EE WAS DRAGGING ROLLER ALONG BELT AND FELT A PAIN IN LT SHOULDER. ORIGINALY REPORTED AS A REPORT ONLY INCIDENT, EE LATER ASKED FOR A DR VISIT. IT WAS FOUND THAT EE HAS A LABRUM TEAR, BONE SPURS AND FLUID IN LT SHOULDER. EE IS BEING SCHEDULED FOR SURGERY AND HAS BEEN TAKEN OFF WORK. EE'S LAST WORKED DAY WAS 11/05/2015.
EE was loading bolts onto the Miner. When EE moved to pick up more bolts, he struck the channel on top of the scoop, causing a laceration on EE's RT arm. This laceration needed 3 sutures to close. In that, we are reporting this as a Medical Treatment (NDL)
EE was lifting a CM cable to hang and felt a pop in his RT shoulder. Initally EE did not feel any pain. After sleeping, when the EE awoke, EE could not move shoulder. EE was removed from work and at this time due to his strained shoulder and has been scheduled an appointment with doctor.
While loading channel on miner, EE was sliding channel forward. The edge of the channel caught and cut through glove. EE received 4 sutures in RT Thumb. This is being reported as a Medical Treatment (NDL).

EE AND A PARTNER WERE CHANGING ROLLERS ON CC2 BELTLINE. THEY HAD A HARD TIME GETTING ROLLER IN. ONCE THE TIGHT SIDE WAS IN, THE NORTH SIDE FELL DOWN AND CAUGHT THE EE'S RT RING FINGER CRUSHING IT FROM THE TIP TO THE FIRST KNUCKLE. THIS CAUSED EE TO LOSE THIS PORTION OF HIS FINGER. EE WAS TAKEN TO RUBY/WVU HOSPITAL. EE WILL PROBABLY OFF FOR UP TO 30 DAYS.
EE WAS LIFTING MINER CABLE INTO HANDLER AND FELT PAIN IN RT SHOULDER. THIS WAS ORIGINALY REPORTED AS A REPORT ONLY INCEDENT. EE ASKED FOR A DR APPOINTMENT AFTER A DAY AND WAS PRESCRIBED PHYSICAL THERAPY. THIS IS NOW BEING REPORTED AS A NDL (MEDICAL TREATMENT).
EE WAS LIFTING MATERIAL OVER HANDRAIL AND LOWERING DOWN WITH ROPE, FELT PAIN IN SHOULDER AND REPORTED AS A REPORT ONLY. OPERATOR WAS JUST INFORMED THAT EE WAS RECEIVING PHYSICAL THERAPY. THIS INCIDENT IS NOW BEING REPORTED AS A MEDICAL TREATMENT (NDL)
While lifting new roller to install in cradle, EE felt pop in elbow. EE was diagnosed with a bicep tendon rupture. He was removed from work by Dr and will have surgery on 4/28/2015. This is being submitted as a lost time accident.
EE WAS PUSHING UP ON A RUN THRU AND FELT A SHARP PAIN IN RT SHOULDER. EE STATED THAT PAIN PROGRESSED THROUGH THE SHIFT. EE REQUESTED TO SEE DR. EE WAS PRESCRIBED PHYSICAL THERAPY. THIS IS BEING SUBMITTED AS A MEDICAL TREATMENT.
EE WAS REMOVING ROPE FROM D-RING ON BARGE HAUL CABLE. THERE WAS TENSION ON THE CABLE AND IT SPRUNG BACK, PULLING WRIST AND LEAVING A SMALL BUMP ON INSIDE OF LT WRIST. EE WAS PRESCRIBED PHYSICAL THERAPY AT A LATER DR. APPOINTMENT. THIS IS BEING SUBMITTED AS A MEDICAL TREATMENT.
Unloading chain falls from truck. One started falling off tailgate, jerking left elbow
EE was helping install a rib bolt. While pushing bolt, EE pinched Rt index finger between a 6'x 6' plate and a "pie pan" causing a laceration. It was determined that EE needed surgery on injured finger. EE had surgery on 4/29/15. EE is now off work for recovery. This is being submitted as a lost time accident.

