The employee stated that he bent over to lift the handle of the tracking and communication cart and when he felt pain in his lower back. On 1-15-14 this incident was deemed a medical reportable because the employee received medical attention in the form of an injection and diagnosed with a sprain.

Showing 50 of 120EE was setting up to sheave belt to lap in preparation for a belt move. During this process, he was pulling the sheave cable up the entry beside a parked scoop. As he pulled the cable, he twisted & struck his left elbow on the parked scoop's canopy. The EE sought medical attention & was given antibiotics for infection because he had struck the bursitis sack in his elbow.
The employee stated that while he was in the process of dropping miner cable and water line that was hung from the mine roof by a rope hanger. The hanger hook caught between his fingers as he tossed the cable to the mine floor. The hook contacted the webbing between his index and middle finger causing a laceration The employee sought medical attention and received 6 sutures.
The employee stated he was attempting to dislodge a rock that was fouled in the chain of a shuttle car, and while pushing the rock over the bumper of the car the rock cantilevered upwards striking his right index finger between the rock and mine roof. This resulted in an avulsion to the tip of the employees right index finger that required 11 sutures.
The employee stated he was lifting muddy line curtain and felt pain in his left shoulder. The employee sought medical attention and received physical therapy, but did not miss any work. After further consideration, this is being filed as a medical reportable.
The employee stated that he and two other employees were lifting miner cable to place in hangers outby the face of #5 entry when he felt pain in his back. The employee sought medical attention and was diagnosed with arthritis and returned to work. After further consideration and because the injury was compensable, this is being reported as a lost time injury.
The employee stated he was bolting in #7 entry and had been experiencing back pain for a week after bending a bolt. The employee stated he was being treated for a back injury through a previous employer at the time of the injury. After further consideration and because the insurer found the injury compensable, this is being reported as a lost time injury.
The employee stated, while traveling to the section a roll of plastic mesh entered the cab of the mantrip and struck him in the back of the head. The employee suffered a cervical strain and contusion.
Employee was installing a set of doors with 2 other employees at the bottom of the slope. 1 door was in place & they were working on the 2nd door. Door was blocked in place & 2 EEs raised 1 side causing the door to creel pinching employee's finger. The accident resulted in a laceration requiring sutures. He returned to work his next shift. Further investigation is ongoing.
The employee stated that he was helping move a roof bolter from #3 entry to #2 entry when the cable reel engaged and caused the RBM cable to strike him in the back of his head. Employee started missing work on 4-20-12.

Injured was sitting beside bolter when roof straps slipped off bolter striking left hand.
Employee using pitot tube to measure air in a mine fan. The pitot tube got too far into the fan, the employee lost his grip on it and the pitot tube whipped around and smacked the employee's hand.
While working on overcast, pulled something in back

Employee was changing shut off value at tail piece as he tighten up value nipple blew off and hit employee in the right knee.
Injured was unloading mtl (angle iron) when angle iron slipped catching finger between angle iron and side board of supply car.

He was building a stopping on belt line. While stacking the block, the metal band that held the block together was already broken and folded back behind the block. He reached for a block and the band came back over the block. He swung his arm into the band, making a small cut on left arm.
Injured was carrying two boxes of resin when he felt pain in mid-section.
(Note: information received 8/7/08). Rib rolled off on top of operator's canopy. He flagged the injured employee around and asked him to swing the canopy in so the rock will fall off. When he did this, the rock started breaking and rolled off and onto his left foot.
