The Somerset elevator failed to operate when called by an underground employee. The elevator contractor was called to the mine to repair. There was no damage to the elevator, the repair consisted of a door adjustment. There were no persons injured, or entrapped due to this event.
A supply trip was being hoisted at the Somerset Slope. A pin broke on a drawbar, along with a safety rope which was connected to the trip. This caused the cars to travel back down the slope and de-rail at the bottom. There were no persons injured or equipment damaged as a result of this occurrence. This report is being filed to satisfy the MSHA Inspectors actions.
The automatic doors on the Somerset Portal elevator stopped working when the motor controlling the door opener circuit failed. The motor was repaired and replaced by the elevator repairmen. The elevator was returned to service on 1/27/2011 at 2:45PM. There were no persons injured as a result of this occurrence.
The Livingston Portal elevator stopped operating when the elevator car stopped below the mine level landing. The elevator repair service responded and placed the elevator back in service at 10:45 AM. There were no persons injured, or entrapped, as a result of this incident.
The Somerset Portal elevator stopped working while at the bottom landing. The doors would not close, and the elevator would not operate. The elevator repair service responded and had the elevator back in service at 8:25 AM. There were no persons injured, or entrapped, as a result of this incident.
The employee started missing work on 11/24/2010, due to surgery for carpel tunnel. The employee alleges this condition exists from years of repetitive motion from working at the mine.
A Fireboss found the elevator doors at the Somerset Portal elevator open on the Surface landing. When he tried closing them they would not completely close which kept the elevator from operating. There were no persons injured or equipment damaged as a result of this incident.
An employee was riding in the Somerset Portal elevator from the mine level to the surface. The elevator stopped approximately 100 ft. from the surface landing causing entrapment of the employee. There were no injuries or equipment damaged as a result of this occurrance.
Employee stated that on 9/13/2010 he was stepping through a personnel door. As he stepped through with his right foot he stepped on a piece of coal twisting his right knee. He has continued to work until 11/15/2010 when he started missing work due to a right knee sprain.
While examining an aircourse, the employee discovered a roof fall in the intersection of #9 Cross-cut of 23 Mains Section. This is a continuation of a previous fall in the same location. There were no persons injured, or equipment damaged as a result of this event.
On 8/11/2010, an employee at the Somerset Portal of Mine 84 tried to use the elevator at this facility. He found that the elevator doors were stuck in the closed position. The elevator repair company was called and placed the elevator back in service at 6:00AM. There were no injuries, or equipment damage resulting from this event.
On 3/17/2010 the employee was in the process of traveling through a personnel door in a stopping when he struck his right elbow against the door frame and felt pain in right shoulder. The employee has continued to work at his regular job at full capacity until 5/17/2010 when he started missing work due to his injury.
On 1/11/10, the employee was helping to lower a pump into a jeep using a come-a-long. The pump started to tip on the jeep floor and the employee grabbed hold of the top of the pump to keep it from falling over,and felt pain in right shoulder. The employee has continued to work at his regular job until 8/2/10, when he started missing to have surgery on his right shoulder.
Three men were entraped in the Livingston Portal Elevator when a fuse had blown in the elevator control room. The elevator had stopped at about 100 ft. in the shaft from the surface. There were no injuries that resulted to any persons in this incident. The men were safely removed from the elevator at 10:20 AM.
The Livingston Portal elevator was removed from service for a bent safety rod. There were no persons injured as a result of this event. The elevator was repaired by the elevator contractor and placed back in service at 8PM.
On 12/10/2009, the employee was walking toward the power center in his work area. He stated that he tripped on a rock in the walkway and fell on to his right side. He continued to work at his regular job until 12/17/2009, when he started missing work due to pain in right leg.
During the normal weekly Airway walk, an examiner discovered a roof fall blocking the aircourse. There was no damage to equipment, and no persons were injured as a result of the event. This occurred at the #9 X-Cut line in the old 23 Mains area.
A surface electrician was in the process of making his routine inspection of the Hallam Portal elevator when it stopped halfway up the shaft. Maintenance personnel reset controls to put the elevator back in service. There were no persons injured or equipment damaged as a result of this incident.
Employee was in the process of cleaning out a shelter hole along the track haulage. His left foot was placed on a coal shelf on the mine bottom by the shelter hole. As he reached down to pick up a large piece of coal toward the back of the shelter hole, his left foot slipped causing him to hyper extend his knee. The employee started missing work on 10/10/09 due to this event.
On 9/19/09 at 4:30AM, the interior doors on the Somerset elevator failed to close. The incident occurred as an employee stepped off the elevator at the mine level landing. The elevator repair service repaired the door lock mechanism and replaced a guidewheel on the elevator in order to put it back in service. There were no persons trapped, or injured as a result of this incident.
A roof fall occurred on an intersection in the old #6 entry of 33Mains. The fall has occurred in an inactive area of the mine. There were no persons injured, or equipment damaged as a result of this event.
