.

Employee was dismantling guard gate of rail car, top half of the gate fell pinching the tip of the middle finger on his right hand.

On 5/21/2008 the employee was pulling cables through stopping and felt pain in left groin area. He has continued to work until 7/9/2008 when he started missing due to pain in groin area.

Employee was helping to carry a belt roller frame along the main line belt. He stated that he felt something snap resulting in pain to his right ankle.

Employee was helping to unload vent tubes from bucket of scoop. He placed his right hand inside one of the tubes. The tube accidentally slid against the scoop bucket causing a laceration to his ring finger on his right hand.

Employee was standing alongside a bundle of steel roof props. He rested his right hand on the base of one of these props. The other end of the prop slid off the bundle which caused the base plate his hand was on to pinch the tip of his right finger against another base plate.

On 9/5/07 EE was in the process of re-fueling a compactor. He strained his left shoulder while removing the fuel nozzls. He continued to work until 11/1/07 at which time he began missing work due to pain in lt. shoulder.

Employee was unloading belt rollers off supply car and felt pain in his right shoulder. The employee has continued to work until 4/25/2008 when he started missing work due to pain in right sho ulder.

Employee was getting out of company truck at the Prep Plant Office. The middle finger on his right hand got caught in the door frame as he closed the driver's side door.

Employee stated that he was pulling on rockdust hose and felt pain in his groin area. He continued to work until 7/23/07 at which time he was diagnosed with a bilateral hernia.

EE complained of left knee pain after working on the longwall face. EE was diagonsed with degeneration to posterior horn lateral meniscus and medial meniscus , started missing time on 6/18/07.

Employee was replacing liner plate in feeder chute. Liner shifted catching his left thumb. Diagnosed with a fracture. (Started missing time 4/26/07.)

Employee was trying to remove a ram jack. He caught the tips of his middle and index fingers between the ram jack and frame of thru conveyor.

Placing trolley pole on wire, felt pain in abdomen. Diagnosed on 5/7/07 with an umbilical and bilateral hernia.

Employee caught right index finger between ram and frame of scoop pinching tip of his finger. (requiring sutures)

Employee was lifting an ATRS pad and felt pain in groin area. He was diagnosed with a hernia and started missing time on 3/6/07.

Employee reported that he felt pain in his left knee cleaning out his operators compartment with his foot. Employee was diagnosed with a meniscus tear to left knee he started to miss time on 12/18/06.

Employee was helping take down vent tubes. He pinched his left thumb between two tubes. Diagnosed with a tuft fracture of thumb. (Did not lose any time.) Was injured 9/13/06 but was not diagnosed till 9/18.

Employee dropped drive cover brace on right hand. He was diagnosed with a fracture to right middle finger at first joint. He returned to full duty and the fracture wasn't discovered till 8/15/06.

Employee was holding a step ladder while a second employee was taking down a c-clamp. It fell striking employee on top of head.

Emoloyee was lifting his tailgate on his truck it slipped out of his hand striking his right knee. He started missing work on 10/20/05.

Employee was changing brake shoes an a locomotive. The brake shoes slipped, pinching his little finger on left hand.

He was changing a 25hp pump and as he pulled it over the hand rail he felt a pain in his right groin. Hernia was diagnosed on 8/30/05. Surgery is scheduled for 9/6/05.

Employee was recovering ventilation tubes. In the process one fell on top of the fan tail tube causing it to deflate. When it inflated it caused the vent tube to pop up striking him on the jaw causing a laceration inside his mouth requiring sutures.

Lifting draw bar after coupling it up, he started to get up and felt pain in lower back.

Employee was setting a rock down on top of a water suction line. He pinched his left middle finger under the rock. He has a hairline fracture of the finger.

Employee pushed a bottom roller under the belt. He stated that he felt pain in his left arm & something popped in his arm.

REMOVING OLD WATERLINE AS EE UNCOUPLED PIPE IT MOVED PINCHING RIGHT RING FINGER BETWEEN PIPE AND ROOF

After carrying a new jack to work area he complained of numbness in his right arm.

1/16/05 EE DID NOT SPECIFY A SPECIFIC INCIDENT. HE STATED THAT AFTER WORKING INSTALLING TROLLEY WIRE OVERHEAD, P1/SHIFT THAT HE FELT TIGHTNESS IN SHOULDER. HE STATED HE FELT IT WAS FROM WORKING OVERHEAD FOR 10-12 YRS AS A WIREMAN. WAS DIAGNOSED WITH A TORN ROTATOR CUFF.

Employee was helping to remove monorail from 3-B beltline. as he was dropping a section of monorail off the belt the flange struck his left wrist.

EMPLOYEE STATED HE WAS THROWING A TRACK SWITCH WHEN HE FELT SOMETHING PULL IN HIS RT. SHOULDER.

EMPLOYEE WAS UNLOADING MONORAIL SECTIONS OFF THE BELT, HIS END PIVOTED PINCHING RT.RING FINGER AGAINST ROOF.

As ee was advancing slider tube towards face he felt something pull in left shoulder.

Two employees were cleaning of top of miner. As they were pushing on a large rock it slid pinching one employee's left middle finger.

EMPLOYEE WAS UNLOADING A SPOOL OF CABLE, WHILE PUSHING HE FELT A PAIN IN HIS LOWER BACK.

Employee felt a "pull" in his groin area while moving supplies on his truck bed. He continued working and was later diagnosed with a hernia.

Employee was taking the scrap basket from dryer to the outside of the prep plant. he cut his rt index finger on a sharp piece of metal on the outside of the basket.

Employee was turning on the water valve for the spray bar of the longwall shear. In the process, the lid on the compartment that the valve was housed in fell pinching the tip of middle finger on rt hand.

EMPLOYEE AND CO-WORKERS WERE CHANGING SHEARER JACK ON MINER. AS THEY WERE PUTTING THE NEW JACK IN PLACE, AND ATTEMPTING TO LINE UP BOTTOM EYE, THE JACK MOVED AND STRUCK THE TOP OF HIS LEFT HAND CATCHING IT BETWEEN THE JACK AND MINER PLATE.

EMPLOYEE PICKED UP A ROCK FROM WALKWAY ALONG BELTLINE. AS HE TURNED TO THROW THE ROCK ONTO THE BELT, HE STRUCK HIS LEFT HAND ON A BELT ROLLER FRAME.

Employee was lifting beams to put in place.

EE ANCHORING BUCKET. AIRLINE END BLEW OFF SPRAYING ROCKS & ONE HIT EE IN THE HEAD.

EE WAS DIAGNOSED WITH A HERNIA WHICH HE BELIEVES IS WORK RELATED DUE TO YEARS OF PERFORMING HIS NORMAL DAILY DUTIES.

EE WAS HELPING TO REINSTALL THE BATTERY LID ON THE SCOOP BATTERIES. AS THE LID SLID INTO PLACE,IT PINCHED HIS RT. RING FINGER BETWEEN THE BATTERY CASE AND THE LID.

EE WAS GREASING THE BEARING OF THE SLOPE BELT BEND PULLEY. HE WAS HOLDING THE GREASE GUN WHEN SLACK FROM EXTENSION HOSE CAUGHT ON THE PULLEY LUG. HE PANICKED AND TURNED TO RUN WHEN HE STRUCK H IS HEAD ON A SUPPORT BEAM. ALSO RIGHT SHOULDER.