While mechanic was working to adjust conveyor chain the conveyor side cover plate slipped and caught EE's rt. hand between the cover plate and side box, causing a simple fracture to the tip of ee's rt. middle finger.
EE WAS REMOVING CHAIN FROM A LOWBOY AND FELT PAIN IN RT SHOULDER. EE REPORTED THE INCIDENT AS A REPORT ONLY INCIDENT. EE REQUESTED TO SEE DR. AND HAD APPOINTMENT ON 5/7/2015. DR. PRESCRIBED PHYSICAL THERAPY STARTING 5/8/2015. THIS IS BEING SUBMITTED AS A MEDICAL TREATMENT (NDL). EE HAS NO LOST OR RESTRICTED DAYS. *EE worked till 2/26/16 when he had surgery on shoulder.
EE was in the process of throwing a rock dust bag over his shoulder when he felt a cramp in his chest. Initial report was a "report only". EE then requested to see a doctor. It was found EE suffered a hernia(un-repaired) which has a standard charge of 50 days LTA. EE is working and had no true lost time. Received doctors results on 2/28/2015.
Setting pipe in holder, ee felt pop in Lt wrist & it locked up. EE had previous wrist injury & was on 7/25/14. Was released for work 1/18/15. Report was delayed waiting for MRI. It revealed degenerative changes of the distal ulna, numerous scattered erosive changes & mild marrow edema carpal bones in setting of erosive arthropathy. Submitted again as occupational illness.
EE filed a REPORT ONLY incident on 1/6/2015 for a LT shoulder injury he received when moving one end of a draw bar out of the way. EE felt pain in his LT shoulder as well as his fingers and back. It was brought to the attention of the operator ON 4/8/2015 that EE has received injections to his shoulder for this occurrence. We are now filing this as a MEDICAL TREATMENT.
EE was building a wall and caught lt ring finger between blocks, causing a laceration. EE received six sutures but is not going to miss work.
EE was lifting the broken Bretby out of the cable tray to work on it. While doing this EE felt a pop in his Lt shoulder. After initial visit to hospital, EE was seen by a upper body specialist and was taken off work on 12/18/14 due to rotator cuff issues.
EE got off motor to throw switch. As he grabbed switch to throw it, switch did not throw or move at all. EE has a torn lt bicep and a detached ligament.
EE was pulling on stuck drill steel from rib and felt a pop in his Rt arm. After doctor evaluation, it was found that EE tore his Rt bicep and is scheduled for surgery 10/30/14.

EE was pulling on ram jack that was stuck on base lift saddle. In doing so, EE felt a severe pain in his abdomen. EE continued to work until surgery to repair his hernia. This surgery is scheduled on 11/19/2014, which will be his first day of lost time.
Jacking gondola back on track, EE was holding crib against rib and his co-worker tripped and dropped a crib on EE's left thumb. EE sustained a broken thumb.
As EE was tossing ties to load in his bucket, he felt pain in right shoulder. EE saw the doctor on Monday 11/3/14 while on vacation leave and the doctor took him off work. Lost time will begin on Monday 11/10/14, the first day EE was scheduled back after vacation.
EE was putting crib on a flat car, crib slipped out from EE's hand. Crib caught Rt ring finger between crib and standard. EE went lost time on 9/2
Picking up scrap grating, piece fell striking left leg.
Employee was changing an impact cradle. While changing cradle it fell on his right hand. Employee had a laceration to the Rt thumb. Employee received 11 sutures.
On #9 bottom EE was unloading shearer tow bar from battery cart into sleeper. EE heard and felt a pop in his LT elbow. EE was in discomfort and could not extend his arm at this time.
EE was unloading bags tools when one caught on sander & EE felt pain in his lower back. EE requested on 7/7/14 to see Dr. EE was diagnosed with lower back strain. EE had no lost time. This is being reported as a medical treatment at this time due to EE appt. and diagnosis on 7/8/14

Employee, shoveling belt and cleaning walkway, felt pain in lower back. EE continued working regular shift. Follow up therapy prescribed by treating physician. EE requested at a later date to see Dr. EE was diagnosed with lower back strain. EE had no lost time. This is being reported as a medical treatment at this time due to EE appt. and diagnosis on 7/3/14.
EE was lifting 2 miner cables & water hose in cable handler when he felt a pull in his abdomen. EE worked through 5-6-14. On 5-7 the EE had a ventral hernia repair done.
Employee installing cable bolt. Coupler on bolt caught roof, bending bolt. Pinched right little finger. Received 5 stitches.
While pulling muck hose from right side to left side, injured turned and struck head against roof bolt plate.
While tightening T Bolt on Victaulic coupler the T Head spun and struck employee's middle finger on right hand.