In the process of making the weekly exam, a roof fall was discovered by the examiner. The location of the fall was in the cross-cut immediatly outby #22 Seal. No persons were injured or equipment damaged due to the event.
Employee was walking down the steps at the Prep Plant. She slipped on the last step twisting her left knee. She has continued to work until 10-2-09 when she started missing work for a torn meniscus.
There were 2 elevator service employees who became stuck on the Livingston Portal elevator. This occurred when they stopped to examine the governor rope and were going to continue up the shaft. There were no injuries or equipment damaged as a result of this incident.
On 3/31/09 the employee was kneeling down using a water hose to clean material from under a belt pan. The hose blew out, causing the employee to be startled and strained his left hamstring in the process of getting out of the way. The employee started missing work on 4/2/09 for a severe left hamstring strain.
In the process of chipping ice in the Livingston Portal elevator shaft, a piece of ice fell damaging an elevator door switch on the shaft bottom. An employee became stuck on the elevator in the process. The employee was able to get off elevator at 7:48PM. There were no injuries as a result of this incident.
Employee was stuck on Livingston Elevator at 1:00 AM the elevator contractor was called and the employee was able to exit the elevator at 1:50 AM. No persons were injured or equipment was damaged as a result of this entrapment.
Failure in crosscut and partial failure of intersection at 90 x-cut off old 33 Mains track at 88 sump. There were no persons injured or equipment damaged as a result of this roof fall.
Elevator at Livingston Portal stopped entrapping two individuals from 2:45 PM to 4:25 PM when they were able to exit the elevator.
Ice that was being chipped from shaft wall fell damaging the wiring for the over travel and slow down switches.
Elevator at Livingston Portal stopped while ascending entrapping an individual from 8:18 PM until 9:05 PM.
Employee got finger caught between frame of scoop bucket and hydraulic ram in scoop bucket, resulted in fractured distal phalanx right middle finger.
Employee was bolting on the left side of the miner, placed his left hand on top of rib protector to help push it out against the rib, rock fell from the roof pinching his fingers between the rock and rib protector. Resulted in 5 sutures in palm side of left index finger.
Elevator at Somerset Portal stopped while ascending entrapping an individual from 1:50 PM until 2:50 PM
Elevator at Somerset Portal damaged, two wires shorted in a relay for the dynamic brake causing the elevator to stop while ascending.
Employee was removing safety chains from between supply cars on slope bottom when he slipped striking his right jaw on something (undetermined) resulting in fracture to right jaw. Incident was not reported until 10-15-2008, began missing work on 10-16-2008.
Employee hit elevator doors with a hand cart while taking equipment off of elevator, knocked the cage door off track while at surface landing, causing the elevator to become inoperable.
A roof fall was discovered in the left wing, intake side of Zedicker shaft. The fall was above the normal anchorage zone. There were no persons injured or equipment damaged as a result of the roof fall.
Employee was diagnosed with Carpal Tunnel Syndrome on 8-25-08, employee started missing work on 9-5-08 due to his condition.
Employee was removing a modulating valve from rock dust tank car. The valve fell off pinching his left index finger between the valve and a reflector bracket that was mounted on the car. Fractured left index finger.
The Livingston Portal elevator ceased operation at 7:45 AM on it's way up the shaft from the shaft bottom. There were 10 men that were riding the elevator at the time. The elevator was able to be brought to the surface at 8:45 AM and the men were safely removed. There were no injuries as a result of this incident.
A roof fall occurred at #92 Cross-cut along 33 Mains track haulage. This was a complete failure of the intersection. There were no persons injured or equipment damaged in this accident.
Employee was helping to remove 2 joints of concrete pipe while pouring cement for belt pad. One of the pieces fell on his right foot. He started missing work on 7/5/2008 due to pain in right foot.
Employee was getting out of compartment after putting motor on charge. He struck his left ear on a fire suppression spray in the process, which required 5 sutures to close the laceration.
Employee was re-railing a trolley jeep. He raised the jeep with a lifting jack. While retrieving another jack from the vehicle, the set jack kicked out causing the jeep to fall. The top of his left foot became caught under the frame.
On June 2, 2008 employee was lifting one end of steel beam when his artificial left hip came out of socket causing him to fall on his left side injuring his left hip and groin area. He was being evaluated by a doctor until June 20, 2008. On June 20, 2008 he was evaluated and diagnosed as being unable to return to work.
Employee was operating a trolley jeep on main haulage. The pole jumped off wire and struck overcast. As employee reached to get pole, it bounced back striking him in mouth and knocking out front tooth.
Employee was dismantling guard gate of rail car, top half of the gate fell pinching the tip of the middle finger on his right hand.
On 5/21/2008 the employee was pulling cables through stopping and felt pain in left groin area. He has continued to work until 7/9/2008 when he started missing due to pain in groin area.
On 5/13/2008 there was a frictional/methane face ignition in the 10B Corridor, 082MMU mining section. There were no injuries to persons or equipment damaged as a result of this accident.
